Carcinosarcomas--a retrospective analysis of 21 patients.

BACKGROUND: Carcinosarcomas (CS) are mixed epithelial and stromal tumours with both components being malignant. Twenty-one patients with carcinosarcomas who were treated at the University of Saarland, Germany, are presented. PATIENTS AND METHODS: The hospital records of all patients were reviewed. RESULTS: The mean age range for homologue CS (homCS) was 60.7 years and for heterologue CS (hetCS) 68.4 years. Post-menopausal bleeding and abdominal pain were the main symptoms. Treatment modalities included surgery, adjuvant radiation therapy and various chemotherapy regimens. The median follow-up time was 17.6 months (homCS) and 31.1 months (hetCS). The 1-year survival rate was 55.6% (hetCS 58.3%) and the 5-year survival rate was 11.1% (hetCS 8.3%). CONCLUSION: Carcinosarcoma is a uterine sarcoma with a poor prognosis. Treatment includes surgery and adjuvant chemotherapy, whereas radiotherapy does not necessarily lead to a benefit. Treatment with trastuzumab might be a new approach in the therapy of HER-2/neu-positive CS.

[AIDS model program of the Federal Government--an interim evaluation].

In 1986 the Government of the Federal Republic of Germany, started--with considerable financial and personal involvement--eight programs for AIDS-Prevention. An interimsreport of the associated research program gives first hints of success and compulsory modifications.

[Sarcoidosis associated with tuberculosis. A study of seven patients (author's transl)].

The authors studied seven patients with sarcoidosis and tuberculosis: the affections co-existed in 3 cases but were separated by varying intervals of time in the 4 other patients. They discuss the diagnostic, therapeutic, and nosological problems posed by these cases, and review the various hypotheses that have been suggested to explain these associations in relation to the pathogenesis of the sarcoidosis.

[The significance of health risks caused by outdoor pollution, demonstrated by cross-sectional studies of pseudocroup in Baden-Württemberg].

In the FRG the question of health effects by ambient (outdoor) air pollution is often considered with respect to respiratory diseases in children, especially group syndrome. Therefore, the results of a cross-sectional study in Baden-Württemberg are discussed as an example for the role of "outdoor pollution" at presently observed concentrations. In the study that was performed in Stuttgart, Tübingen/Reutlingen and Freudenstadt, the parents of 8.420 children at the age of 6 years participated (response rate 93%). Their reports on diseases were additionally validated and completed by the paediatricians concerned. The "crude" prevalence for the age group 0-6 years is about 9% each for croup syndrome and obstructive bronchitis. This is strongly influenced by sex, disposition in the family, education and nationality of the parents. In contrast, influences of ambient air pollution are less clear or missing. Pollution measurements on a sufficient spatial scale were only available for Stuttgart, where logistic regression showed after adjustment for the relevant confounders only that croup syndrome was slightly increased in streets with high traffic load. For childhood asthma (prevalence 1.5%) a correlation with traffic-dependent pollutants (NO2, NO, CO) and traffic load is found. These results are compared with those from North Rhine Westphalia, and their limitations are discussed.

Oncogene expression in vivo by ovarian adenocarcinomas and mixed-mullerian tumors.

Six-micron paraffin sections of paraformaldehyde-fixed specimens of 24 ovarian benign and neoplastic specimens were assayed for tumor cell-specific oncogene expression by a sensitive, quantitative in situ hybridization technique with probes for 17 oncogenes, beta-actin, and E. coli beta-lactamase. In the benign, borderline, and invasive adenocarcinomas, multiple oncogenes, including neu, fes, fms, Ha-ras, trk, c-myc, fos, and PDGF-A chains, were expressed at significant levels relative to a housekeeping gene (beta-actin). In the mixed-Mullerian tumors, a rather different pattern of oncogene expression was observed, characterized primarily by expression of sis (PDGF-B chain). For the adenocarcinomas, statistical analysis demonstrated that expression of several genes (fms, neu, PDGF-A) was closely linked to others (c-fos, c-myc) known to have important roles in the control of cell proliferation, but only one gene, fms, correlated very strongly with clinicopathologic features (high FIGO histologic grade and high FIGO clinical stage) predictive of aggressive clinical behavior and poor outcome. The authors discuss the role that tumor epithelial cell expression of the fms gene product might play in the auto- and paracrine control of growth and dissemination of ovarian adenocarcinomas.

[Candidiasis involvement of the small intestine complicated by perforation. A case of favorable course].

We report the case of a 51 year-old man who developed candidiasis of the small bowel associated with intestinal perforation. This condition is very rarely recognized ante mortem. Here, the diagnosis was established by pathological examination of a surgically resected specimen of small bowel at the time of surgery. A complete work-up failed to disclose any predisposing condition to digestive candidiasis. Antifungal therapy resulted in complete recovery.

[Transient bone marrow aplasia associated with hepatitis. Possible effect of lithium therapy (author's transl)].

A 70-year-old patient developed a bone marrow aplasia following the onset of an apparently infectious hepatitis. During the course of his illness he experienced episodes of severe sepsis, that were treated with antibiotics and leucocyte transfusions. The authors suggest that lithium therapy might be of benefit for such patients because of his granulopoïetic properties.

[Complex recto-colic adenomatous and hamartomatous polyposis with hyperplastic gastric polyps in a 13-year-old girl].

We report the case of a 13-year-old girl who presented mixed polyposis coli and gastric polyps. The rectal and colonic polyps were of the adenomatous and hamartomatous type (i. e. juvenile and Peutz-Jeghers polyps) without any intermediate features; in the stomach, the polyps were of the hyperplastic type. Microadenomas of the appendix and lymphoid hyperplasia of the ileum were also found. The karyotype was normal and the HLA group was A2 B12. No family history of polyps was discovered. To our knowledge no such case (complex polyposis) has been previously described. The present findings suggest that the usual pathological classification of polyposis may be arbitrary and that various pathological types may represent different aspects of the same spectrum of disease. Absence of "intermediate" polyps rule out the hypothesis of the sequence "hyperplastic polyp-juvenile polyp-adenoma".

[Evaluation of the results of treatment of Graves' disease by synthetic antithyroid drugs].

A study of 43 case records of Grave's disease followed up for at least two years after stopping treatment with synthetic antithyroid drugs, revealed 7 patients who did not relapse, 12 cases of rebound, (less than six months after stopping treatment), and 21 cases of recurrence (more than six months) of 10 cases (23%) had several recurrences. In relation to the 87 patients who formed this series, the cure rate after a single acute attack was only 50%. The good tolerance of antithyroid drugs, their almost complete efficacy, (2 failures out of 87 patients), indicate however that for a first attack they should be given for at least 18 months. Rebounds and recurrences suggest that radical treatment, mainly surgery, still have their place.
