[Surgical treatment of subclavian steal syndrome and innominate steal syndrome. Long-term results].

Thirty-four patients who underwent carotid-subclavian bypass (64.7%), aorto-carotid bypass (17.6%), endarterectomy of subclavian artery (5.8%) or endarterectomy of innominate artery for "subclavian" or "innominate steal" syndromes, had a follow-up studies 3-8 years since surgery. All patients demonstrated originally a differential in systolic blood pressure in the upper extremities. The surgical indications were posed on the basis of neurologic symptoms of vertebrobasilar insufficiency and the angiographic evidence of "subclavian (79.4%)" or "innominate (20.6%) steal". The carotid-subclavian bypass has been the preferred surgical treatment technique in patients with "subclavian steal". This operation is indicated also in nearby asymptomatic patients in order to prevent the evolution of the natural history of vertebrobasilar insufficiency. Longterm results are considered satisfactory in surgically treated patients.

[Inflammation of the anterior chest wall: its assessment by magnetic resonance].

Magnetic resonance (MR) is the imaging method used to assess primary and secondary inflammatory lesions of the chest wall. Five patients with inflammatory lesions of the anterior chest wall were submitted to MR examinations in order to define the role of this method in both identification and assessment of the extent of tissue involvement. In all the examined patients MR Imaging accurately showed the involved muscle groups and the presence/absence of mediastinitis. MR Imaging easily detected sternal and clavicular osteomyelitis; in one patient only MR failed to detect osteomyelitis of the first rib. The accurate assessment of soft tissue infections enabled us to select the most suitable surgical therapy to reduce esthetic damage. Two cases were restudied after chest wall reconstruction by means of rotated pectoral flaps. In the 2 patients in whom the differential diagnosis between inflammatory lesion and recurrent tumor could not be made by means of MR Imaging, CT-guided needle biopsy was performed.

[Pertussis. Critical revision of its physiopathology. Implications].

The re-examination of Bordetella pathogenic power and of the mechanisms which regulate relations between these bacteria and men make us conclude that in pertussis and with intramuscular vaccines an antihistaminic "coverage" is needed and antipertussis vaccinations is to be made by natural way. Via aerosol, minor intrinsic toxicity of acellular vaccines will be determinant.
