[Pituitary hemangiopericytoma].

Information on the first observation of the hypophyseal hemangiopericytoma having the clinical symptoms of hypophyseal adenoma is presented. It is noted that such tumours can be mistakingly diagnosed as adenomas. Therefore on the basis of light microscopical, histochemical and electronmicroscopical data a tactical diagnostical algorithm is worked out and proposed that facilitates the distinguishing of hemangiopericytoma among other tumours of the hypophysis anterior lobe.

[Comparative clinico-morphological characteristics of peace-time anaerobic infections].

Altogether 26 cases of anaerobic infection (AI) of various etiology were analysed. Local tissue ischemia and operations on gastrointestinal organs in patients with secondary immunodeficit conditions are the factors facilitating the development of AI. The distinction is made between clostridial AI with a rapidly progressing gaseous gangrene and the non-clostridial AI with a slower course of a serous-purulent phlegmoma. Anaerobic myositis is observed in all forms of AI. Depending on the localisation of AI, anaerobic cellulitis and fasciitis are mentioned which occur mainly in non-clostridial AI. The treatment of AI and the mechanisms of death are discussed.

[Pathology of cerebral artery aneurysms in the light of their surgical treatment].

Surgical treatment of cerebral aneurysms had a significant influence on the pattern of the course, manifestations, and outcomes of the disease. Variants of complications of surgical interventions occurring in operations made early and late after rupture of the aneurysms in open and intravascular accesses are analysed, as well as morphological aspects of the cerebral vasospasm. A previously unknown mechanism of growth and shape formation of aneurysms associated with separation of the intima of the carrying vessel is described. The current concepts on the mechanisms of aneurysm rupture are analysed. The importance of exfoliating hematomas which in some cases precede the rupture is emphasized. The capacity of aneurysm wall for regeneration is limited. Healing of the rupture occurs predominantly at the expense of hematoma organization around the aneurysm sac. The "defective" regeneration is the cause of frequent recurrent ruptures and limit the use of aneurysm ballooning in the acute period of their rupture.

[Capillary telangiectasia of the pons].

Three observations of teleangioectases of the Varolian bridge verified at a histological examination are presented. In one of the cases the telangioectases were concurrent with an astrocytoma of the left hemisphere, and were a casual finding. In another case the teleangioectases have led to a massive hemorrhage to the Varolian bridge and the patient's death. In the third case the teleangioectases were revealed at the histological examination of the Varolian bridge affected by an astrocytoma. The development of the tumour was, possibly, due to reactive gliosis withing the zone of a vascular developmental defect. Clinical, roentgenological, pathomorphological, and histological data are presented. The authors point out that histological examinations are obligatory in cases of hemorrhages to the Varolian bridge, so that their origin should be properly documented.

[Giant aneurysms of the cerebral vessels].

The structure of 20 giant aneurysms (GA) of cerebral arteries was studied. The main characteristics of GA structure are presented, the microscopic picture of GA wall is described in detail, the morphological data determining pseudotumour symptoms in this disease are given. Complications of surgical treatment are analysed separately. The GA group is stated to be of various origin and to include both atherosclerotic and congenital aneurysms. The described morphological features of GA may be important for treatment by modern methods (intravascular interventions, stereotactic punctures).

[Unique tissue differentiation in peripheral nervous system tumors (glandular neurofibroma)].

A rare observation of glandular differentiation in neurofibroma of the right orbit is described. Data on another 7 similar tumors reported in the literature are presented. The problems of histogenesis of such neoplasias and difficulties of their diagnosis are discussed. Glandular neurinomas and neurofibromas are characterized by a malignant course and trend for relapses favouring high lethality in this disease.

[Certain clinico-anatomic features of large and giant arterial aneurysms of cerebral vessels].

On the basis of examining 72 patients 14 of whom later died it has been concluded that the most substantial differences of large and giant aneurysms of cerebral arteries from small ones consist in development of focal neurological symptoms and intracranial hypertension: this is due to both compression of the brain structures and disturbance of the cerebral circulation caused by aneurysms more than 2 cm long. The intracranial hypertension is the most frequent in aneurysms over 3 cm long, especially, if those aneurysms are localized in the vicinity of cerebral liquor ducts. Patients with arterial aneurysms exceeding 2 cm develop hemorrhagic complications in 2/3 of the cases. While using modern treatment methods (stereotaxic and intravascular) one should take into account the peculiarities of the structure of large and giant aneurysms in the brain.

[Intrauterine candidiasis of the brain].

Clinical data and morphological pictures of two cases of intrauterine affection of the brain with fungi of the genus Candida are presented. Tissue reactions in the brain were of productive inflammation nature and were accompanied by calcium deposition, focal hemorrhages, and the development of pseudoporencephalia. Changes in resistance of the baby occurred as indicated by hyperplasia of the lymphoid apparatus and the occurrence of nonspecific inflammation in other organs. When calcifications are found in the brains of newborn babies, it is recommended that proper examinations be done to exclude intrauterine mycosis.

[Kidney morphology in the hemolytic-uremic syndrome].

A case of hemolytic-uremic syndrome (Gasser syndrome) in a boy of 7 is described. The disease was accompanied by thrombocytopenia, anemia, hemorrhagic syndrome and acute renal insufficiency (ARI). Morphological lesions consisted in the predominant involvement of the kidneys presenting thrombosis of the glomerular capillaries and reglomerular arterioles, fine cortical necroses, and considerable degenerative changes in the tubular epithelium. The peculiar features of the disease included low intensity of hemolysis and the lack of the oligoanuric phase of ARI.

[Racemose arteriovenous angioma (malformation) of the brain].

242 cases of brain arterio-venous malformations (AVM) are analysed clinically and anatomically. Their pathology, histological, ultrastructural and immunohistochemical characteristics, therapeutic pathomorphosis are described. The concept is suggested according to which AVM is a collective term including true developmental defects, dysembryogenetic tumors and outcomes of various pathologic processes (trauma, brain circulation and compensatory-adaptive disturbances, etc.). The necessity of therapeutic methods corrections in agreement with the disease pathogenesis is outlined.

[Vascular abnormalities of the spinal cord ].

Vascular anomalies of the spinal cord were studied in 49 patients. Thirty-three cases were morphologically studied (30 cases presented with operative findings and 3 cases with autopsy material). The anomalies presented as defects of vascular genesis without signs of blastomatous growth (venous and capillary-venous malformations, arteriovenous anastomose and fistulas, varicosis of the spinal cord, etc. located in the spinal cord, its roots, coats, adjacent fat, and vertebra which affect the spinal cord, roots or vessels participating in their blood supply and venous outflow. In 41% of the cases, the above anomalies were associated with congenital anomalies of other systems and organs. The anomalies are clinically characterized. Specific signs are given to differentiate them from their intracranial analogues. Recommendations are given on pathological examination of the content of the vertebral canal in vascular anomalies.

Central Nervous System Vascular Malformations↗

[Intracranial cavernous angiomas: ultrastructural and immunochemical study].

43 brain cavernous angiomas (CA) were clinically and morphologically studied. The presence of smooth muscles and endotheliocytes in the trabeculae was confirmed. The sources of blood supply and the nature of medullary changes at the CA periphery were clarified. The hypothesis of CA hormonal dependence has been rejected.

[Early and late results of the intravascular treatment of cerebral aneurysms with a detachable balloon-catheter].

The paper analyzes the outcomes of intravascular treatment in 87 patients with cerebral aneurysms with separable a balloon catheter with a valve device. In the cold and hemorrhagic periods, mortality rates were 6.4 and 40%, respectively. The postoperative follow-ups lasted 1 to 17 years (mean 7.7 years). Recurrent subarachnoidal hemorrhage (SAH) was noted in 6.5% of cases, with not earlier than 4.5 years after intervention. The remaining patients (even with partial aneurysmal occlusion) had no recurrent SAH.
