Evaluation of post-pneumonectomy space by computed tomography.

Twenty-two patients who had undergone pneumonectomy for various reasons were evaluated by computed tomography (CT) in order to study the post-pneumonectomy space (PPS). In 13 of 22 cases, the residual PPS contained fluid, even years following surgery. In 9 of 22 cases, the PPS was obliterated. Obliteration of the PPS, one of the events that may follow pneumonectomy, is significantly correlated with the expansion of the nonoperated lung (coefficient, 0.84). Obliteration of the PPS does not present a significant coefficient of correlation with the following parameters, also considered in this study: time interval between surgery and CT, presurgical tests of respiratory function, hemidiaphragm elevation, and retraction of operated hemithorax. The fluid contained in the PPS does not organize but persists or is reabsorbed. Mediastinal shift depends on the expansion of the residual lung. The mediastinum rotates following right pneumonectomy and shifts following left pneumonectomy. The usefulness of CT in the follow-up of the pneumonectomized patient is stressed.

Computed tomography of deep fat masses in multiple symmetrical lipomatosis.

Deep fat masses were evaluated by computed tomography (CT) in 15 patients with multiple symmetrical lipomatosis. In 4 patients, peritracheal accumulations of fat were observed. In 3 of them, tracheal compression by lipomatous tissue was demonstrated: 2 were asymptomatic and the third had severe respiratory insufficiency secondary to blockage of the airway by the vocal cords as the result of recurrent nerve palsy. In 6 patients, lipomatous tissue occupied the potential space between the spinal scapulae and the trapezius, supraspinatus, and infraspinatus muscles. In 2, calcification of lipomatous masses was observed. There was no relationship between extension of subcutaneous fat and accumulation at deep sites. CT facilitates early detection of peritracheal lipomatous tissue and is helpful in follow-up when deep fat accumulation is responsible for space-occupying lesions requiring surgery.

[Computerized tomography in diffuse post-traumatic brain damage].

Among 851 patients admitted following a head injury at the Neuroradiological Service and Neurosurgical Department of the University Hospital of Padua from January 1979 to June 1981, 51 presented a diffuse damage of the white matter of the brain. The CT scan, the only neuroradiological method able to show such a lesion, does never reveal large lesions but only minimal alterations of the cerebral parenchyma, opposite to the severity of clinical picture. We discuss the etiopathogenetic, anatomopathologic and neuroradiologic aspect of diffuse brain damage, emphasizing, the technical difficulties in the demonstration, on CT scan, of some lesions, particularly at the level of the brainstem.

Posttraumatic intraventricular haemorrhages.

Of a series of 350 patients studied for blunt head trauma by CT scan 10 were found to have an intraventricular haemorrhage (IVH); in 8 cases we could find concomitant CT abnormalities as well as intracerebral contusion or haemorrhage, and in two cases no other CT abnormality was noted. CT scan represents the first reliable and non-surgical tool for identifying this process. Two possible mechanisms that govern the formation of an IVH are postulated: a) an erosion of the ventricular wall by an intracerebral haemorrhage; b) the rupture of subependymal veins deformed by the negative pressure following dilatation of the ventricular wall. The prognosis in our cases is severe.

Production of prostaglandins in homogenates of kidney medullae and cortices of spontaneously hypertensive rats fed menhaden oil.

Menhaden oil (MO), whose polyunsaturated fatty acids consist mainly of (n-3) fatty acids, was fed to spontaneously hypertensive rats to determine the effect of (n-3) fatty acid on the in vitro production of prostaglandins produced from arachidonic acid (20:4[n-6]). Capacity to form PGE2 and PGF2 alpha was impaired in homogenates of kidney medullae and cortices from rats fed the MO diet compared to rats fed the control diet. The lower amounts of diene prostaglandins produced corresponded to the decrease in the amount of 20:4 (n-6) in the tissue. Possibly changes produced in tissue lipids by dietary fatty acids affect prostaglandin production by reducing the availability of substrate in tissue lipids.

[The CT-scan and hypocycloidal cysternography with water soluble non ionic contrast media (metrizamide and iopamidol) in sellar and parasellar lesions (author's transl)].

A group of 30 patients with sellar or parasellar lesions were investigated by CT-scan and hypocycloidal tomography after lumbar injection of water-soluble non ionic contrast media. The combination of the 2 techniques (CT-scan and tomography) allowed very satisfactory diagnostic results and the excellent tolerance of non-ionic contrast was confirmed in this patient series.

[Phonocardiographic characteristics of normally functioning Lillihei-Kaster mitral prostheses. Study of 61 cases (author's transl)].

A phonomechanocardiographic investigation has been carried out on 61 patients who had undergone mitral valve replacement with a Lillehei-Kaster prosthesis between May 1972 and April 1976 (Follow-up 48-96 months). All prostheses were considered to be well functioning according to clinical, electrocardiographic, radiological and laboratory findings. Based on the results of the present study the phonomechanocardiographic features of normal functioning Lillehei-Kaster mitral prostheses are the following: 1) a double or triple closing sound; 2) a small single or double opening sound recorded in 67.21% of our series. The opening sound, preceding the 0 point of the apexcardiogram of approximately 29.8 msec is most likely related to the beginning of the motion of the disc, whereas the opening sound coincident with or following the 0 point of the apexcardiogram is produced at the end of the opening movement of the disc; 3) the presence of meso or telediastolic clicks in patients with atrial fibrillation during longer diastolic periods or occasionally in patients with sinus rhythm, mild aortic regurgitation and or 1st. degree A-V block; 4) an apical diastolic rumble present in 68.85% of our cases, regardless of the size of the prosthesis, which does not necessarily indicate a prosthesis malfunction; 5) a systolic ejection murmur, recorded on the left upper sternal border in all our patients, is probably related to the turbulent blood flow around the prosthetic prongs which protrude into the left ventricular outflow tract.

Primary brain stem haematomas. Diagnosis and treatment.

Case histories are reported of 18 patients in whom the diagnosis of primary brain stem haematoma and brain stem haemorrhage was made by computed tomography (CT). The possibility of an early diagnosis of brain stem haematoma, and the differential diagnosis of brain stem haemorrhage by a combination of clinical and tomodensitometric criteria is emphasized. It is concluded that CT is a highly reliable method for the diagnosis, location, and management of brain stem haematoma.

Computed tomography of the liver in von Gierke's disease.

The computed tomography findings in the liver of a patient with von Gierke's disease are presented. Precontrast scans demonstrated diffuse decreased density throughout the liver. In the postcontrast scans, a focal right sided hyperdense area was visualized.

Exophthalmos from aneurysmal bone cyst of the orbital roof.

An aneurysmal bone cyst of the orbital roof, causing a monolateral exophthalmos in a 15-month-old child with no previous history of head injury, was operated upon and histologically misdiagnosed as a giant cell tumor. Radiation treatment in a dosage (3,740 rad) appropriate for the latter diagnosis apparently caused a massive calcification of the residual mass with persisting exophthalmos. At a second operation the lesion was removed completely and a radical change in the histopathological features was found. 12 years after surgery, there is no evidence of residuals upon roentgenographic and CT scan examinations and the exophthalmos has practically disappeared, while there is a normal visual function in the affected eye. Some points of histopathological and gross pathological differential diagnosis between aneurysmal bone cysts and giant cell ("myeloplaxis") tumors are discussed.

[Fragmentation of pulmonary emboli (author's transl)].

Preliminary results of attempts of pulmonary artery thrombi fragmentation after venous embolism are presented. Fragmentation is directly obtained by angiographic catheters (F 9 or 10). By this method we attempted to remove the block of some pulmonary branches, in order to obtain a perfusion of them and to improve fibrinolysis. This method is useful in patients who are not candidated for surgical embolectomy.
