Primary spontaneous pneumothorax in men.

To determine the risk factors, clinical presentation, and long-term outcome of primary spontaneous pneumothorax (PSP), all available cases of PSP in the Israeli Defense Forces (IDF) were retrospectively reviewed. Using the computerized IDF database, all men who had pneumothorax were identified and a telephone interview obtained with 343 individuals, 286 of whom had PSP and agreed to participate in this study. These 286 persons experienced 498 episodes of PSP, with 314 medical records available for review. Average length of follow-up from the first episode of PSP was 107.2 months. Of the study population, 11.5% had a positive family history for PSP. The height-to-weight ratio was significantly and positively correlated with the number of PSP episodes per person, with the regression coefficient being 0.49. The age distribution of those with PSP had a monophasic pattern, with the average age being 25.3 years on first episode. More than one episode of PSP occurred in 41.3% of patients. No ipsilateral recurrences were observed after surgery. Treatment with a sclerosing agent seemed to be more effective in preventing ipsilateral recurrences of PSP than chest tube insertion alone (26.5% vs. 38.3%). Primary spontaneous pneumothorax is an important medical problem, especially in men of military age. It has a significant inherited component in its etiology, with asthenic body build being an additional risk factor. Based on a literature review and the data obtained from this study, it was concluded that chest tube insertion with intrapleural instillation of a sclerosing agent is the preferred nonsurgical invasive treatment modality in PSP.

Immune reconstitution following peripheral blood stem cell transplantation, autologous bone marrow transplantation and allogeneic bone marrow transplantation.

The rate and pattern of recovery of total lymphocytes, T cell subsets, B cells and NK cells were compared for 12 months following recovery phase peripheral blood stem cell (PBSC) autotransplantation (n = 49), autologous (n = 7) and allogeneic BMT (n = 11). The PBSC group had a significantly faster recovery of total lymphocyte count, total T cells (CD3+ cells), CD8 cells and CD4 cells than the allogeneic BMT group. The pattern of earlier recovery of CD8 cells than CD4 cells was the same for each type of transplant. Reconstitution following autologous BMT was intermediate between PBSC and allogeneic BMT. Multivariate analysis identified type of transplant, number of mononuclear cells transplanted and conditioning regimen as significantly influencing immune recovery.

Common allergic skin conditions.

Skin conditions regarded as having an 'allergic' origin are extremely common. Immunologically specific hypersensitivities to food and aeroallergens and idiosyncratic reactions to food chemicals play a role in some of the individuals suffering from these conditions, but in others intrinsic or unknown factors dominate. Rashes are frequently seen in relation to food intolerance and adverse drug reactions that share common mechanisms.

Mumps orchitis among soldiers: frequency, effect on sperm quality, and sperm antibodies.

This study reports on 72 young soldiers who suffered from a recent epidemic of acute mumps, with special emphasis on 19 who suffered from mumps orchitis and whose spermograms were in the fertility range. The immunological work-up for antibodies in sera or seminal plasma gave normal results for the patients diagnosed with orchitis, as well as the controls. The only remarkable finding was an increased risk of borderline significance for orchitis in smokers. The present study has proven that the presence of antisperm antibodies does not play a role in the etiology of mumps orchitis.

Membranous nephropathy with graft-versus-host disease in a bone marrow transplant recipient.

A 43-year-old man developed the nephrotic syndrome 26 months after allogeneic bone-marrow transplantation for chronic myeloid leukemia. This occurred during an exacerbation of graft-versus-host disease (GVHD) and both problems remitted after therapy with cyclosporine and prednisolone. Renal biopsy showed ultrastructural and immunofluorescence evidence of membranous nephropathy. Anti-nuclear antibodies (but not antiglomerular or anti-renal tubular epithelial antibodies) were detected in his serum. Experimental GVHD in mice has been associated with immune complex glomerulonephritis and the presence of IgG autoantibodies which has been attributed to abnormal T (donor)/B (recipient) cell co-operation. This association can be extrapolated to the human GVHD where autoantibody formation is better described than immune complex glomerulonephritis.

A new Streptococcus group A M-29 variant isolated during a suspected common-source epidemic.

In the summer of 1988, a large epidemic of acute pharyngitis occurred in an Israeli military base. The clinical features were those of acute pharyngitis. The epidemic curve was characteristic of a common-source outbreak, possibly food-borne. Throat swabs from a sample of cases were positive for group A streptococci. Nine isolates from the epidemic were further evaluated at the local reference laboratory and serotyping showed that all were of the same strain with a distinct M protein that is a hitherto undescribed variant of M-29. We discuss the significance of unusual strains of beta-hemolytic Streptococcus appearing in food-borne outbreaks.

Interleukin-5, interleukin-3, and granulocyte-macrophage colony-stimulating factor cross-compete for binding to cell surface receptors on human eosinophils.

Human interleukin (IL)-5 receptors were characterized by means of binding studies using bioactive 125I-labeled IL-5. Of purified primary myeloid cells, eosinophils and basophils but not neutrophils or monocytes expressed surface receptors for IL-5. Binding studies showed that eosinophils expressed a single class of high affinity receptors (Ka = 1.2 x 10(10) M-1) with the number of receptors being small (less than 1000 receptors/cell) and varying between individuals. Among several cell lines examined only HL-60 cells showed detectable IL-5 receptors which were small in numbers (200 receptors/cell) and also bound 125I-IL-5 with high affinity. The binding of IL-5 was rapid at 37 degrees C while requiring several hours to reach equilibrium at 4 degrees C. Specificity studies revealed that the two other human eosinophilopoietic cytokines IL-3 and granulocyte-macrophage colony-stimulating factor (GM-CSF) inhibited the binding of 125I-IL-5 to eosinophils. No competition was observed by other eosinophil activating or nonactivating cytokines. The inhibition of 125I-IL-5 binding by IL-3 and GM-CSF was partial up to a concentration of competitor of 10(-7) M with GM-CSF consistently being the stronger competitor. Converse experiments using IL-5 as a competitor revealed that this cytokine inhibited the binding of 125I-IL-3 and of 125I-GM-CSF in some but not all the individuals tested, perhaps reflecting eosinophil heterogeneity in vivo. Cross-linking experiments on HL-60 cells demonstrated two IL-5-containing complexes of Mr 150,000 and Mr 80,000 both of which were inhibited by GM-CSF. The competition between IL-5, IL-3, and GM-CSF on the surface of mature eosinophils may represent a unifying mechanism that may help explain the common biological effects of these three eosinophilopoietic cytokines on eosinophil function. This unique pattern of competition may also be beneficial to the host by preventing excessive eosinophil stimulation.

Pregnancy outcome among native Indians in Saskatchewan.

OBJECTIVE: To determine the difference in pregnancy outcome between native Indians and the provincial population in Saskatchewan. DESIGN: Retrospective analysis of data collected from all birth and death registration forms. SETTING: Saskatchewan. MAIN OUTCOME MEASURES: Incidence of low birth weight and rates of stillbirth and of neonatal and infant death. RESULTS: The neonatal death rate was higher in the Indian population than in the provincial population during the study period; the difference between the two groups in the rate decreased markedly after 1982. The rates of stillbirth and infant death were also higher among the Indians, and the difference persisted during the study period. The incidence of low birth weight and the rate of stillbirth were highest in the youngest and oldest maternal groups in the provincial population; however, the pattern was markedly different among the Indians, teenaged mothers having the best outcomes. CONCLUSION: Further studies are needed to determine the relation between maternal age and fetal outcome among native Indians.

Mumps vaccination of young adults is unwarranted.

A clinical and epidemiological study of mumps was carried out over a 18-month period and included 104 soldiers (61% of the reported cases in the Israel Defence Forces). The average number of days off work was 12.9 +/- 9.2 days. The most common complication was orchitis (19/72 men; 26%), followed by 3 cases of pancreatitis (3%) and 2 cases of meningitis (2%). 13 men with orchitis were tested, and all had normal spermograms. No long term important sequelae were described. The transmission rate was low with no respondent reporting more than one secondary case among soldiers serving in the same unit. The socio-economic level of mumps patients was comparable to that of the general army population at the time of the study. Our data do not support a mass immunization program of the adult population.

Seasonality and long-term trends of pediculosis capitis and pubis in a young adult population.

Little is known about seasonality and long-term trends in the incidence of pediculoses. Such information would help both public health workers and clinicians in the prevention and management of the problem. In the Israel Defence Forces the pediculoses are individually notifiable, and this provides a unique database for the study of this subject. The incidence of head louse infestation increased from 5.2 per 1000 in 1977 to 18.1 per 1000 in 1979, and then declined to 3.6 per 1000 in 1987. The incidence was higher in the warmer months. Pubic louse infestation increased from 7 per 1000 in 1977 to 14.9 per 1000 in 1983, and then declined to 4.6 per 1000 in 1987. The incidence was higher in the cooler months. Possible causes of the seasonality and long-term trends are discussed.

Reciprocal inhibition of binding between interleukin 3 and granulocyte-macrophage colony-stimulating factor to human eosinophils.

125I-labeled recombinant human interleukin 3 (IL-3) bound, at 4 degrees C, to a single class of high-affinity receptors on human eosinophils with an apparent dissociation constant (Kd) of 470 pM, but it did not bind to human neutrophils. 125I-labeled recombinant human granulocyte-macrophage colony-stimulating factor (GM-CSF) also bound to a single class of high-affinity receptors on eosinophils with an apparent Kd of 44 pM and on neutrophils with an apparent Kd of 70 pM. These binding characteristics were consistent with the biological activities of IL-3 and GM-CSF on eosinophils and with the lack of stimulation of neutrophil function by IL-3. Specificity studies under conditions shown to prevent receptor internalization showed that the binding of 125I-labeled IL-3 to eosinophils was partially inhibited by GM-CSF but not by other cytokines. Reciprocal experiments with 125I-labeled GM-CSF showed that IL-3 but not other cytokines partially inhibited binding to eosinophils. In contrast, the binding of 125I-labeled GM-CSF to neutrophils was not inhibited by IL-3 or other cytokines tested. Quantitative inhibition binding experiments on eosinophils showed that the reciprocal inhibition between IL-3 and GM-CSF was not complete up to a concentration of heterologous ligand of 100 nM. These results show that (i) IL-3 binds to eosinophils but not neutrophils and (ii) IL-3 and GM-CSF specifically interact on the surface of eosinophils, providing a possible mechanism for the overlapping activities of IL-3 and GM-CSF on these cells.

Characterization of a p43 human thymocyte antigen.

Two murine monoclonal antibodies (MoAbs), designated WM-25 and WM-35, have been prepared which react with human thymocyte cell surface membrane antigens. Both antigens were found to be expressed by normal and leukaemic immature human thymocytes. However, comparative examination by tissue section staining, immunoprecipitation, and electrophoresis, as well as competitive binding studies, demonstrated clear differences in the antigens detected by these two MoAbs. Antibody WM-35 identified a 48 kilodalton (kD) protein expressed on epidermal Langerhans cells as well as thymocytes, and appears similar to the previously reported Mabs OKT-6 and NA1-34. In contrast, MoAb WM-25 immunoprecipitated a 43 kD antigen, non-covalently associated with a 12 kD light chain, which was not present on Langerhans cells. Competitive binding and sequential immunoprecipitation studies confirmed that the 48 and 43 kD proteins are antigenically distinct. These results support the concept of at least two forms of thymocyte antigens within Cluster of Differentiation One (CD-1).

Frequency and characteristics of lingual thyroid not detected by screening.

Four patients with lingual thyroid glands presenting beyond the neonatal period have been evaluated at the Hospital for Sick Children, Toronto since the advent of neonatal TSH screening. All were female, clinically euthyroid at diagnosis and presented with symptoms of a lingual mass. We estimate that 1.6% of lingual thyroids are missed by this TSH based thyroid screening program and approximately 1/600,000 live births present in childhood or adolescence with a lingual thyroid. Physicians should still include lingual thyroid in the differential diagnosis of a mass at the base of the tongue.

Combined 21-hydroxylase and 11beta-hydroxylase deficiency: patient report and molecular basis.

A female newborn (46, XX) with ambiguous genitalia was initially diagnosed by biochemical criteria as having classical congenital adrenal hyperplasia caused by 11beta-hydroxylase deficiency. Shortly after effective treatment was administered, she developed a salt-wasting crisis with severe electrolyte imbalance. DNA analysis revealed a homozygous splice mutation in the second intron of the CYP21 gene, for which both parents were heterozygous. No mutations were found in the entire CYP11B1 gene, thus proving that the 11beta-hydroxylase deficiency was not caused by a gene mutation but rather was a secondary event, possibly due to androgen suppression of 11beta-hydroxylase activity.

Fenfluramine and mazindol: acute reversible cardiomyopathy associated with their use.

An unusual side effect of amphetamine-like drugs is described. A patient with acute cardiomyopathy was discovered to have been taking Fenfluramine and Mazindol at the prescribed dose. Within a week after abstention from the drugs and appropriate cardiac treatment her cardiomyopathy resolved. The authors suggest that this effect is due to the combination of the two drugs and represents a drug interaction.
