Experimental hepatic encephalopathy. Changes of the level of wakefulness in the rat with portacaval shunt.

The level of wakefulness in rats with portacaval shunt has been evaluated by measurement of the reactivity of the reticulocortical system by electric stimulation of the brain stem reticular formation during slow wave sleep. The voltage level of stimulus necessary to provoke cortical awakening is significantly decreased after portacaval shunt. This modification of the level of wakefulness is in accordance with the diminution of total sleep and the decrease in threshold of reaction to painful stimuli observed in rats after portacaval shunt. The measurement of the reactivity of the reticulocortical system is a simple procedure which permits quantification of post-shunt encephalopathy in the rat.

Electrocorticographic distrubances after portacaval shunt in the rat: a quantitative evaluation.

Electrocorticographic recordings were analyzed in two groups of Wistar rats with end-to-side portacaval shunt and sham portacaval shunt. The main electrical abnormalities observed in the shunted rats concerned slow wave sleep. Quantitative evaluation of these disturbances was made by measuring the duration of spindles normally present in this phase of sleep. Longer duration of the spindles appears to be a reliable index of electrocorticographic disorders after portacaval shunt in the rat.

Early recovery of albumin synthesis after liver transplantation.

The delay necessary for an orthotopic transplanted liver to recover in normal activity has never been defined. This is of great importance whenever liver transplantation is considered in patients with severe liver insufficiency. This delay was studied in dogs using albumin synthesis and BSP clearance as tests of liver function. Albumin synthesis was normal as early as 2 h following graft revascularization while BSP clearance was not yet normal by this time. It is suggested that transplanted livers will be rapidly able to corect metabolic disorders and above all coagulation problems in patients with terminal hepatic failure.

[Chronic duodenal ulcer. Treatment by anterior fundus seromyotomy with posterior truncal vagotomy].

Anterior seromyotomy of the gastric lesser curvature with additional posterior truncal vagotomy has recently been proposed by Taylor in the elective surgical treatment of chronic duodenal ulcer disease. We report here a technical variant of this procedure. Our results are similar to those following proximal gastric vagotomy, with no operative mortality, minor functional disorders (100% Visick grade I or II at six months), significant reduction of gastric acidity (88.1% reduction of the basal acid output at 6 months) and no ulcer recurrence in the short-term follow-up. This technique offers several advantages over the operations previously designed. It is easy to perform and not time consuming; moreover, the procedure does not take into account possible variations of the vagus nervi, does not require dissection of the abdominal oesophagus and does not entail the risk of lesser curvature necrosis.

[Candidiasis involvement of the small intestine complicated by perforation. A case of favorable course].

We report the case of a 51 year-old man who developed candidiasis of the small bowel associated with intestinal perforation. This condition is very rarely recognized ante mortem. Here, the diagnosis was established by pathological examination of a surgically resected specimen of small bowel at the time of surgery. A complete work-up failed to disclose any predisposing condition to digestive candidiasis. Antifungal therapy resulted in complete recovery.

Management of colorectal cancer in patients with cirrhosis and a LeVeen shunt.

Two cirrhotic patients with a LeVeen shunt presented with a large bowel cancer. In one patient, the shunt was removed and the venous catheter was ligated prior to the performance of a colon resection. The postoperative course was uneventful. A new valve was inserted and connected to the venous catheter two months later. The second patient had a carcinoma of the rectum. In order to prevent ascites and to ease the colorectal resection he had preliminary construction of a portacaval shunt. Six weeks later, he underwent an anterior resection of the rectum. The postoperative course was uneventful except for a self limiting episode of febrile subacute intestinal obstruction. These two cases demonstrate that it is possible to resect colorectal cancer in patients with cirrhosis, ascites and a peritoneovenous shunt provided measures are taken to avoid specific complications due to the presence of the shunt, ascites or portal hypertension.

[Plasma and synovial fibronectin. Results of personal research].

The authors report the results of an assay of plasma fibronectin and synovial fluid fibronectin conducted in 104 patients with arthropathy, including 26 cases of rheumatoid arthritis, 43 cases of mechanical arthropathy and 35 cases of non-rheumatoid arthritis. The plasma fibronectin levels do not vary significantly between the different groups of patients, but the mean value for the whole group of patients is significantly lower than that of a control population. The mean value of synovial fluid fibronectin was significantly higher in the patients with rheumatoid arthritis (583 +/- 76 mg/1) than in the patients with non-rheumatoid arthritis (379 +/- 58 mg/1) or mechanical arthropathy (367 +/- 32 mg/1). The assay of synovial fibronectin could therefore provide useful information for the aetiological diagnosis of inflammatory arthropathies. The local origin, from the synovial membrane itself, of the fibronectin in the synovial fluid is suggested by the fact that the synovial concentration of this protein is superior to the serum concentration in 50 to 90 per cent of cases, depending on the disease.

Dental status and fluoride consumption among five-year-old schoolchildren in Strasbourg, France.

An epidemiological survey of the caries prevalence in 1743 five-year-old children was conducted in Strasbourg in 1991 and compared with a similar study performed in 1973 (Jung et al, 1975). In 1991, 44.2 percent of the children were caries-free and 3 percent of the sample had experienced extractions of carious primary molars. The average values of the dfs and dft indices were 2.93 and 6.10, respectively, with a caries reduction of 35 percent when compared to the 1973 data. The majority of children (87.96 percent) used fluoridated dentifrices and 49.5 percent of them benefited from at least another combined fluoride supplementation. The latter showed lower caries indices than the unsupplemented ones. The average plaque index and gingival index values were 0.62 and 0.1, respectively: 46.4 percent of the children had a plaque index of 0.
