The zygomaticus implant protocol in the treatment of the severely resorbed maxilla.

The zygomaticus dental implant, designed by NobelBiocare, was developed primarily for the treatment of the severely resorbed maxilla. Brånemark has reported an overall success rate of 97.6% with the placement of over 200 zygomatic implants during the period 1989 to 2001. There are well-defined characteristics within the patient's resorbed skeletal frame which guide the surgical placement of the zygomatic implant, thus determining whether the surgeon should use an implant with a 45 degrees or 55 degrees angulated head. This allows implant-supported restoration of the resorbed maxilla with a fixed cross arch prosthesis in the maxilla without a bone graft to the posterior maxilla. The use of a modified head angulation of 55 degrees, with implant placement as close to the crest of the edentulous ridge as possible, allows restorative clinicians to achieve an ideal restorative position in the posterior maxilla. The use of a zygomatic implant with a 55 degrees head reduces the buccal cantilever by 20%.

Constrained and unconstrained postures of the mandible--a break with tradition?

Evaluation of detailed electrognathographic recordings of various movements of the mandible (and points on it) suggested interpretations at variance with some traditional concepts. Data previously accumulated from several sources were subjected to critical analysis. An objective was to assess the manner in which two rotations of the mandible influenced its postures during trials of mastication and deliberate lateral excursions. Rotations about a vertical and an antero-posterior axis appear to facilitate a greater functional posturing potential of the mandible than has previously been reported. In most instances the influence of the rotations upon mandibular posture is considerably greater than the size of the recorded angles would suggest. Some of the resulting postures seem to challenge concepts derived strictly from cadaverine anatomy. For example, during mastication the condyle does not simply glide forward and backward relative to the eminence but may be distracted downward away from it for a part of the time. Furthermore, the ability of a subject to effect a considerable variety of mandibular postures challenges traditional reliance upon constrained mandibular movements as a means of evaluating function.

Beta-blocker therapy: identification and management of side effects.

The purpose of this study was to develop and test a new beta-Blocker Visual Analog Scale designed to identify and quantify the impact that the side effects of beta-blocker therapy have on people's lives, and the self-management practices people use to mediate their influence. Instruments included the 20-item beta-Blocker Visual Analog Scale and the Profile of Mood States. Subjects had hypertension; 51 men were involved in a larger study involving antihypertensive medications and exercise, and 19 men and women were receiving beta-blocker therapy as first-line drug of choice. Estimates of internal consistency reliability, content validity, and concurrent and discriminant validity were moderately strong. The most problematic side effects were related to lack of sleep, vivid or active dreams, lack of energy and pep, diminished interest in sexual activity, and changes in vision. Among self-management practices used to mediate side effects were planning rest and activity periods, thinking carefully before reacting, and seeking out others for support.

Identifying nursing research priorities: a first step in program development.

This study determined nursing research priorities and focused on the development of a nursing research program within a single institution. Of the 715 nurses within the Department of Nursing, 286 (40%) responded to the first round of a two-round Delphi technique, and 137 (48%) responded to the second. The following clinical priorities were determined: prevention and treatment of pressure sores, pain management, treatment of nosocomial infections, and patient education. Nonclinical priorities included staffing and its effects on turnover and patient care, job satisfaction, and factors influencing the quality of nursing care. Serendipitous findings underscored the relevance of the Department of Nursing's conceptual framework to nursing practice and research, revealed the need for dissemination of current research results, and suggested that nurses want to participate in research without assuming responsibility beyond their expertise.

The effects of implementing a professional practice model.

Professional practice is a reasonable and viable alternative to the traditional nursing practice models used in many hospitals. The authors describe the implementation of a professional practice model and its effects on cost per patient day, sick time, turnover rates, and job satisfaction indicators.

Cardiovascular disease and women.

Cardiovascular disease is a significant problem for women. Coronary artery disease has long been considered a man's disease and women have been excluded as a population at significant risk for cardiovascular disease. Women are poorly informed about cardiovascular disease, which is the greatest risk to their health. Many of the risk factors for heart disease are similar for men and women such as age, family history, cigarette smoking, hypertension, high serum cholesterol levels, and diabetes. Women have unique risk factors which include menopause, oral contraceptive use, and non-contraceptive use of estrogen. Gender differences also exist in the clinical presentation of heart disease. Primary prevention is aimed at smoking cessation, diet modification, and increased physical activity. Health care providers need to understand the risk factors associated with cardiovascular disease in women and the importance of prevention, recognition of symptoms, and timely referral.
