Evaluation of CSF rhinorrhea by computerized tomography with metrizamide.

This study evaluates the ability of metrizamide computerized tomographic cisternography (MCTC) to delineate the site of leakage in patients with cerebrospinal fluid (CSF) rhinorrhea. From 1981 to 1986, thirteen patients were examined by MCTC to localize the site of CSF leakage. A total of 17 studies were performed. Thirteen scans (76%) identified the site of CSF leakage. Nine of these scans were confirmed surgically. The other four scans were performed on patients who refused surgery. Of the 15 scans in patients with active CSF leaks, 13 (87%) were positive. Of the two patients with inactive CSF leaks, neither was positive. One patient is presented in whom MCTC was both diagnostic and therapeutic. In conclusion, MCTC has a high success rate in localizing the site of active CSF leaks.

Posterior chamber intraocular lens implantation in the absence of posterior capsular support.

Because of the high incidence and great variety of complications associated with anterior chamber intraocular lenses, we have developed a technique for the implantation of a posterior chamber intraocular lens in the absence of posterior capsular support. The posterior chamber IOL is placed in the ciliary sulcus by suturing the superior haptic to the iris and the inferior haptic to the sclera at the ciliary sulcus. We have used this technique successfully in both complicated extracapsular surgery and secondary intraocular lens implantation.

Follow-Up Studies

Acute idiopathic blind spot enlargement. A big blind spot syndrome without optic disc edema.

We examined seven patients who had a syndrome of symptomatic monocular blind spot enlargement without optic disc edema. Two patients had previous blind spot enlargement that resolved over several months. The scotoma in each patients was absolute, measured 15 degrees to 20 degrees in diameter, had steep geographic margins, and extended to within 5 degrees to 10 degrees of fixation. Typically, patients had normal visual acuity, color vision, pupillary responses, and ophthalmoscopic findings. Photostress recovery, tested in two patients, was prolonged in the affected eye. Fluorescein angiography showed no abnormalities corresponding to the scotoma. Orbital computed tomographic scans in three patients and visual evoked responses in one patient were normal. Multifocal electroretinography, performed in two patients, showed loss of retinal waveforms in a large region surrounding the optic disc. Our findings suggest that retinal dysfunction produces this big blind spot syndrome, but we do not know its cause.

Autotransplantation of parathyroid tissue into sternocleidomastoid muscle.

Four cases of secondary hyperparathyroidism were treated by total parathyroidectomy with autotransplantation into the sternocleidomastoid muscle. These total parathyroidectomy patients are presented to demonstrate the reliability of parathyroid autotransplantation into the sternocleidomastoid muscle. Our technique is described in detail, and all procedures were successful. In one case, the patient was found, in retrospect, to have an adenoma in the transplanted parathyroid tissue. When the patient developed graft-dependent hypercalcemia, a portion of the graft was easily excised under local anesthesia and the patient became normocalcemic. Parathyroid tissue should be transplanted into the sternocleidomastoid muscle rather than other sites because of easy accessibility, one operative site, less graft ischemia, a low incidence of infection, and a high success rate due to excellent blood supply.

Predictive value of clinical, laboratory, pathologic, and treatment variables in steroid/immunosuppressive resistant lupus nephritis.

Twenty-seven patients with lupus nephritis and nephrotic syndrome had persistent disease activity despite an adequate trial of corticosteroids and immunosuppressive drugs; 30% were Asians, compared with 7% of our overall SLE population. Two years later, seven had a very good outcome and seven a poor outcome. Thirty clinical, pathological, laboratory, and treatment variables were analyzed in a good versus poor responder subset comparison in an effort to determine which factors were associated with favorable outcome. Administration of pulse steroids (P = .069) and a low biopsy chronicity index (P = .048) were associated with the good responder subset. Serum creatinine, biopsy class, blood pressure, complement, and anti-DNA values at entry as well as the choice of immunosuppressive drug were not helpful in predicting outcome. All seven good responders were plasmapheresed (P = .026). Patients with refractory lupus nephritis who have a low biopsy chronicity index may benefit from the use of pulse steroids or plasmapheresis, and controlled studies are suggested.

Effects of a televised smoking cessation intervention among low-income and minority smokers.

In November 1985, a television smoking cessation program was broadcast for 20 days on the noon and 9 p.m. news. Smokers in West Garfield Park, an inner-city impoverished area of Chicago, were randomly assigned either to a comprehensive intervention or to a no-intervention control condition. Although 100,000 self-help manuals had been distributed throughout Chicago, none of the controls in this low-income area had obtained a manual. The intervention consisted of providing the smokers a self-help manual, the televised broadcast, weekly support meetings, and supportive phone calls. At a 4-month follow-up, 20% of treatment participants were abstinent compared to 9% of controls. The results indicated that intensive supplementary opportunities enhanced participation by low-income smokers in a media smoking cessation program.

Detection of cerebral lateralization of function using EEG alpha-contingent visual stimulation II.

The replicative reliability of durations of alpha-blocking following visual stimulation over different cortical sites was assessed with the method of alpha-contingent stimulation. Fourteen right-handed undergraduates were tested in a randomized factorial design. Words or geometric designs of equal luminance were presented by computer to randomly selected visual half-fields contingent on the occurrence of alpha at one of 4 placements (left and right occipital and left and right temporal). The control (mean/s) of alpha-blocking durations was significantly greater for contralateral visual half-field stimulation compared to ipsilateral visual half-field stimulation. This and other results were interpreted in terms of the concept of replicative reliability of retinal-cortical connections to occipital and temporal lobes and affirm the validity and sensitivity of the method.

A systematic approach to analysing sports performance.

A systematic approach to the analysis of sporting events is discussed. The basic tenet is that objective quantification of critical events is required for a complete analysis. This approach encompasses diverse areas of methodology and research, such as quantification, biomechanics and physiology. The theoretical underpinning are behavioural, in so far as athletes produce observable behaviour that can be quantified and ultimately changed. Several areas of analysis are linked together in a general framework and each one of these areas is dealt with in detail.

The real time analysis of sport: an overview.

This paper describes a specific system analytic approach to the coaching and teaching of team sports. Computer technology is used to acquire the sequential history of critical events that occur during competition. A system is described that allows the analyst to enter data directly on a microcomputer and then compute relevant statistics for immediate use by the coach or teacher. The implications for the microcomputer system's use in high performance sports is also discussed.

Relation of free silicone to human breast carcinoma.

Of 12 women with carcinoma of the breast and coexistent silicone mastopathy, nine had had injections of liquid silicone for breast augmentation; three had leaking silicone-gel prostheses. The clinical findings indicated that early diagnosis was obscured by the silicone-induced mastopathy, which rendered the interpretation of physical findings and mammograms difficult. The pathologic findings were suggestive of a possible adverse effect of the presence of free silicone within the breast tissue, axillary nodes, and axillary fat. Although no causal relationship between silicone and breast carcinoma is implied, a heightened awareness of the possible coexistence of silicone mastopathy and breast carcinoma is necessary.

The prevalence of peripheral arterial disease in a defined population.

Because patients with peripheral arterial disease (PAD) may be asymptomatic or may present with atypical symptoms or findings, the true population prevalence of PAD is essentially unknown. We used four highly reliable, sophisticated noninvasive tests (segmental blood pressure, flow velocity by Doppler ultrasound, postocclusive reactive hyperemia, and pulse reappearance half-time) to assess the prevalence of large-vessel PAD and small-vessel PAD in an older (average age 66 years) defined population of 613 men and women. A total of 11.7% of the population had large-vessel PAD on noninvasive testing, and nearly half of those with large-vessel PAD also had small-vessel PAD (5.2%). An additional 16.0% of the population had isolated small-vessel PAD. Large-vessel PAD increased dramatically with age and was slightly more common in men and in subjects with hyperlipidemia. Isolated small-vessel PAD, by contrast, was essentially unrelated to sex, hyperlipidemia, or age, although it was somewhat less common before age 60. Intermittent claudication rates in this population were 2.2% in men and 1.7% in women, and abnormalities in femoral or posterior tibial pulse were present in 20.3% of men and 22.1% of women compared with the noninvasively assessed large-vessel PAD rate of 11.7%. Thus assessment of large-vessel PAD prevalence by intermittent claudication dramatically underestimated the true large-vessel PAD prevalence and assessment by peripheral pulse examination dramatically overestimated the true prevalence.

Risk spreading as an adaptive strategy in iteroparous life histories.

It has long been conjectured, though without satisfactory proof, that life tables with a long reproductive span are advantageous in an environment where fecundity or immature survival rates fluctuate randomly. In the present analysis we recast the nonlinear Leslie matrix problem as an autoregressive time series model for the birth rate, with random addition and removal of newborn. This transformation renders the model linear with respect to the environmental variation, allowing ready solution for the ultimate population size and for the conditions resulting in stationarity of the population distribution. We show that for life tables where the fecundities of all adult age classes are the same (no restrictions are put on the survivorship schedule, or on the age at first reproduction), and where density dependence operates via total adult density, the realized growth rate is less than the growth rate calculated from the mean Leslie matrix associated with the population's growth history. The degree of the discrepancy increases with the environmental variability, and decreases with iteroparity, thus completing a proof which confirms the correctness of the initial conjecture for a class of biologically reasonable life-table models.
