Elevated thrombocyte calcium content in uremia and its correction by 1 alpha(OH) vitamin D treatment.

Intracellular calcium plays an important role in the regulation of platelet function. It has also been demonstrated that platelet functions are impaired in uremia. A rise in intracellular calcium has been shown in several tissues and has been held responsible for the impaired function of several organs seen in uremia. This study was undertaken to evaluate whether the calcium (Ca) content of thrombocytes is elevated in uremia and, if so whether treatment with an active vitamin D metabolite might correct this abnormality. In 10 patients on chronic hemodialysis, platelet Ca content was determined by a technique utilizing consecutive freezing and thawing of platelet-rich plasma. The platelet Ca content of uremic patients was found to be markedly higher (20.86 +/- 0.9 ng/200,00 platelets, p less than 0.01) than that of a group of 20 normals (12.8 +/- 1.2 ng/200,000 platelets). 1 months after treatment with 1 alpha (OH) vitamin D at a dosage of 0.5-2.5 microgram/day, the platelet Ca content of the dialysis patients decreased to 14.99 +/- 2.14 ng/200,000 platelets (p less than 0.05). The data show that in dialysis patients the platelet Ca content is markedly elevated in comparison with that of normals, and the treatment with 1 alpha (OH) vitamin D may significantly reverse this abnormality. It is suggested that elevated Ca content may play a role in the pathogenesis of uremic platelet dysfunction, and that 1 alpha (OH) vitamin D administration may be of benefit in correcting this disorder.

Ultrastructural and functional studies on human platelets incubated with diclofenac sodium (Voltaren).

The in vitro effect of diclofenac sodium (Voltaren) on the ultrastructure of healthy donors' platelets was examined and compared with those induced by aspirin (ASA). In distinction to ASA, which causes loss of platelet pseudopodia, Voltaren induced an increase and marked elongation of these pseudopodia. The implication of this finding in the explanation of the decreased platelet aggregation caused by the drug is discussed. Voltaren increased the phagocytotic activity of the individual platelet, although the overall ability of the cells to phagocytize latex particles was not markedly increased. Platelets incubated with Voltaren showed a decrease in their total protein synthesizing capacity. Voltaren did not exert any effect on the internal ultrastructure, platelet factor 3, and calcium content of the incubated platelets.

Kallikrein and prekallikrein determination in a number of pathological conditions using the Centrifichem analyzer.

An improved method for the determination of kallikrein (K) and prekallikrein (PK) is described. The chromogenic substrate N-Benzoyl-L-Prolyl-L-Phenylalanyl-L-Arginyl-p-Nitroanilidhydrochlorid is used and adapted for routine determinations with the Centrifichem 500 analyzer. A group of 30 healthy persons are compared with 10 allergic patients, three patients with hereditary angioneurotic edema and three patients with migraine attacks and the normal and pathological values described and discussed.

Coagulation studies for severe liver disease detection in a gastroenterologic department.

15 patients with clinical, histological and laboratory findings of cirrhosis were investigated. When the results of routine hemostasis tests--thrombin time, reptilase time and fibrinogen quantitation by clotting methods--were prolonged, further immunological determinations were done. Normal results in the immunological determinations together with prolonged ones obtained by clotting methods are suggestive of dysfibrinogenemic states. A possible survey of the treatment with the help of these determinations is proposed.

Extensive pharyngeal hemorrhage induced by dipyridamole.

An extensive hemorrhage in the pharynx appeared in association with dipyridamole administration to a diabetic patient. The hemorrhage caused dysphagia and subsided when the drug was discontinued. Coagulation tests were normal except for a decreased platelet aggregation. Platelet factor 3 and the aggregation tests with adenosine diphosphate and epinephrine were abnormal during the hemorrhagic event and returned to normal after discontinuation of the drug.

Platelet functions before, during and after labor.

Platelet functions were evaluated in 20 healthy pregnant women in the last months of pregnancy, during labor, and 5-12 weeks after labor. The results showed an increased platelet count, bleeding time and nitroblue tetrazolium (NBT) test, as well as an increased plasma calcium level postlabor. ADP- and epinephrine-induced aggregation during labor was markedly decreased, followed by a statistically significant increase postlabor. These findings are in correlation with the high incidence of thromboembolic phenomena occurring postlabor.

Blood hyperviscosity in psoriasis.

A study of platelet function and whole-blood viscosity in 11 patients with the confirmed diagnosis of psoriasis revealed a significant elevation in whole-blood viscosity (3.33 +/- 0.37 cP) as compared with that found in a control group (2.80 +/- 0.1 cP). The platelet count and platelet aggregation with ADP, epinephrine and collagen as well as platelet malonyldialdehyde were all within the normal range in all the patients. It is suggested that the increased blood viscosity may contribute to the higher incidence of occlusive vascular disease occurring in patients with psoriasis.

Inhibition of platelet aggregation by contrast media.

The effect of 30% Urografin on platelet aggregation induced by adenosine diphosphate (ADP), epinephrine, and arachidonic acid was examined. In vivo and in vitro experiments in 10 nondiabetic subjects and in vitro experiments in 7 diabetic-uremic patients showed a statistically significant decrease in platelet aggregation 20 minutes after in vitro incubation or in vivo infusion with Urografin. Urografin concentration was about 2.4-2.5%. The possibility of bleeding induced by contrast media and the connection with acute renal failure in diabetic-uremic patients are discussed.

Effect of vitamin E on platelet aggregation in diabetic retinopathy.

The effect of vitamin E on platelet aggregation was investigated in a group of 10 patients with diabetic retinopathy. Adenosine diphosphate induced platelet aggregation was inhibited in the patients' group as well as in the controls in the presence of vitamin E. An increased platelet aggregation was obtained with arachidonic acid in both groups when platelet-rich plasma was incubated prior with vitamin E. The clot retraction inhibition test parallels the findings of aggregation obtained with the platelet aggregation meter.

Long-term electrode implantation for recording cochlear electrical activity in guinea pigs.

Comparative measurements of the cochlear electrical activity over prolonged periods require long-term implantation of electrodes. During this time, changes in the electrode recording ability and/or impairment of the sound-conducting system, could erroneously be attributed to the experiment. To avoid this risk, an easy and reliable technique is described. The procedure uses the Fallopian canal as a route by which to approach the cochlear potentials' generators, while leaving the middle ear untouched.

Decreased calcium content in patients with digoxin intoxication.

The platelet calcium content, the plasma calcium level and the serum digoxin were examined in 90 individuals divided into the following four groups : I -- patients with digoxin intoxication; II -- treated with digoxin; III -- treated with digoxin and furosemide, and IV -- control subjects. The patients with digoxin intoxication showed a lower platelet calcium content as compared to subjects of the other groups (p less than 0.001), whereas the serum digoxin level was significantly higher than in the patients of groups II and III (p less than 0.001). Since the examination of the platelet calcium level is a relatively simple and rapid procedure, with reliable results, its use as a routine method in the determination of digoxin intoxication is suggested.

The effect of pH on platelet and coagulation factor activities.

After observing that lavage of the stomach with alkaline solutions has a beneficial effect on the control of gastric hemorrhage, we examined the platelet and coagulation factor activities under conditions of lower and elevated pH. The results showed that change of the pH from acidosis to 7.0 and even to slight alkalosis induces platelet aggregation, platelet calcium and serotonin release, as well as platelet factor II availability. The prothrombin and partial thromboplastin times approached normal levels, whereas the fibrinogen level did not change significantly. The results obtained may serve as an explanation for the control of the intragastric bleeding in patients treated by maintenance of their gastric pH at 7.0.

The danger of "yellow dyes" (tartrazine) to allergic subjects.

Oral administration of 50 mg tartrazine to 122 patients with a variety of allergic disorders caused the following reactions: general weakness, heatwaves, palpitations, blurred vision, rhinorrhoea, feeling of suffocation, pruritus and urticaria. There was activation of the fibrinolytic pathway as shown by reduction of plasminogen with high pre-kallikrein and low kallikrein values. Reduction in complement activity (CH50) was seen in three out of sixteen reactions.
