Mediastinal and hilar lymphadenopathy due to Pneumocystis carinii infection in AIDS patients: CT features.

OBJECTIVE: Extrapulmonary involvement by Pneumocystis carinii (PC) in acquired immunodeficiency syndrome (AIDS) patients is uncommon. Our purpose is to describe three AIDS patients with low cluster differentiation cell (CD4) counts, who were receiving aerosol pentamidine prophylaxis, and who had large noncalcified hilar and mediastinal lymphadenopathy. MATERIALS AND METHODS: Chest radiography, CT, and clinical and laboratory records of three AIDS patients who had radiographic evidence of mediastinal and hilar lymphadenopathy were retrospectively reviewed. RESULTS: Symptoms were quite similar in all patients: weight loss, fever, cough, and mild dyspnea. Mediastinal and hilar widening was noticed on chest radiography. Contrast-enhanced chest CT revealed multiple noncalcified lymphadenopathy with supracarinal predominance in all cases. Mediastinoscopy with mediastinal and hilar lymph nodes sampling was performed in all patients. Histologic examination of the biopsy specimens demonstrated a necrotic foamy tissue without any calcification, and silver stain coloration showed abundant PC organisms in each case. CONCLUSION: In AIDS patients, PC infection should be considered as a possible cause of noncalcified hilar and mediastinal lymphadenopathy along with such possibilities as atypical infections and neoplasms, especially in severely immunocompromised hosts (low CD4) with aerosol pentamidine prophylaxis.

MR imaging of the excised human brainstem: a correlative neuroanatomic study.

The purpose of this study was to examine an excised human brainstem with high-resolution MR imaging. The MR images were correlated with myelin-stained histologic specimens and standard anatomic descriptions. Structures visualized included nuclei and/or fibers of all the cranial nerves, with the exception of the trochlear nerve. The topography of the brainstem is discussed. Major tracts such as the corticospinal spinothalamic, spinocerebellar, medial and lateral lemniscus, and other ascending and descending tracts and intrinsic brainstem nuclei such as the olivary complex, pontine nuclei, red nucleus, and inferior and superior colliculi were identified. Functional neuroanatomy of the brainstem is reviewed. This neuroanatomic description and review of the complex MR anatomy of the brainstem should assist in the delineation of brainstem structures on MR imaging.

[Alcoholic pancreatitis and pseudocysts. Prospective study of 20 cases in Réunion Island].

In the french indian ocean's island whose name is Reunion, between 1984 and 1986, 20 pancreatic pseudocysts, exclusively encountered in alcoholic patients, were submitted to a prospective study. Most of the lesion were very symptomatic, large and extrapancreatic. Accordingly to Marseille's classification, most of the patients were suffering from acute or chronic relapsing pancreatitis. 18 patients were submitted to a therapeutic procedure. Internal drainage was performed in 9 patients, and external drainage was performed in 5. In addition, pancreatic resection was carried out in 2 patients, and needle aspiration in 2. Comments concern potential specificities of the population which was studied. Special emphasis concern the necessity of preoperative computerized tomographic scanning, the indications of percutaneous external drainage and the long-term survival of patients.

[Eye injuries due to pellet guns. Apropos of 7 cases].

The authors expose a series of seven cases of ocular trauma due to pellet guns. First, they describe the different kinds of lesions, their treatment, and their development. Then, they study the prognostic points, the mechanism of the secondary complications, and the therapeutic conduct to adopt in front of this type of perforating injuries with non-magnetic intraocular foreign body. They insist on the severity of this type of ocular trauma, and on the necessity of a modification of the law concerning pellet guns.
