Immunomodulation by interleukin-2 receptor targeted therapy.

The interleukin-2 receptor (IL-2R) is expressed on proliferating T-lymphocytes following antigen stimulation. Activated IL-2R bearing lymphocytes accumulate as cellular infiltrates in autoimmune thyroiditis, insulin-dependent diabetes mellitus, rheumatoid arthritis and graft rejection. Affected cells in Hodgkin's disease, hairy cell leukaemia, non-Hodgkin's lymphoma, cutaneous T-cell lymphoma and lymphoid blast crises of chronic myeloid leukaemia also express IL-2R. Anti-IL-2R monoclonal antibodies or chimeric IL-2R toxins provide a way of selective elimination of such cells. These have been used in experimental models of autoimmunity and transplantation with beneficial results, providing a novel way of selective immunosuppression. In open uncontrolled trials, chimeric IL-2R toxin was found to be safe and effective in patients with refractory rheumatoid arthritis, insulin-dependent diabetes mellitus and IL-2R bearing leukaemias and lymphomas.

Mild-to-moderate malnutrition and small intestine of young rhesus monkeys.

The effect of severe malnutrition and protein deficiency on small intestine has been documented, but the literature on the effect of mild-to-moderate protein-energy malnutrition (PEM) on small intestine is scant. Mild-to-moderate PEM is most prevalent in India. Twenty-four young rhesus monkeys weighing 1.5-2.0 kg were divided into two groups, control and experimental. Mild-to-moderate PEM was induced in the experimental group by giving half of the required normal diet providing 2.42 g protein.kg-1.day-1 and 55 kcal.kg-1.day-1. Body weight, serum protein, and D-xylose were measured before starting the experiment, at PEM stage, and after rehabilitation. Experimental monkeys representing group I were killed after a 25-30% reduction in body weight along with control group 1 animals at 12 wk. The rest of the experimental animals were rehabilitated for another 10-12 wk and killed along with their respective controls (control group 2). Brush-border membrane vesicles were prepared from three parts of the small intestine. Viable vesicles were used for the uptake of [U-14C]L-proline. Alkaline phosphatase and enterokinase were also measured. Uptake of L-proline amino acid and the activity of both enzymes were found to be decreased significantly in the PEM group; a D-xylose test was abnormal. All animals recovered after rehabilitation. These results indicate that even mild-to-moderate malnutrition affects the absorptive and digestive capacity of the brush border of the small intestine, which reversed back on rehabilitation.

[Effect of the light regime and electromagnetic fields on mammary carcinogenesis in female rats].

Outbred female rats from the age of 1 month were kept at rooms with 12 h light: 12 h dark (LD), 24 h light (LL) and 24 h dark (DD) regimens and 2 weeks later were exposed to 3 weekly i.v. injections of NMU at the dose of 50 mg/kg and starting in 2 days after first NMU administration-to variable or static magnetic fields (VMF and SMF). Exposure to LL regimen significantly promoted whereas the exposure to DD regimen significantly inhibited NMU-induced mammary carcinogenesis. RIA of serum samples from rats maintained under different light regimens has shown that exposure to LL followed by decrease of melatonin level and increase of prolactin level at all of modifying factors (NMU, VMF, SMF). Surgical light deprivation inhibits NMU-indiced mammary carcinogenesis in female rats. The primary role of pineal function in the modifying effects of light/dark regimens and electromagnetic fields on mammary carcinogenesis are suggested.

Fluoroscopic transbronchial fine needle aspiration for diagnosis of peripheral pulmonary nodules.

Transbronchial fine needle aspiration (TBNA) was performed under fluoroscopic guidance in 21 patients with either solitary (15 patients) or multiple (6 patients) peripherally located nodules. Good aspirate could be obtained in 16 patients (76.1%). Ten were confirmed to be malignant and 2 turned out to be tubercular. Nonspecific benign inflammatory aspirate was obtained in the other four. There were no significant complications. TBNA is thus an effective, safe and quick method for diagnosis of solitary/multiple pulmonary nodules.

Response to steroid therapy in patients of idiopathic pulmonary fibrosis: a retrospective analysis.

Fifty four patients of idiopathic pulmonary fibrosis were evaluated retrospectively to see the effect of prednisolone therapy on the course of the disease. The diagnosis was established based on clinical history, physical findings, radiology, HRCT, spirometry and transbronchial biopsy whenever possible. Each patient received prednisolone 1 mg/kg/day for 6 weeks which was then tapered gradually till a maintenance dose was achieved. Six patients (11.1%) died within 3 months (3 within 1 month) of presentation because of rapid progression of the disease and inspite of addition of intravenous methyl prednisolone. Improvement in symptomatology, particularly cough and dyspnoea, were quite variable. Twenty six (48.1%) reported subjective improvement by 20-80% of the initial symptoms. However, in most cases this improvement was for the initial 3-6 months of therapy. Subsequently either they deteriorated or remained stable. There was no clinical improvement in the remaining 20 patients (improvement in symptoms by 0 to less than 20%). There was no radiological improvement in any case. FVC improved at 3 months only after treatment compared to the baseline values (p < 0.01). However, subsequently there was no significant improvement in the lung function. Of the 37 cases whose initial FVC value could be obtained, 8 patients (21.6%) showed an improvement by more than 20% of their baseline values during their follow up period. Of these, 4 had the improvement by 3 months; 3 by 6 months; and one showed improvement at 1 year and 6 months. Five patients (13.5%) had a decrease in FVC by 10% or more during their follow up. One patient developed sputum positive pulmonary tuberculosis nine months after steroid therapy.
