[Complications and psychosomatic problems after tubal sterilization].

From 1969 to 1974, 2080 patients were sterilized, 1702 by the pelviscopic method (electrocoagulation), 375 by laparotomy, and 3 vaginally. About two thirds of the sterilisations by laparotomy were carried out after delivery or during other surgical procedures. 15% of all women were sterilized for medical reasons; the other cases had to be 35 years of age or at least 30 if they had more than 3 children. The overall complication rate was 5% with both methods. Complications during pelviscopy included hemorrhage, bowel injury and burns. Postoperative complications after laparotomy were hemorrhage, thrombosis and wound dehiscence. Only half of the patients could be followed up; there were 4 pregnancies among 1308 women. In more than 50% the sexual relations had improved, in only 3% they had worsened. About 97% of the pelviscopically sterilized patients were satisfied with surgical method chosen and with their present status. It is recommended to allow sufficient time to think the decision over prior to the operation.

Food intake patterns of infants with high serum cholesterol level at six months.

In our longitudinal study on the influence of nutrition upon health during infancy we found in a cohort of 556 infants in good health, 6.4% of infants at six months with a total serum cholesterol level above 200 mg/100 ml. Because of the known importance of hypercholesterolemia for health we studied the role of the food intake patterns during the first six months in relation to the serum cholesterol. We analysed the 24 hours dietary recalls, the dietary habits and a set of biochemical determinations used for evaluation of nutritional status: 35 infants, 21 girls and 14 boys, were compared with a control group matched for sex and birth date. There were no differences in mean body weight and socio-demographic characteristics. Daily energy and protein intake was higher in the study group. Analysing milk consumption we found that the daily intake was always higher in the high cholesterol group. The solid foods were introduced earlier in the study group, i.e. for 46% of infants in the second week of life. The serum levels of vitamins A, C, and E, and of triglyceride were higher in the high cholesterol groups. At twelve months 33% of infants with the cholesterol level above 200 mg/100 ml we included in the studied group remain still with a high level. This tendency underlines the need of further research.

Progestin binding in testes from three siblings with the syndrome of male pseudohermaphroditism with testicular feminization.

We have found a specific binding protein for synthetic progestins 6,7-[3H]methyltrienolone (R1881) and 17,21-dimethyl-19-norpregna-4,9-diene-3,20-dione (R5020) and in the testis cytosol from three "sisters" with the complete form of the testicular feminization syndrome. The binding component sediments in the 8S region of sucrose gradients. It is saturable. The apparent affinity constant (Ka) for R5020 was determined in two cases and found to be 1.8 and 0.6 X 10(8) M-1. The number of binding sites calculated from Scatchard plots is relatively high: 572 and 826 fmol/mg protein. Competition studies indicate that this putative receptor is specific for natural and synthetic progestins but not for 5 alpha-dihydrotestosterone and cortisol. Similar progestin binding could not be found in normal human and rat testes.

Vitamins C and E in spontaneous abortion.

A group of 50 spontaneously aborting women was compared with the same number of pregnant women whose pregnancies terminated uneventfully. Biochemical parameters were studied and particular attention was paid to the vitamin C and E levels in both groups. A significantly higher percentage of aborting women had individual values of serum alpha-tocopherol above the 0.50 mg/100 ml normal limit. There was no significant difference in overall frequency distribution of vitamin C values in both groups. However, at the end of the first trimester the mean values of vitamin C were significantly higher in the aborting group. This could not be interpreted as an evidence of causal association.

Reproducibility of impedance cardiography hemodynamic measures in clinically stable heart failure patients.

BACKGROUND AND AIMS. One of the greatest challenges confronting physicians who are managing the care of patients with heart failure is to acquire objective data that signals treatment effectiveness and/or disease progression. The aim of this study was twofold: 1) to determine the extent to which (real time) impedance cardiography measurements obtained with a specific medical device (the BioZ) were reproducible in outpatients with clinically stable heart failure; and 2) to establish "normal" ranges of one week hemodynamic variability in this population of patients. Information of this nature would help clinical cardiologists and primary care practitioners to evaluate the implications of their patient's visit-to-visit hemodynamic variability. METHODS. A one group, prospective, time series design was used. The sample consisted of 62 patients who had clinically stable heart failure and who were being treated in an outpatient heart failure clinic at a university medical center. BioZ hemodynamic measures of cardiac output, contractility, and after load were obtained at five points in time: two, 10, and 60 minutes resting following a 40-50 foot walk on the first day and at two and 10 minutes resting following a 40-50 foot walk on the second day, one week later. RESULTS. Small but significant changes in cardiac output and cardiac index (mainly due to changes in heart rate) were seen during the 60-minute period on week one. Stroke index did not change during this period. In general, reproducibility between measurements taken on the same day and between days was quite good. Establishment of 95% confidence intervals helped define boundaries of variability in this population. Further clinical evaluation of the four patients whose values exceeded the 95% confidence intervals revealed unexpected, potentially relevant changes that could have accounted for their interday variability. Conclusion. The BioZ impedance cardiography measurements are responsive to hemodynamic activity-rest changes and are reproducible at a one week interval in clinically stable heart failure patients being treated in an outpatient clinic. Stroke index is a better measure of patient status than cardiac output or cardiac index. (c)2000 by CHF, Inc.
