[Surgery of acute aortic dissection by use of a biological glue. Experimental study (author's transl)].

The improvement in surgical technic in the treatment of dissecting aneurysm of the aorta should permit a reduction in mortality and secondary complications. With this object, an attempt to stick together the two parts of the aneurysm with biological glue, was submitted to an experimental study. The operative protocol included on the one hand, creation of a dissecting aneurysm in the thorax of the dog, and secondly, repair of the latter with biological glue of variable composition. 27 dogs were thus operated on : -- 4 dissections were treated by simple suture (control group); -- in 23 cases, the two parts of the dissecting aneurysm were stuck together with gelatin-resorcin glue which was polymerised either by formaldehyde alone, or by a mixture of glutaraldehyde-glycerinaldehyde or by a mixture formaldehyde-glutaraldehyde. The best results of adhesiveness and tissue tolerance were obtained with the latter mixture.

[Anastomosis between subclavian artery and V. A. at C1-C2 level. Case report and reappraisal of surgical possibilities on the third portion of the vertebral artery (author's transl)].

A reappraisal of surgical possibilities on the third portion of the Vertebral Artery (V.A.) above C2, have been done from an anatomical study on 20 autopsied specimen. A route passing between the Internal Jugular Vein and the Sternocleido-mastoidian muscle allows a rather simple approach of the transverse process of C1. After dividing two muscles taking insertion on this process, 1,5 cm of the V.A. can be exposed. For larger exposure of the artery, the Foramen Transversaris of C1 must be unroofed and the artery dissected in the guttering of the posterior arch of Atlas. This surgical route was used in a case of aneurismal dysplasia at C3 level. An anastomosis between Subclavian Artery and V.A. at C1-C2 level was realized with an autologous saphenous vein graft. The keypoints are the highest possible freeing of the XI nerve and the head position. Rotation and extension move the transverse process and the posterior arch of Atlas superficially and anteriorly.

[Re-operation for myocardial revascularisation. 15 cases (author's transl)].

Among 1110 patients undergoing aorto-coronary bypass procedures, 15 required late re-operation (average interval 28 months) for myocardial revascularisation. This was performed following the recurrence of anginal symptoms, on average 15 months after the first operation. Six resections of localised stenosis of the vein graft and eleven "new" aorto-coronary bypass were performed. One patient died early, 12 hours after surgery, while 2 died 3 and 26 months later. Nine of the 12 survivors had no symptoms 4 to 63 months after the re-operation. Thus in the presence of angina recurring after aorto-coronary bypass, a new follow-up coronary arteriogram should be performed in order to seek lesions which might benefit from further attempted surgical treatment.

[Congenital arteriovenous fistula of the broad ligament. 1 case].

Following a case report of a congenital arterio-venous fistula of the broad ligament, the authors set out the different types of vascular malformation seen in the pelvic region and they attempt to define the place of treatment. The treatment of these lesions involves embolisation of the vascular pedicle and an attempt at surgical excision, alone or one after another.

Adnexa Uteri↗

[Surgical treatment of mitral diseases with left atrial ectasia. New technic].

The authors describe a new technique of "H" plication of the left atrium which reduces the size of the chamber in patients with mitral insufficiency and ectasia of the left atrium. The development of massive artrial thrombosis during the immediate postoperative period results in a mortality of more than 50 percent. It was for this reason that plication of the left atrium was used in association with valve replacement using a pig heterograft in 3 patients with good immediate and mid-term results. Follow-up cineangiography was performed 6 months after surgery in 2 patients.

[Gelatin resorcin formaldehyde glue in vascular surgery (author's transl)].

The use of this biological glue appeared to us to be of particular interest in the surgical treatment of dissecting aneurysms of the aorta. An experimental investigation revealed its hemostatic and adhesive properties, and demonstrated its merit when used as material in vascular embolization.

Aortic Dissection↗

[Surgical treatment of acute dissection of the ascending aorta].

The authors present 20 cases of acute dissection of the aorta operated on between 1967 and 1974, and 8 more recent cases operated on in 1975-1976. The various factors in diagnosis and surgical treatment are discussed. The results that 60% of patients survive to leave hospital, and the long-term survival (with a mean follow-up of 3 years) is 45%. The main determining factor is survival time appears to be the age of the patient. The authors concluded by advocating operation for all cases of acute dissection of the aorta in patients under 45. Conversely, operation in not advised in patients over 60 years of age.

Acute Disease↗

[Emergency myocardial revascularization with assisted circulation for early extension of infarction].

In order to reduce the oxygen consumption of the myocardium and preserve the areas around the infarction, still alive but undergoing ischemia, 8 patients with early extension of their infarction were placed under circulatory assistance by intra-aortic counter-pulsation. In 8 patients, the pain disappeared and did not recur, permitting left ventriculography and coronary arteriogrpahy. This examination is often considered high risk, but in no patient in our series, during the acute phase of myocardial infarction, were there any complications. 6 patients underwent operation, and aortic counter-pulsation was used during the post-operative period. In all, eight coronary by-pass operations were carried out and, in one case, part of the ventricular wall was resected. All patients are still alive, none have heart failure or residual angina; the follow-up period is now 2 years for the first case.

Assisted Circulation↗

[Dissecting aneurysm rupturing into the pericardium in a 10-year-old child. Temporary surgical success].

Report of the case of a child in whom a marked dilatation of the ascending aorta was discovered at the age of 7 years, because of a symptomless systolic murmur, and who developed at the age of 10 a clinical picture of dissecting aneurysm. He was operated upon successfully, and was shown to have on operation a stenotic bicuspid aortic valve. There were no signs of fibrous tissue dystrophy. Death occured suddenly at the age of 14.
