Pharmacology of a new non-steroidal, anti-inflammatory agent: 6,11-dihydro-11-oxodibenz [b, e] oxepin-2-acetic acid (HP 549).

HP 549 is an orally effective non-steroidal anti-inflammatory agent with moderate analgesic and antipyretic activity. It is active in adjuvant-induced polyarthritis when given prophylactically or therapeutically. HP 549 also inhibits carrageenan-induced paw edema in the rat, an activity which is not altered by adrenalectomy. The analgesic activity of HP 549 was demonstrated in phenylquinone writhing. However, HP 549 produced variable results in the Randall-Selitto analgesia test. The anti-pyretic activity of HP 549 appears to be weak. HP 549, unlike other pharmacologically active anti-inflammatory drugs, does not produce gastric irritation at effective doses and is 45 times less ulcerogenic than indomethacin. Also the acute therapeutic indices for HP 549 are more favorable than for indomethacin.

Synthesis of spiro[isobenzofuran-1(3H),4'-piperidines] as potential central nervous system agents.

Synthesis of 1'-methyl-3-phenylspiro[isobenzofuran-1(3H),4'-piperidine] (7a, HP 365) and the demethyl analogue 9a (HP 505) was prompted by recognition of an aminoalkyl(aryl)isobenzofuran moiety common to the antidepressants talopram (Lu 3-010) and trans-10,11-dihydro-5,10-epoxy-5-[3-(methylamino)propyl]-5H-dibenzo[a,d]cyclohepten-11-ol (MK-940). Convenient laboratory synthesis of 7a was provided by lithiation of 2-bromobenzhydryl methyl ether, followed by addition of 1-methyl-4-piperidone and acid-catalyzed cyclization. N-Dealkylation by standard methods afforded 9a. Synthesis of analogues was stimulated by discovery of marked inhibition of tetrabenazine-induced ptosis for lead compounds 7a and 9a. Optimal antitetrabenazine activity is associated with the 3-phenylspiro-[isobenzofuran-1(3H),4'-piperidine] moiety where nitrogen is basic. Modification of this moiety by introduction of large nitrogen substituents or a C-3 substituent greater than H significantly reduced antitetrabenazine activity. A series of analogues with aromatic substituents was investigated; however, few of these compounds were significantly more active than 7a and 9a. Compound 9a was selected for additional studies.

Corticoid uptake by the paraventricular nucleus in the hypothalamus of the duck, Anas platyrhynchos.

3H-corticoids were localized by autoradiography in small neurons in the area of the magnocellular paraventricular nucleus of mallard ducks. Correlative data show that: (1) the label is principally unmetabolized steroid, (2) the hypothalamus competitively binds corticosterone, (3) the paraventricular nucleus contains immunoreactive neurophysin, is richly innervated by boutons of monoaminergic nerves and is involved in the adaptive response to osmotic stress.

Bone growth kinetics IV: a preliminary investigation on a biorhythm in human osteogenesis.

Multiple doses of oxytetracycline were administered at regular but different intervals to groups of patients undergoin orthopaedic surgery. It was found that doses of the drug were incorporated at bone growth sites most frequently if the intervals between those doses were 3 or 6 days. This suggests a biorhythm in human bone growth with a periodicity of 3 days: this biorhythm may be altered by hormones or drugs.

A study of tobacco carcinogenesis. XLII. Bioassay in A/J mice of some structural analogues of tobacco-specific nitrosamines.

The tumorigenic activities in A/J mouse lung of the tobacco-specific nitrosamines, N'-nitrosonornicotine (NNN), 4-(methyl-nitrosamino)-1-(3-pyridyl)-1-butanone (NNK) and several structural analogues were evaluated. The analogues were N-nitrosopyrrolidine (NPYR), 5'-carboxy-N'-nitrosonornicotine (CNNN), N-nitrosoproline (NPRO) and 1-(3-pyridyl)-2-buten-1-one (PBO). The results were as follows (dose in mumol per mouse/lung tumors per mouse): NNN (100/1.8 +/- 1.4); NPYR (100/3.9 +/- 1.5); CNNN (200/0.3 +/- 0.5); CNNN (100/0.5 +/- 0.6); NPRO (100/0.6 +/- 0.7); NNK (20/7.2 +/- 3.4); PBO (20/0.7 +/- 1.0); saline control (0.0.5 +/- 0.7). Several conclusions were drawn from this assay. NNK and NPYR were more tumorigenic than NNN. CNNN was non-tumorigenic and thus appears to have potential as a monitor for endogenous formation of tobacco-specific nitrosamines. The alpha,beta-unsaturated ketone PBO does not appear to be an ultimate tumorigen of NNK or NNN.

Local and national uses of a Road to Recovery evaluation.

Evaluation fellows from the George Washington University School of Public Health and Health Services conducted an evaluation of the Road to Recovery program of the Mid-Atlantic division of the American Cancer Society. The evaluation included qualitative analysis of program operation, mailed surveys, in-depth interviews with patients and drivers, and interviews with social workers from treatment centers. Results indicated that patients and drivers were satisfied with the program. Patients appreciated the ability of drivers to provide personalized, reliable service. The recruitment of sufficient drivers to meet transportation demand was a problem. High staff turnover and a lack of electronic tracking of standard information hindered program monitoring. A Mid-Atlantic Advisory Transportation Group reviewed the findings and made recommendations for service improvement. The Mid-Atlantic division evaluation contributed to an "evaluation synthesis" in which participants from the three divisions that had conducted Road to Recovery evaluations examined study data and made recommendations for reorganizing the national transportation program. A Transportation Program Design Team then held fact-finding meetings and adopted goals and objectives for a new national transportation program. The primary lesson learned was the far-reaching effects that a single program evaluation may have for various stakeholders and for an organization.

Selenium in particulates and gaseous fractions of smoke from cigarettes prepared from tobacco grown on fly-ash-amended soil.

Cigarettes prepared from tobacco grown in pots of soils amended with soft coal fly ash were smoked, and the mainstream and gaseous fractions were analyzed for total selenium concentration. Fly-ash-grown and control (soil-grown) tobaccos contained, respectively, 0.79 and 0.03 ppm selenium. The quantities of selenium (ng per cigarette) found in the mainstream particulate and gaseous fractions were, respectively, 62.4 and 246.0 for the fly-ash-grown and 8.6 and 12.0 for the control treatments. Studies of the absorption, retention, effects, metabolism, and excretion of selenium in the body are reviewed.

Routine myocardial revascularization with the radial artery: a multicenter experience.

BACKGROUND: Current literature documents use of the radial artery (RA) for myocardial revascularization only as an alternative conduit in cases where the saphenous veins have been previously harvested or are unsuitable for use. Large-scale routine clinical use of the RA as the conduit of choice has not been reported. METHODS: This prospective study evaluated the harvest of the RA from 933 patients and the subsequent use of the conduit as a preferred coronary artery bypass graft second only to the left internal thoracic artery in 930 of these patients. RESULTS: Unilateral RA harvest was performed in 786 patients and 147 patients had bilateral RA harvest. A total of 1080 RAs were harvested; 214 (19.8%) originated from the dominant forearm. There was a mean of 3.30+/-0.93 grafts per patient of which 2.43+/-0.83 were arterial grafts. The mean number of RA grafts was 1.43+/-0.53. Operative mortality was 2.3% with none due to the RA graft(s). There was no ischemia nor motor dysfunction in the operated hands. Thirty-two (3.4%) patients experienced transient thenar dysesthesia that resolved in 1 day to 6 weeks. CONCLUSIONS: Our results demonstrate that routine total or near total arterial myocardial revascularization may be achieved safely and effectively with the use of one or both RAs in conjunction with the internal thoracic artery.

Breast-feeding and optimal visual development.

The goal of the present study was to determine whether dietary supply of omega-3 essential fatty acid (EFA) influences visual development in healthy pre-term and full-term infants. Visual status was examined in human milk-fed infants (ample dietary omega-3 EFA supply) and corn oil-based formula-fed infants (no dietary omega-3 EFA; standard formula prior to 1987). At 57 weeks postconception (4 months adjusted age), both pre-term and full-term human milk-fed infants had significantly better visual evoked potential (VEP) and forced-choice preferential-looking (FPL) acuity than formula-fed infants. Acuity was correlated with a dietary omega-3 sufficiency index from red blood cell membranes obtained at 57 weeks postconception. At 36 months, full-term human milk-fed children had significantly better random dot stereo acuity and letter matching ability than formula-fed children. Stereo acuity and performance on the letter matching test were correlated with a dietary omega-3 sufficiency index from red blood cell membranes obtained at 4 months. These results suggest that dietary omega-3 fatty acids play an important role in visual development.

Evaluation of a stented polyurethane mitral valve prosthesis.

In the search for the perfect valve prosthesis, polyurethane has been used for its excellent physical properties, but thrombosis and embolism have been real problems. The authors investigated the performance and durability of a 23 mm vacuum formed polyurethane Tricuspid Semilunar Valve, in the mitral position. The valve was implanted in eight sheep using standard cardiopulmonary bypass and intermittent aortic cross clamping. Two early failures were due to valve tears. After modification of the manufacturing process, six further valves were implanted in six sheep. All were anticoagulated with warfarin. The valves were calcified on echocardiography and had increasing transvalvular gradients beginning 3 months postoperatively. All six survived and were killed 1-2.5 years after implantation. Despite gross calcification of the leaflets, medium-term function was good. Careful serial sections of brain showed no evidence of thromboembolism. Both kidneys contained small calcium emboli, but no evidence of infarction. Despite calcification, with good medium-term function and no cerebral emboli, this valve may find application in temporary cardiac assist devices.

Management of the temporomandibular joint surgical patient.

Comprehensive management of the temporomandibular joint surgical patient necessitates a thorough presurgical evaluation and definitive postsurgical management. The article provides an overview of TMJ surgical techniques, including arthroscopy, which has significantly decreased the need for open joint procedures. Early postsurgical intervention is recommended and specific therapeutic paradigms are outlined. Adjunctive technique to reduce inflammation, muscle guarding, edema, and collagen bonding are provided to facilitate manual joint mobilization and therapeutic exercise. Optimal success depends upon a strong professional relationship between the treating dentist, oral surgeon, and physical therapist.
