The study investigates the hypothesis that obsessional traits may be related to telic dominance (a state in which the individual pursues goals seen by him as essential) while obsessional symptoms may be related to extreme vacillation between telic dominance and its opposite state of paratelic dominance (in which the individual concentrates upon the purely diversionary nature of an activity). Using questionnaire methods and normal subjects, the hypothesis received cautious support.

Although it has been used in a number of small-scale pieces of research in recent years, there exist as yet no adequate standardization data for the Sandler-Hazari Obsessionality Inventory, a device designed to yield scores for both obsessional traits and obsessional symptoms. The present article describes research aimed at contributing towards a partial standardization of the Inventory using 736 subjects spread over six different samples. Results suggest that obsessional traits and symptoms are present in the non-clinical as well as in the clinical population, though the Inventory appears able to discriminate reliably between the two. Results also suggest that there may be a low-level correlation between traits and symptoms in both clinical and non-clinical populations, and that the presence of unscored buffer items in the Inventory may reduce the risk of clinical and non-clinical subjects faking good in their responses to it.
A study on the effects of hyperthermia on B.N.A. induced bladder carcinomas of the dog has been carried out since 1966. The hyperthermic treatment has been performed directly in the bladder using an original equipment especially designed. In a large majority of the animals definite cancericidal changes have been shown. The most favorable results were obtained in the treatment of carcinomas not infiltrating the muscular layer of the bladder. In these cases malignant recurrency was not shown up to five years after treatment. Good palliative effect is also refferred in the treatment of advanced bladder carcinoma of the man.
A series of patients with acute renal failure (ARF) and acute abdomen collected by the St. John the Baptist Hospital's Nephrology and Dialysis Division over the period 1970-75 is examined. A high rate of mortality was noted, due to causes that were mostly independent of ARF Death was often related to failure to resolve acute abdomen. The progression of ARF appeared to be related to that of the abdominal affection with prompt resolution of the latter. These findings suggest that constriction of the afferent arteriole may offer a partial, functional basis for ARF with the corollary that it has a good chance of being reversed, even after protracted periods of anuria. Renal complications associated with disseminated or localised intravascular coagulation appear to be rare. Treatment was best directed to early and frequent dialysis, with surgical resolution whenever indicated, irrespective of the presence of ARF.

We report our experience with bilateral adrenalectomy for treatment of Cushing's syndrome and we compare the outcome of laparoscopy with open surgery in terms of effectiveness and safety. A series of 23 patients underwent bilateral adrenalectomy for treatment of Cushing's syndrome [Cushing's disease in 16, ectopic ACTH syndrome in 2, and ACTH-independent macronodular adrenal hyperplasia (AIMAH) in 5 cases]. From 1993 to 1996, all patients were treated using an open approach (Group A), while from 1997 all patients were treated using a transperitoneal laparoscopic approach (Group B). The comparison between the 2 groups was performed considering patients characteristics, operative times, blood losses, intraoperative and post-operative complications, analgesic consumption, post-operative hospital stay and recovery. Open surgery was performed in 10 patients and laparoscopy in 13 patients. No significant difference was recorded between the two groups as to patients' characteristics and complications. Mean operative time was significantly increased in Group B, while post-operative hospital stay was significantly longer in Group A. Laparoscopic bilateral adrenalectomy can be safely and effectively employed to treat Cushing's syndrome. However, long operatives times may represent a limitation especially in high risk patients.
Changes in blood testosterone, LH and FSH induced by the surgical management of BPH were studied. It is felt that the observation of depressed testosterone and enhanced LH mean levels one year after surgery, substantiates a previously advanced personal view that hyperplastic nodules are heavy consumers of testosterone.
In a prospective trial conducted by the Gruppo Onco Urologico Piemontese, newly diagnosed prostate cancer patients with bone metastases were randomized to receive goserelin (3.6 mg subcutaneously every 4 weeks) or goserelin plus mitomycin at 14 mg/m2 i.v. every 6 weeks. Treatment was planned to be continued until progression. The study was interrupted because of inadequate accrual rate when 63 patients had been recruited. A long-term follow-up (median, 47 months), performed to counterbalance the limited number of patients included, revealed no difference in time to progression and overall survival between the study treatments. However, 56.5% of assessable patients allocated to the chemotherapy arm presented a > or =90% reduction of prostate-specific antigen levels compared with 36.3% in the goserelin group, and previously elevated levels normalized in 73.9% versus 45.4%. Non-progressing patients received 5-7 cycles of mitomycin C with acceptable toxicity, but the cytotoxic treatment was interrupted early in all cases within the first year due to cumulative myelotoxicity. In conclusion, the results, although inconclusive, fail to support a clear advantage in terms of cost/benefit of chemotherapy plus hormone therapy over hormone treatment alone in advanced prostate cancer with bone involvement.

The paper reports the case of a particularly large retroperitoneal liposarcoma (diameter 60 x 28 x 32 cm, weight 11.200 kg) and focuses on the diagnostic and therapeutic aspects of this tumour. The need for radical surgery to remove the entire mass is stressed, even if it is particularly large; likewise, the patient must be carefully monitored since retroperitoneal recurrence is frequent and must be treated, wherever possible, surgically.
A rare complication of positioning of an IUD represented by utero-vesical puncture is reported in a woman of 37 who was using this contraceptive technique for the first time. The IUD was removed endoscopically, about 2 years after its positioning. The observation was an almost chance event by echography performed for vague pains in the suprapubic region.
Gallstone ileus is a rare condition whose diagnosis may be puzzling. A series of 4 cases of gallstone ileus is presented. The last twenty years' literature was reviewed and compared with our results. Enterolithotomy was performed in all patients and no surgery was performed on the gallbladder and/or the fistula. The outcome was favourable in all cases, thus supporting our procedure.
