Pseudocataplexy: gelastic-atonic seizures.

Gelastic-atonic seizures are characterized by laughing and then loss of muscle tone. They are difficult to differentiate from laughter-induced cataplexy and may be indicative of Niemann-Pick disease Type C or variants. We studied an 11-year-old girl who had gelastic-atonic seizures that were triggered by laughing. The nature of her attacks was established by ictal EEG ambulatory recordings at home. There was no evidence of Niemann-Pick disease.

Absence status manifested by compulsive masturbation.

Absence status manifested by compulsive masturbation occurred in a 41-year-old man with generalized epilepsy. Ictal simple and complex sexual manifestations during partial seizures in focal epilepsy are rare, but, to our knowledge, our report of this sexual automatism during absence status in generalized epilepsy is unique.

Periodic EEG patterns in cerebral fat embolism.

A 24-year-old male developed fat embolism syndrome (FES) 18 hours after accidental tibial and metatarsal fractures. The onset was characterized by coma, generalized seizures, respiratory distress and hypoxemia. Multiple prolonged portable electroencephalographic (EEG) recordings showed a variety of progressively changing patterns including continuous high amplitude EEG rhythmical synchronous slowing (CHERSS), triphasic waves, bilateral independent periodic lateralized epileptiform discharges (BIPLEDs), frontal intermittent delta activity (FIRDA), and low amplitude irregular generalized theta. Three months after the accident, the patient was neurologically normal and his EEG exhibited only rare brief bursts of anterior theta in the waking recording. I conclude that EEG periodic activity such as CHERSS, triphasic waves, BIPLEDs, and FIRDA can occur in patients suffering from cerebral fat embolism. It reflects a non-specific diffuse encephalopathy not necessarily associated with a poor prognosis. Prolonged and frequent bedside EEG recordings will demonstrate better these fluctuating abnormalities in cerebral fat embolism (and probably in coma of other origin) than standard less frequent recordings. Very likely, these periodic EEG patterns have not been previously identified in cerebral fat embolism because they were not in vogue among electroencephalographers nor widely accepted at the time studies dealing with this syndrome were written.

Isodense cerebellar hematoma.

One week after chiropractic manipulations, a 60-year-old normotensive man suffered acute onset of vertigo, ataxia, and dysarthria. CT disclosed an isodense mass of the posterior fossa. Vertebral arteriography showed that the avascular mass arose from the right cerebellar hemisphere. Hydrocephalus developed, and suboccipital craniotomy revealed a large cerebellar hematoma due to a small arteriovenous malformation. A similar episode occurred 2 months later. CT isodense cerebellar hematomas should be considered before giving anticoagulant therapy.

Postural reflex gelastic seizures.

A 26-year-old man had gelastic seizures ("laughing attacks") preceded by an aura of sexual pleasure. They were reflexly induced by volitional hyperextension of his neck and trunk. His EEGs showed a right temporal spike focus. We discuss the characteristics of this unique case.
