[Study of neurovegetative function in type 2 diabetes mellitus in childhood].

The passive (tilting) orthoclinostatic test was performed on 44 young subjects affected by type 1 insulin-dependent diabetes. In 11 cases the test was repeated after a 3-4 years interval. The results, compared with age matched controls, showed in the diabetic patients a supersensitivity of the two antagonist systems, increasing with age and duration of the disease, up to an overexcitability and a functional exhaustion. It would thus seem that diabetic dysautonomia involves not only the peripheral nervous system but also the hypothalamic centers, with an impoverishment of the capacity of self-regulation. The hypersympathicotonus in diabetic young patients seems to be only apparent, as a result of the hypersensitivity of an overstimulated system.

Immunophenotypic, cytogenetic and molecular investigations in two cases of CALLA positive acute myeloid leukemia.

Clinicopathological and cytogenetic features of two patients with acute myelogenous leukemia (AML) whose blast cells coexpressed myeloid-associated antigens and CALLA are described. Leukemia cells revealed myelomonocytic (FAB-M4) and monocytic (FAB-M5) features, while the nonblast cell population exhibited trilineage myelodysplasia in both cases, a finding suggestive of multiple-cell-lineage involvement. Cytogenetically, a deletion of the long arm of chromosome 6 was found in one patient, and normal metaphases were detected in the other. Molecular studies disclosed a rearrangement of the IgH locus in one patient. Clinically, these patients were unresponsive to antimyeloid regimens including Daunorubicin and Cytarabine, two agents normally also effective on lymphoblastic leukemias, possibly indicating the need for alternative protocols for the treatment of CALLA positive AML.

[Correlations of heart rate and arterial pressure in the orthoclinostatic test. Study of 3500 subjects].

The study deals with the data resulting from the analysis of 3497 passive orthoclinostatic tests performed in the neurological Clinic of Pisa during the last 10 years. Blood pressure and heart rate values both in supine and upright positions have been analyzed, particularly focusing the orthostatic hypotension, which were 184 over 3497 tests. The results show that in young subjects only hypersympatheticotonic hypotension are present, due to a dysfunction of the regulating hypothalamic centres, while at more advanced age there is a prevalence of orthostatic hypotension with low differential values of heart rate. Differential values of heart rate as a function of age have been the calculated.

[Nutrition of the very low birth weight newborn infant (less than 1500 g). Experience with a formula for the premature infant].

After a short review on the topic of nutrition of very low-birth-weight infants, particularly the very small preterms, results are reported on a trial with an adapted milk for prematures. Subjects involved had a gestational age of 28-34 weeks and weighed less than 1500 g thus representing a sample of very low birth weight babies. By using levels of MCT in the milk, a protein quota of 3, 15 g/Kg/day with 40/60 casein whey-protein ratio, an electrolyte content slightly higher than in most adapted formulas (for fullterm babies) and low osmolarity, good results were obtained for body weight growth, length, head circumference and subcutaneous fat. Growth indices were similar to those typical of the terminal months of intrauterine development as well as a satisfactory overall portrayal of biochemical indices of the nutritional status of VLBW babies.

Immunophenotypic and genotypic characterization of B-cell chronic lymphocytic leukemia patients from northern Italy.

BACKGROUND AND METHODS: Immunoglobulin (Ig) gene expression and rearrangements provide information about cell lineage, clonality and differentiation of neoplastic lymphoid cells. We performed flow cytometry and Southern blot analysis in 25 patients with chronic lymphocytic leukemia (CLL) from the Po River delta region (Northern Italy), in order to correlate surface immunological phenotype and heavy and light chain gene rearrangements with clinical stages. RESULTS: In these 25 CLL cases, as in the larger group of 165, k-positive (k+) patients were more frequent than lambda-positive (lambda +) ones by a ratio of 59:41, with a predominance of lambda + in stage II (46:54) and of K+ in stage IV (77:23). However, the survival behavior of lambda + and k+ patients was not statistically different. Ig gene rearrangements were found in all patients in accordance with the immunophenotype. When Ig gene analysis was performed on two different occasions in 7 of the 8 who received chemotherapy, there was an increase in the percentage of the germline band. This variation was inversely related to changes in WBC count. In the treated subject who showed a reduction of both WBC count and the germline, there was rapid disease progression and short survival. CONCLUSIONS: Analysis of Ig gene rearrangements in our CLL patients demonstrated: i) the monoclonality of neoplastic cells; ii) no correlation between rearrangements and clinical stages, and iii) that this analysis may be a useful tool in evaluating biological behavior in selected CLL cases showing a discrepancy between hematological data and clinical response to therapy.
