Inherited C3 deficiency with recurrent infections and glomerulonephritis.

A 10-year-old Laotian boy had homozygous deficiency of the third component of complement and recurrent bacterial infections beginning at age 5 months. Cellular and humoral immunity were normal, as were polymorphonuclear leukocyte chemotaxis and bactericidal activities. Serum complement-mediated hemolytic, chemotactic, and opsonic activities were deficient. In vitro addition of purified C3 to patient serum restored hemolytic complement to normal levels, and plasma infusion during each of four episodes of pneumonia significantly enhanced serum opsonic activity for as long as 36 hours. A renal biopsy specimen revealed mesangiopathic glomerulonephritis, although significant levels of circulating IgG immune complexes were not detected. These findings further support the association of C3 deficiency with immune-complex disease and suggest that plasma infusion may be an adjunct to antibiotic therapy in the management of severe pyogenic infections in patients with C3 deficiency.

Mixed-pattern immune deposit glomerulonephritis in a child with inherited deficiency of the third component of complement.

The renal histopathology of a 7-year-old Laotian male with inherited deficiency of the third component of complement, recurrent infections, and persistent hematuria and proteinuria is described. The histologic changes are predominantly those of mesangiopathic disease with isolated changes resembling type I membranoproliferative glomerulonephritis and transmembranous glomerulonephritis. IgG, IgA, IgM, C4, and fibrinogen, but not C3, were detected by immunofluorescence in mesangial zones and in segments of capillary walls. A normal distribution of C3b receptors was present along all capillary walls. This report provides additional support for the association of congenital C3 deficiency and immune deposit glomerulonephritis.

Myocardial beta-receptors in experimental renal and aortic coarctation hypertensive rabbits.

We investigated the effect of inducing two-kidney, one clip (2K, 1C) and aortic coarctation (AC) hypertension in rabbits, on myocardial beta-adrenoreceptor number and affinity, using 3H-dihydroalprenolol as a beta-ligand. Six weeks after surgery, mean arterial blood pressures were: sham-operated controls C, 75.6 +/- 2.3 mmHg (n = 11); 2K-1C 96.3 +/- 2.5 mmHg (n = 6, P less than 0.005); and AC, 114.1 +/- 4.8 mmHg (n = 10, P less than 0.0005). Bmax values at six weeks for C, 2K,-1C and AC were 234.5 +/- 35.7, 139.0 +/- 10.3 (P less than 0.05) and 121.0 +/- 12.8 (P less than 0.005) fmol/mg protein respectively. There were no differences in KD. Both 2K, 1C and AC hypertension significantly decreased myocardial beta-receptor density; this may be a 'down-regulatory' response to increased circulating or myocardial catecholamine concentrations or to enhanced sympathetic activity.

Partial lipodystrophy, C3 nephritic factor and clinically inapparent mesangiocapillary glomerulonephritis.

A case of partial lipodystrophy with C3 nephritic factor was found to be associated with mesangiocapillary glomerulonephritis although all clinical parameters of renal function were normal. Diagnosis of mesangiocapillary glomerulonephritis required renal biopsy. Nephriti factor obtained from this patient was immunochemically related to nephritic factor isolated from the serum of patients with typical mesangiocapillary glomerulonephritis without partial lipodystrophy.

Effect of sodium intake on gentamicin nephrotoxicity in the rat.

Gentamicin nephrotoxicity was examined in rats on normal, high, and low sodium diets. Low sodium diet markedly potentiated nephrotoxic effects of the drug as evidenced by animal mortality, renal failure, pathological changes, and increased renal cortical concentration of the drug. High sodium intake reduced the cortical concentration of gentamicin but renal function and ultrastructure were similar to normally fed rats given in the same dose.
