Prevalence and correlates of diphtheria toxin antibodies among young adults in Israel.

The paucity of information about immunity against diphtheria of young adults in Israel prompted us to analyse sera from a random sample of 480 recruits (263 males and 217 females) aged 18-19 years. Antitoxin antibody levels were determined by means of ELISA. Of the recruits 58.1% had antibody values greater than 0.1 IU/ml; 38.5% had amounts between 0.01 and 0.1 IU/ml, which is considered low when using the ELISA method, and 3.3% had less than 0.01 IU/ml. The results of this study suggest that a booster dose of antidiphtheria vaccine should be given to adults in Israel in order to ensure adequate antibody levels.

Treatment results of stereotactic interstitial brachytherapy for primary and metastatic brain tumors.

A total of 41 stereotactic interstitial brain implants in 39 patients were performed for recurrence after teletherapy (recurrence implant), or as part of initial treatment in conjunction with teletherapy (primary implant). Implanted tumors consisted of malignant gliomas (33), other primary brain tumors (3), and single metastatic lesions (3). All patients were temporarily implanted with Ir-192 using a coaxial catheter afterloading system; two patients were implanted twice. Survival post-implant for glioblastoma multiforme (GBM), 13 patients, was 10 months whether implanted primarily or for recurrence. Mean time to recurrence, measured from initiation of teletherapy to implantation, was 10 months. Twenty patients with anaplastic astrocytoma (AA) had a median survival post-implant of 23 months for primary implants (7 patients) and 11 months for recurrence implants (13 patients). Mean time to recurrence, measured from initiation of teletherapy to implantation, was 19 months. Three patients (9%) of the evaluable group required reoperation for symptomatic mass effect, all with initial diagnosis of AA. Survival for this subgroup was 14, 22, and 32 months post-implantation. Using stereotactic techniques, interstitial brachytherapy of brain tumors was technically feasible with negligible acute morbidity and mortality, and appeared to offer limited prolongation of control for a subset of patients with recurrent malignant gliomas. The role of this modality in primary treatment for malignant gliomas needs to be further defined by prospectively randomized trials.

A comparative experimental study of fetal phono- and movement-sensors from Amsterdam, Cambridge and Edinburgh.

This paper describes the experimental studies and results of a collaborative investigation between three European institutes: Amsterdam, Cambridge and Edinburgh. The object of this investigation and the collaborative exchanges was to evaluate and compare different sensors for recording fetal sounds and movements from each centre. This would be carried out in a series of experimental and clinical tests involving researchers from each of the European institutes, and using three sensors: one from each centre. Experimental measurements have been performed regarding the conversion gain, frequency response, equivalent input noise and dynamic range of the transducers. The measurements were carried out using a vibration-free table testing rig to evaluate these characteristics of each of the sensors. The equivalent input noise ranged from 50 nm to 1600 nm for the transducers studied.

Experimental transscleral iontophoresis of ciprofloxacin.

Ciprofloxacin was administered into the aqueous humor and vitreous body of the rabbit eye by transscleral iontophoresis. Positively and negatively charged forms of the drug molecule were tested. Therapeutic concentrations of ciprofloxacin were achieved in the aqueous body only when the negatively charged drug molecule was used. Ciprofloxacin did not reach the vitreous body in therapeutic concentrations in either the positively or negatively charged form, but higher concentrations were achieved when the drug was negatively charged. Peak levels were obtained in the aqueous and vitreous bodies (0.62 micrograms/ml and 0.19 micrograms/ml, respectively) one hour after transscleral iontophoresis of negatively charged ciprofloxacin at 5 mA for 15 minutes.

Sociodemographic factors associated with serum anti-Shigella lipopolysaccharide antibodies and shigellosis.

The association between various sociodemographic variables and the presence of anti-Shigella sonnei lipopolysaccharide (LPS) antibodies was examined in a random sample (N = 383) of male Israeli conscripts. Of the male conscripts, 190 (49.6%) had pre-existing antibodies against S. sonnei LPS (defined as HA titres of greater than or equal to 1:10 after treatment of sera with 2-mercaptoethanol). Univariate analysis revealed a significant positive association between the presence of humoral anti-S. sonnei LPS antibodies and sociodemographic variables including Eastern origin (p = 0.007), low socioeconomic status (p = 0.0016), and the number of siblings (p = 0.023). When multiple logistic regression was used to control simultaneously for the effects of the other variables, ethnic origin emerged as the strongest correlate of anti-S. sonnei LPS antibodies. On the other hand, the association of the sociodemographic variables in subjects suffering from S. sonnei infection during their military service, was in the opposite direction (p less than 0.001 for both socioeconomic status and ethnicity). These findings suggest differences between subpopulations in acquired immunity to S. sonnei due to differences in exposure to the homologous organism prior to military service.

Developing a more focal magnetic stimulator. Part I: Some basic principles.

Some general properties of currents induced by magnetic stimulators in volume conductors of any shape are first reviewed. Then, the property of focality (concentration at some internal point) of the current induced in a spherical model of the head is discussed, due to coils of various orientations and configurations. It is shown that one important property for focality is the complete absence of the radial component of current, regardless of the coil used. The theoretically computed current distributions in the sphere produced by three different coils are illustrated. These are (1) a circular coil parallel to the head (very nonfocal). (2) a circular coil erect on the head (more focal), and (3) a figure-eight coil parallel to the head (most focal). A computational search for yet more focal coil configurations shows that the figure-eight focality can only be altered somewhat, but basically not improved. A plot of focality of the figure-eight coil versus the diameter of its coils shows an improvement in focality with decreasing diameter down to one cm. At this diameter, the induced current at a level 2 cm below the coil is concentrated in a band 1.5 cm wide (half-maximum points). Finally, it is noted that as the diameter of the figure-eight coil is decreased, the current in the coil necessary for stimulation increases rapidly, with increasing engineering problems.

Prospective study of the association between serum antibodies to lipopolysaccharide O antigen and the attack rate of shigellosis.

A means for determining immune status against shigellosis would significantly improve the design and evaluation of interventional and other epidemiologic studies. Previous case-control studies have indicated the potential role of humoral antilipopolysaccharide antibodies. To test this proposition, 190 soldiers serving in a field unit were monitored prospectively for 2.5 months for shigellosis. Blood samples were taken at the beginning of the follow-up period and tested for serological evidence of prior exposure to Shigella sonnei and Shigella flexneri. The risk for acquiring S. sonnei shigellosis was 3.7 times higher for individuals lacking homologous antibodies (P less than 0.02). The risk for acquiring S. flexneri shigellosis was 2.4 times higher for individuals lacking antibodies, although a low attack rate for S. flexneri resulted in numbers too small to achieve statistical significance. While the importance of the serum antilipopolysaccharide antibodies in protection against the disease remains unclear, these findings demonstrate that they are strong markers of acquired immunity. Serological markers should be incorporated in epidemiologic studies of shigellosis and in the design and evaluation of future trials of potential anti-Shigella vaccines.

Lead enhances lipopolysaccharide and tumor necrosis factor liver injury.

Lead markedly augments the lethality of endotoxin lipopolysaccharide (LPS) in rats. In this model of LPS toxicity, the liver is severely injured. Much of the tissue injury produced by LPS is thought to be mediated by the cytokine tumor necrosis factor (TNF). Tumor necrosis factor recently has been speculated to be a mediator of several models of liver injury such as that produced by galactosamine. To investigate the possible role of TNF in the lead-enhanced LPS toxicity model, we administered doses of lead acetate (15 mg/kg), LPS (100 micrograms/kg), or TNF (6.25 x 10(6) U/kg) that produced minimal changes in liver enzymes. However, when lead was administered simultaneously with either LPS or TNF, serum aspartate transaminase, alanine transaminase, alkaline phosphatase, glutamyl transpeptidase, and plasma triglyceride levels were markedly increased. Lead + LPS treatment increased both peak serum TNF concentrations and TNF "area under the curve" as compared with LPS alone. We conclude that lead not only enhances LPS lethality but also LPS liver injury. Furthermore, lead enhances TNF liver injury and increases LPS-stimulated serum TNF levels. These data suggest that the lead-enhanced LPS model offers a system for studying TNF-induced liver injury.

Increased plasma and ventricular fluid interleukin-6 levels in patients with head injury.

The cytokine interleukin-6 (IL-6) plays a major role in initiating the acute phase response, especially in the production of acute phase reactants such as C-reactive protein. The objectives of this study were to determine whether plasma or ventricular fluid IL-6 levels were elevated at time of admission after head injury and whether plasma IL-6 levels related temporally to clinical improvement of levels of acute phase reactants. Thirty patients with Glasgow Coma Scale (GCS) scores of 3 through 10 were observed for 15 days after head injury. Peak elevation of plasma IL-6 occurred on admission (85 +/- 12 U/ml; normal level is less than 2 U/ml) and then decreased during the hospital course to a level of 29 +/- 4 U/ml on day 15. Plasma IL-6 levels decreased significantly faster in patients with admission peak 24-hour GCS scores of 8 through 10 compared with patients with GCS score less than 8 (p less than 0.01). Patients had markedly elevated and variable ventricular fluid IL-6 levels on admission (mean 3880 +/- 2022 U/ml; normal, less than 2 U/ml). A temporal relationship was found between plasma IL-6 levels and multiple acute phase reactants thought to be mediated by IL-6. We conclude that plasma and ventricular fluid levels of IL-6 are elevated after head injury and that plasma IL-6 level is temporally related to acute phase reactants and clinical improvement. We suggest that IL-6 may play an etiologic role in many of the metabolic or nutritional sequelae of head injury.

The response of ataxia-telangiectasia lymphoblastoid cells to neutron irradiation.

The response of control and ataxia-telangiectasia (A-T) cells to increasing doses of high-linear-energy-transfer (LET) ionizing radiation (neutrons) was compared. Ataxia-telangiectasia cells were markedly more sensitive to neutron irradiation than were control cells. The D0 value for the two A-T cell lines was 0.4 Gy while the value for controls was approximately 1.4 Gy. Fast neutrons were considerably more effective than gamma rays in inducing cell death in both cell types, but the sensitivity factor remained approximately the same as with gamma rays. A minimal depression of DNA synthesis was observed in ataxia-telangiectasia cells after neutron irradiation, similar to that reported previously after gamma irradiation. The extent of inhibition was not significantly greater in control cells, contrary to that seen with gamma rays. In time-course experiments a significant difference in degree of inhibition of DNA synthesis was observed between the cell types. Low doses of fast neutrons induced a G2-phase delay in both cell types, but the degree and extent of this delay was greater in ataxia-telangiectasia cells as observed previously with low-LET radiation.

Developing a more focal magnetic stimulator. Part II: Fabricating coils and measuring induced current distributions.

First, our program for fabricating stimulator coils that are focal is reviewed. These are in the figure-eight shape, where the goal is to make them successively more focal, hence of successively smaller size. Although smaller coils require larger pulse currents, with resulting stress and heat problems, operating prototypes were readily fabricated with circle diameters down to 2.5 cm. However, for smaller diameters, the coil casings were fractured, and new casing techniques are being explored. Second, our program is reviewed for measuring the distribution of current induced in saline tanks by stimulator coils, to determine their focality. A coaxial probe measures the potential difference between two neighboring points in the tank, yielding the local current density. Measurements are presented due to several shapes of tanks and coil configurations. In a spherical tank, results confirm the theory that there can be neither radial current induced anywhere in the sphere nor any current at the sphere center. In tanks approximating a semi-infinite volume and the human limb, arrow-map distributions are shown, due to a commercial "pancake" coil and several figure-eight coils. In the semi-infinite tank, where the distribution can also be computed theoretically, the measured distribution agrees with the computations, thus validating the measurements. In the limb tank, the distribution is compressed and is somewhat more focal than in the semi-infinite tank, depending on the coil orientation.

Differential transport properties of two mdr gene products are distinguished by progesterone.

P-glycoprotein is an integral membrane protein that is overproduced in multidrug-resistant cells. It is likely to function as an energy-dependent drug efflux pump to maintain intracellular drug concentrations below cytotoxic levels. Individually isolated multidrug-resistant murine cell lines, J7.V1-1 and J7.V3-1, overproduce P-glycoproteins encoded by the mdr1b and mdr1a genes, respectively. The transport properties of these cell lines and the drug binding characteristics of their P-glycoproteins have been compared. It is concluded that 1) the mdr1a gene product is a more efficient efflux pump than the mdr1b gene product, and 2) whereas a single class of vinblastine binding sites is present in J7.V1-1 membrane vesicles, there appears to be two classes of such sites in J7.V3-1 membrane vesicles. The effects of verapamil and progesterone, two compounds that are known to interact with P-glycoprotein, have been analyzed in the two cell lines. Progesterone inhibited drug binding and efflux and increased drug sensitivity to vinblastine with more potency in J7.V1-1 cells than in J7.V3-1 cells. It is concluded that progesterone, but not verapamil, can be used to differentiate the two mdr gene products in the mouse.

Serologic survey for Borrelia burgdorferi antibody in horses referred to a mid-Atlantic veterinary teaching hospital.

Blood samples obtained from 13 of 100 (13%) and 6 of 91 (7%) horses at the George D. Widener Hospital for Large Animals in the months of June and October, respectively, had antibody to Borrelia burgdorferi as determined by ELISA. Horses from the states of New York, Maryland, Delaware, New Jersey, and Pennsylvania were seropositive for B burgdorferi. The frequency of antibody response in horses from New Jersey was greater (P less than 0.05) than the frequency of antibody response in horses from Pennsylvania or that of horses from the other states combined. Statistically significant difference was not found when a comparison was made between horses with serotiter and open diagnosis of neurologic or musculoskeletal disease and horses with negative serotest results and open diagnosis of neurologic or musculoskeletal disease.
