Management of pulmonary atresia with intact ventricular septum.

Infants with pulmonary atresia and intact ventricular septum pose a difficult clinical problem. Pulmonary valvotomy has been widely recommended for relief of the right ventricular obstruction, and most infants also have had an aortopulmonary shunt placed to ensure pulmonary blood flow. We have evolved a different approach that includes placement of a right ventricular outflow tract patch initially and continuation of prostaglandin E1 infusion postoperatively until the need for a shunt can be determined. We report here our management of 15 neonates with this diagnosis and suprasystemic right ventricular pressures. All of the infants were placed on a regimen of prostaglandin E1 before the operation to improve pulmonary blood flow, and all had an outflow patch placed early in life. Satisfactory postoperative right ventricular function, which would allow both outflow patching and ductus ligation, could be confidently predicted in only two of the 15 patients. For nine of the 15 an outflow patch was placed and prostaglandin was infused postoperatively to provide pulmonary blood flow until right ventricular function became adequate. Early in the series, three other infants were judged to need an aortopulmonary shunt in addition to decompression by an outflow patch, and one infant had only a shunt placed. Postoperatively, adequate pulmonary blood flow was present in all, and 11 of the 15 (73%) survived. Three of the deaths (average 2.8 days) after the outflow patch operation probably resulted from premature cessation of the prostaglandin infusion. One neonate with an outflow patch and a shunt died of myocardial ischemia because of coronary artery steal through right ventricular sinusoids. One late death occurred suddenly in the child with only a shunt, presumably because of an arrhythmia. The remaining survivors (10/15, 67%) are alive and have had complete repair. Study of these patients has also revealed that the definition of adequate right ventricular size needs to be more liberal. Five of the 10 surviving patients had a residual atrial septal defect with a right-to-left shunt at the ages of 1 to 3 years, but balloon occlusion of the atrial septal defect during cardiac catheterization revealed that the right ventricle in these patients was functionally adequate. These five children subsequently underwent closure of the atrial septal defect, and in two the aortopulmonary shunt was also taken down. In summary, correction was achieved in all survivors, in contrast to reported studies in which many patients are living with shunts.(ABSTRACT TRUNCATED AT 400 WORDS)

Familial complex autosomal translocations involving chromosomes 7, 8, and 9 exhibiting male and female transmission with segregation and recombination.

A family showing a complex translocation between chromosomes 7, 8, and 9 with breakpoints at 7q21, 7q33, 8p23, and 9p23 is described. The proband had been referred because of repeated spontaneous abortions. This is only the second family to be ascertained in this way. Twenty-three other cases of complex translocations are briefly reviewed, eight of which were de novo in origin and 15 familial. All but one of the familial cases showed maternal transmission only. The present family shows both maternal and paternal transmission and is thought to be the first exhibiting recombination from a male carrier. The origin and transmission of the complex translocation is followed through three generations.

Renal vascular complications associated with the percutaneous removal of renal calculi.

Significant immediate and delayed vascular complications occurred in 4 of 140 patients (3 per cent) undergoing percutaneous removal of renal and ureteral calculi. An understanding of renovascular anatomy, use of a safety guide wire and intraoperative availability of an angiographic balloon catheter may help to prevent and to treat bleeding problems.

Percutaneous intrarenal electrosurgery.

A 16F nephroscope with a 5F cutting element was used for percutaneous removal of kidney stones in 10 patients in whom an open operation otherwise would have been necessary. Stone removal was accomplished via percutaneous incision of narrowed infundibula, caliceal diverticula and the ureteropelvic junction. A thorough knowledge of renal vasculature, meticulous care and fluoroscopic control are necessary for successful percutaneous intrarenal electrosurgery.

Dual stiffness Malecot (Stamey) catheter.

A new catheter with a soft plastic shaft and a stiff mushroom has been developed for drainage of various fluid-filled cavities. It can be introduced with a flexible guidewire system. Its dual stiffness provides greater comfort for and acceptance by the patient.

Peripheral nerve allograft: an immunological assessment of pretreatment methods.

An in vivo immunological assay was used to determine quantitatively the nerve allograft response in a rat model. The nerve allograft was then pretreated with a number of pretreatment methods that have been used clinically and experimentally. Using the same immunological assay to determine quantitatively the nerve allograft response, we found that lyophilization and high doses of irradiation could modify the nerve allograft in such a way as to make it less immunogenetic to the host animal. The other methods of pretreatment did not alter the antigenicity of the allograft.

Isolation of Yersinia enterocolitica from pigs in the United Kingdom.

A survey of the incidence of Yersinia enterocolitica from pigs in the United Kingdom was carried out by two laboratories using pre-enrichment culture methods on colon/caecal contents, faeces and tonsil tissue. Altogether 1931 specimens were cultured and 112 strains of Y enterocolitica were isolated. Only two strains were related to biotype 4, human serotype O:3 and none to biotype 2, human serotype O:9. All other isolates belonged to biotype 1. Many serotypes were identified, the predominant ones being O:6 (30) and O:5 (15) but 41 strains could not be serotyped. Possible reasons for the low incidence of serotype O:3 are discussed.

An ecological assessment inventory to facilitate community recreation participation by severely disabled individuals.

Severely disabled individuals have long been systematically excluded from actively participating in normalized recreation/leisure activities in integrated community settings. Severely disabled individuals should be taught functional and age appropriate skills, based upon the performance characteristics of nonhandicapped peers. In order to present this position, typical assumptions of leisure skill instruction for severely disabled individuals are discussed and opposing points of view are presented. Finally, the article presents a concrete strategy, or inventory, that is divided into three interrelated areas including: skill selection and skill/facility description, component skills and adaptations for full/partial participation, and supportive skills. The inventory can be used by therapeutic recreation specialists and other educators to develop functional, age appropriate leisure skill instructional content. It is expected that this approach, coupled with longitudinal planning, will facilitate the provision of opportunities for severely disabled individuals to actively participate in normalized recreation/leisure skills in integrated community settings.
