Metabolic studies in postmyocardial infarction patients.

One hundred and seven patients registered in the secondary preventive programme in Prague 4 were followed up for an average period of 3 years. The endpoints of the study were taken as criteria of the progression of clinical atherosclerosis (non-fatal myocardial infarction, cardiovascular death). The authors tried to detect the causes responsible for the high prevalence of impaired glucose tolerance, diabetes and hyperlipoproteinaemia. They analyzed the interrelationships of food intake, insulin secretion, serum fatty acids and lipoprotein phenotypes. There were 6% of non-fatal myocardial reinfarctions and 13% of cardiovascular death after 3 years. Multiple regression analysis identified the following 3 factors closely associated with cardiovascular death: 1) history of myocardial infarction 2) low proportion of linoleic acid in total lipids 3) hyperlipoproteinaemia. The authors show that analysis of the relationships of triglycerides, insulin and serum fatty acids could be of value for identifying the factors involved in a faster progression of atherosclerosis.

Precursors of atherosclerosis in schoolchildren.

In a longitudinal study, the authors examined all the pupils 12-14 years of age in two schools at Kutná (Central Bohemian Region). Serum cholesterol (CH), triglycerides, high-density lipoproteins (HDL), low-density lipoproteins (LDL), blood pressure and pulse rate were determined in 279 children (129 boys and 150 girls). Almost 2% of the children had blood pressure levels of 135/80 mmHg (18/10.6 kPa) or higher. 11% of the boys and 9% of the girls had a total serum cholesterol level higher than 5.17 mmol/l (200 mg %), and in 3% of the children it even exceeded 5.68 mmol/l (220 mg %). Hypertriglyceridaemia (1.4 mmol/l = 100 mg %) was found in 7% of the boys and 6 % of the girls. Mean LDL/CH and LDL/HDL cholesterol ratios were significantly higher in the group of hypercholesterolaemic children than in those with low total cholesterol. The study confirms that abnormal lipids, lipoproteins and blood pressure values may be present already in early life. About one-third of the children of the studied population carry at least one of the factors recognized as predictors of diseases due to atherosclerosis in adults.

Education and the risk factors of ischaemic heart disease.

Screening for the risk factors of ischaemic heart disease among the male residents of the district of Prague 4 aged 40--49 years was undertaken under the "National multifactorial primary preventive study of myocardial infarction and stroke". Of the 11 091 men invited, 5395 met the defined criteria and were admitted to the study. 58% of them were included in the risk group. The level of attained school education was ascertained at the initial examination. The lowest percentage of persons with hypercholesterolaemia, overweight, elevated blood pressure and smokers was found among university graduates. The results are compared with data of other studies and possible causes of the findings are discussed.
