Potential regulation of membrane trafficking by estrogen receptor alpha via induction of rab11 in uterine glands during implantation.

The steroid hormone estrogen profoundly influences the early events in the uterus leading to embryo implantation. It is thought that estrogen triggers the expression of a unique set of genes in the preimplantation endometrium that in turn control implantation. To identify these estrogen-induced genes, we used a delayed implantation model system in which embryo attachment to endometrium is dependent on estrogen administration. Using a mRNA differential display (DD) method, we isolated a number of cDNAs representing mRNAs whose expression is either turned on or turned off in response to an implantation-inducing dose of estrogen. We identified one of these cDNAs as that encoding rab11, a p21ras-like GTP-binding protein (G protein), which functions in the targeting of transport vesicles to the plasma membrane. In normal pregnant rats, rab11 mRNA was expressed at low levels on days 1-2 of pregnancy, but its expression was markedly enhanced (approximately 6- to 8-fold) between days 3-5 immediately before implantation. In situ hybridization and immunocytochemistry revealed that rab11 expression in the uterus was predominantly in the glandular epithelium. In ovariectomized rats, the expression of rab11 mRNA was induced in the endometrium in response to estrogen. To determine whether this effect of estrogen was mediated through its nuclear receptors, we examined rab11 expression in a transformed endometrial cell line, Ishikawa. In transient transfection experiments, we observed that overexpression of estrogen receptor (ER) alpha or beta induced endogenous rab11 mRNA in a hormone-dependent manner. ER bound to an antagonist, ICI 182,780, failed to activate this gene expression. These findings, together with the observation that ER alpha but not ER beta is detected in the glands of the preimplantation uterus, indicate that rab11 is one of the proteins that are specifically induced by estrogen-complexed ER alpha in rat endometrium at the onset of implantation. Our results imply that estrogen, which induces the synthesis of many growth factors and their receptors and other secretory proteins that are thought to be critical for implantation, may also facilitate their transport to the membrane and/or secretion by stimulating the expression of rab11, a component of the membrane-trafficking pathway. This study therefore provides novel insights into the diverse cellular mechanisms by which estrogen, acting via its nuclear receptors, may influence blastocyst implantation.

Collagen integrin receptors regulate early osteoblast differentiation induced by BMP-2.

Studies in several cell types indicate that the actions of integrin receptors for extracellular matrix and receptors for growth factors are synergistic in regulating cellular differentiation and function. We studied the roles of the alpha1beta1 and alpha2beta1 integrin collagen receptors in regulating the differentiation of 2T3 osteoblastic cells in response to bone morphogenetic protein (BMP)-2. The immortalized 2T3 cell line was established from the calvaria of mice transgenic for a BMP-2 promoter driving SV40 T-antigen. These cells require exogenous BMP-2, as well as ascorbic acid and beta-glycerolphosphate, for expression of a mature osteoblast phenotype and formation of a mineralized matrix. To determine how integrin receptors for collagen-I affect BMP-2 signaling, function-perturbing anti-rat alpha1 and/or alpha2 integrin subunit, or anti-type I collagen (Col-I), antibodies were added to human recombinant (hr)BMP-2-treated 2T3 cultures at confluence (C0) or at 4 or 8 days postconfluence (C4, C8). After 4 days of exposure to the antibodies, cultures were assayed for alkaline phosphatase (ALP) mRNA levels and enzyme activity and for cAMP production in response to parathyroid hormone. Addition of anti-collagen-I or both anti-integrin-alpha1 and -alpha2 antibodies to C0 cultures blocked expression of these early osteoblast markers by more than 90%, and also blocked mineralization (0.5-1.8% control) of these cells. In all cases, adding anti-alpha1 or anti-alpha2 antibodies separately produced partial effects, while their combined effect approached that of anti-collagen-I. When antibodies were added to more differentiated 2T3 cells, the inhibitory effects decreased. 2T3 cells carrying constitutively active BMP receptor (caBMPR-IB) showed elevated ALP activity without hrBMP-2; this constitutive activity was also suppressed by alpha1 and alpha2 integrin antibodies and by anti-Col-I antibody. Together, our data suggest that a signal(s) from collagen integrin receptors regulates the response to BMP downstream of BMPR-IB and upstream of the regulation of ALP mRNA and other early markers of osteoblast differentiation.

Development of immune complex trapping: experimental study of lymphoid follicles and germinal centers newly induced by exogenous stimulants in mouse popliteal lymph nodes.

The development of immune complex trapping in newly-induced lymphoid follicles of draining popliteal lymph nodes was investigated in young adult mice, which had been given bilateral injection of hemocyanin (KLH) or phytohemagglutinin (PHA), each absorbed onto alumina. HRP-anti-HRP immune complex was injected into the footpad 1 day before sacrifice. Using three series of semi-serial cryostat sections prepared from each popliteal node, the number of lymphoid follicles in each node was counted, and follicular localization of the in vivo injected and in vitro applied immune complexes in each follicle was determined. By day 5, a large germinal center had developed within each preexisting follicle. A large number of 'new' secondary follicles, each containing a small PNA-positive germinal center, appeared outside pre-existing follicles, from day 5 through day 11 in KLH-treated nodes, and from day 7 through day 14 in PHA-treated nodes. Shortly after their appearance, new secondary follicles showed no in vitro or in vivo trapping, but subsequently, many of the new follicles began to display in vitro trapping, at first weakly but later intensely. Occurrence of in vivo trapping in new follicles took some time and was first recognized when new follicles showed intense in vitro trapping. At day 21 or 25, many of the new follicles showed both in vitro and in vivo trapping. It was concluded that in lymph nodes treated with a stimulant, secondary follicles containing germinal centers can be formed de novo in the extrafollicular zone where the follicular trapping microenvironment is absent, but subsequently the microenvironment capable of trapping immune complexes develops at the site of formation of new follicles.

Rejoining of DNA double-strand breaks in Ku80-deficient mouse fibroblasts.

The role of Ku80 in the repair of DNA double-strand breaks (DSBs) was examined in fibroblasts derived from a Ku80 knockout mouse model described by Nussenzweig et al. (Nature 382, 551-555, 1996). Primary fibroblasts from Ku80+/+ and Ku80-/- mice were immortalized by transfection with plasmids containing either the human MYC proto-oncogene or the Simian virus 40 (SV40) T antigen and were used to measure induction and rejoining of DSBs after exposure to ionizing radiation. The number of DSBs in the cells was quantified by either asymmetric field-inversion gel electrophoresis (AFIGE) or clamped homogeneous electrical-field gel electrophoresis (CHEF). The latter method was introduced for a more reliable quantification of repair even when DNA degradation occurs in a fraction of the irradiated cell population during the postirradiation incubation time. The results confirm that Ku80-deficient mouse fibroblasts are sensitive to ionizing radiation and demonstrate that the increased radiosensitivity may result from a deficiency in DSB rejoining. The results further indicate that unless techniques are employed that allow for distinction between DNA degradation and DNA repair, erroneous conclusions may be drawn regarding the potential of cells to repair DSBs.

[Experimental research on in vivo gene-therapy of retinoblastoma].

OBJECTIVE: To investigate the expression condition of exogenous Rb gene in nude mouse vitreous transplanted retinoblastoma(RB) and its influence on the growth of the RB tumor in vivo. METHODS: Establishing the model of RB transplantation in nude mouse vitreous, followed by constructing the retrovirus vector of Rb gene and transfecting it into the model by using liposome. The expression condition of Rb gene was analyzed by immunohistochemistry and flow cytometry. The growth condition of the RB transplantation tumor was examined ophthalmologically and pathologically. RESULTS: The exogenous Rb gene expressed in the RB transplantation tumor for at least 7 days and partially suppressed the growth of the RB tumor. The level of suppression was related with the amount of the Rb protein expressed and the time to transfect the Rb gene after the RB transplantation tumor was established. CONCLUSION: The exogenous Rb gene can express in the RB transplantation tumor and can partially suppress the growth of the RB tumor in vivo.

Aspergillus mastoiditis in acquired immunodeficiency syndrome.

OBJECTIVE: This study aimed to analyze the clinical presentation, diagnosis, management, and results of treatment in a series of three patients with acquired immunodeficiency syndrome (AIDS) in whom Aspergillus mastoiditis developed. This study also aimed to compare these aspects of Aspergillus mastoiditis in patients with AIDS with three additional cases present in the current literature. A classification system for fungal infections of the ear and temporal bone is proposed. STUDY DESIGN: The study design was a retrospective case review. SETTING: The study was conducted at multiple tertiary referral centers. PATIENTS: Three individuals with diagnosed AIDS and mastoiditis resulting from culture-proven Aspergillus were studied. INTERVENTION: Patients were treated with both medical and surgical methods including local and systemic antimicrobial/antifungal agents and mastoidectomy. MAIN OUTCOME MEASURES: These measures included return of facial nerve function, control/resolution of disease, and survival. RESULTS: All three patients in this series initially presented with otalgia and otorrhea and intact facial nerve function. Facial nerve paresis developed in all patients between 5 and 12 weeks after initial symptoms. Paresis uniformly improved or resolved after mastoidectomy. Two patients treated with systemic antifungal therapy and prompt surgical debridement after development of facial palsy had full resolution of infection. One patient had full recovery of facial paresis and the other had partial recovery. The third patient was lost to follow-up after initial treatment with antimicrobials and surgery and died 3 months later without a clear etiology. CONCLUSIONS: Aspergillus mastoiditis is an unusual infection in patients with AIDS. Because of its rarity, fungal mastoiditis in immunocompromised individuals can result in a significant delay in diagnosis and treatment. The decision between conservative antimicrobial therapy and aggressive surgical treatment also can present a therapeutic challenge in the management of these life-threatening infections, especially in patients with existing immunodeficiency and illness. Early surgical debridement followed by antimicrobial therapy may be life preserving in this patient population.

Monogenic disorders of obesity and body fat distribution.

Recently, great progress has been made towards understanding the molecular basis of body fat regulation. Identification of mutations in several genes in spontaneous monogenic animal models of obesity and development of transgenic models have indicated the physiological roles of many genes in the regulation of body fat distribution. In humans, mutations in leptin, leptin receptor, prohormone convertase 1 (PC1), pro-opiomelanocortin (POMC), melanocortin 4-receptor (MC4-R), and peroxisome proliferator-activated receptor (PPAR) gamma2 genes have been described in patients with severe obesity. Most of these obesity disorders exhibit a distinct phenotype with varying degrees of hypothalamic and pituitary dysfunction and a recessive inheritance, whereas MC4-R mutation has a nonsyndromic phenotype with dominant inheritance. These mutations suggest the critical role of central signaling systems composed of leptin/leptin receptor and alpha-melanocyte stimulating hormone/MC4-R in human energy homeostasis. Although the genetic basis of monogenic disorders of body fat distribution, such as congenital generalized lipodystrophy and familial partial lipodystrophy, Dunnigan variety, is still unknown, the genes for these have recently been localized to chromosomes 9q34 and 1q21-22, respectively. The advances in our knowledge of the phenotypic manifestations and underlying molecular mechanisms of genetic body fat disorders may lead to better treatment and prevention of obesity and other disorders of adipose tissue in the future.

Abdominal computed tomography scan in pediatric blunt abdominal trauma.

The purpose of this study was to evaluate the role of abdominal CT scans in pediatric patients and correlate the findings with the clinical examination. A 2-year retrospective review of 88 patients with an abdominal CT scan after blunt trauma was performed. Seventy-two patients were identified with complete clinical examination data available. In its ability to predict the need for surgery, the CT scan had a sensitivity of 67 per cent and a negative predictive value of 98.7 per cent. The combination of the clinical examination and the CT scan findings did not miss any significant injuries. No patient with a soft, nontender abdomen and a negative CT scan required an abdominal operation. We conclude that the CT scan alone may miss clinically significant injuries. In blunt abdominal trauma in the pediatric population, the CT scan findings should be coupled with the clinical examination to ensure that no significant abdominal injuries are missed.

Analgesic effects of diclofenac suppository and injection after preoperative administration.

Diclofenac sodium (100 mg) has been introduced in the Caribbean as a suppository formulation. In a randomized single-blind (observer-blind) clinical trial, the postoperative analgesic efficacy of diclofenac administered either as a conventional intramuscular injection (75 mg) or as the available suppository formulation (100 mg) was studied in 44 adult male patients undergoing herniorrhaphy in same day surgery. Diclofenac was administered preoperatively at induction of anesthesia to patients (grades ASA I and II) after they had given informed consent. Evaluation of analgesia on the visual analog scale (VAS) did not differ significantly between the two treated groups at three assessment times: on admission to the recovery room, the postoperative ward and at discharge. The times for requests for additional analgesia and the number of patients requesting further analgesia did not differ. Patients who received the suppository were discharged earlier than those who received the injection (40 min vs. 65 min p = 0.02). This preliminary study of the two marketed formulations of diclofenac demonstrated that both preparations provided equivalent analgesia but patients who received the suppository preparation were discharged earlier.

[The effect of intradermal injection of collagenase in implanted hypertrophic scar in nude mice].

OBJECTIVE: Effects of collagen degradation in hypertrophic scar by intradermal injection of collagenase were observed. METHODS: Hypertrophic scar was implanted into nude mice and collagenase was injected locally. Light and electron microscopic observations were carried out. RESULTS: Collagen degradation in hypertrophic scar by intradermal injection of collagenase was observed. The implanted scars became soft, thin and small. CONCLUSION: These results suggested that local injection of collagenase may be a useful therapeutic tool for hypertrophic scar.

[Inquiring the causes of recurrence of hepatocellular carcinoma after surgical resection].

OBJECTIVE: To analyze the causes of recurrence of hepatocellular carcinoma (HCC) after resection according to pathologic findings of the resected primary tumor and angiographic features of the recurrent tumor. METHODS: In this series, 142 cases with recurrent HCC were analyzed with respect to (1) size, number, gross and histologic findings of the primary tumor, (2) time when recurrence occurred, (3) size, number, blood supply, staining property of, and deposition of lipiodol oil in the recurrent tumor. Following angiography, arterial embolization was performed. RESULTS: In 101 of the 142(71.1%) cases, the primary tumor was > 5 cm in diameter, and in 41 cases (28.9%) it was < 5 cm. In 67.7% of the cases, the capsule of the primary tumor was incomplete or absent. In 47 cases (33.1%), satellite tumor nodules were seen during operation but they were seen on pathologic sections in 94 cases (66.2%). Tumor thrombus was present in the portal vein in 26 (18.3%) and 121 cases (85.2%) during operation and on pathologic examination, respectively. In the majority of the cases (99/142), recurrence occurred within 6 months after operation. The recurrent foci consisted of multiple tumor nodules of < 5 cm in 68.3% of the cases. On angiography, the recurrent tumors were rich in blood supply with good deposition of lipiodol. CONCLUSION: Recurrence is apt to occur in HCC patients with large (> 5 cm) primary tumor which has incomplete or no capsule, satellite tumor nodules and portal vein tumor thrombus. It is suggested to perform angiography 1-2 months after surgery to detect early recurrence and, if confirmed, the patients can be treated by transcatheter arterial chemo-embolization.

[Oxygen utilization coefficient in predicting the severity of critical illness].

OBJECTIVE: To evaluate the role of oxygen utilization coefficient (O(2)UC ) in predicting the severity of various diseases. METHODS: Gas analysis of arterial blood and central venous blood and calculation of O(2)UC [O(2)UC = (SaO(2) - SvO(2))/SaO(2)] were carried out in different groups (group 1, 16 cases of trauma, all survived; group 2, 18 emergency cases undergoing cardiopulmonary resuscitation(CPR); group 3, 13 cases undergoing CPR in intensive care unit, all died; group 4, 13 critically ill patients. RESULTS: O(2)UC was 0.30 +/- 0.08, 0.67 +/- 0.27 and 0.60 +/- 0.15 in the groups 1, 2 and 3 respectively. The difference between group 1 and 2 was significant (P < 0.001). O(2)UC persisted to increase, and maintained high for a long time (more than 8 approximately 12 hours), the prognosis of the patients was bad. The critical limitation of O(2)UC was 0.40. In group 4 O(2)UC was closely correlated with the severity of clinical condition. CONCLUSION: O(2)UC is a convenient, reliable and sensible predictor in the treatment of critically ill patients.

[Greater reduction of urinary albumin excretion in hypertensive patients with perindopril than nifedipine].

OBJECTIVE: To examine the effect of perindopril and nifedipine on renal function in patients with essential hypertension (EH). METHODS: 52 cases with EH were divided randomly into two groups: perindopril group (4mg qd, n = 26); nifedipine group (10 mg tid, n = 26). Treatment period lasted for 24 weeks. Parameters of renal function were measured before and after treatment. RESULTS: (1) After treatment, the arterial pressure decreased in a similar way in the two groups, urinary albumin excretion was significantly less after treatment in the perindopril group [urinary albumin excretion/24 h (mg/24 h): 66.7 +/- 25.9 vs 104.5 +/- 41.8, P < 0.01; urine protein/24h (g/L): 0.17 +/- 0.039 vs 0.24 +/- 0.064, P < 0.01], GFR (ml/min) was higher in the perindopril group after treatment (125.5 +/- 15.2 vs 112.2 +/- 13.2, P < 0.05). No significant changes were observed with nifedipine (P < 0.05). (2) No significant correlation between the decrement of urinary albumin excretion and of SBP, DBP was found in both groups (P > 0.05). CONCLUSION: It is shown that perindopril, but not nifedipine, is able to reduce urinary excretion of albumin in patients with essential hypertension independently of its effective antihypertensive property. It is probable that the positive effect of perindopril on microalbuminuria is attributable to modification in intrarenal hemodynamics or to change in glomerular permeability.

[The value of 99mTc-MIBI image axillary lymph node in selecting operative method for breast cancer].

OBJECTIVE: To retain axillary lymph node not infiltrated by cancer cell. METHODS: Breast cancer was diagnosed by fine needle aspiration cytology (FNA) or tumor biopsy before operation. Axillary lymph node was examined by surgery and (99m)Tc-MIBI image. Pathological examination of the axillary tissue after operation is used as gold standard in comparison of the two examination methods. RESULTS: The sensitivity, specificity, and accuracy of surgical examination are lower than those of (99m)Tc-MIBI image (P < 0.05). CONCLUSIONS: (99m)Tc-MIBI image combined with surgical examination can be used as a method for retaining axillary lymph node.

[Experiment and clinical application of targeting treatment with adriamycin magnetic albumin microspheres in human gastric carcinoma].

OBJECTIVE: To study the pharmacokinetics in experimental animals and observe the concurrent anticancer effects in human advanced gastric cancer as an adjuvant chemotherapy, when adriamycin magnetic albumin microspheres (ADM-MAM) were combined with external static magnetic fields. METHODS: The drug concentration of targeting tissues in the animals and the stability of ADM-MAM in human gastric juices were determined. Clinical and histopathological changes were observed in 55 cases of advanced gastric carcinomas after targeting treatment. RESULTS: The peak concentrations of the targeting tissue were highest after ADM-MAMs were administered for 2 hours and remained at the high level for a long time. ADM-MAM maintained its stability in human gastric juices. Targeting treatment improved the patients' symptoms, raised the rated of tumor resection, prolonged patients' lifetime, and enhanced histopathological effects (P < 0.01). CONCLUSIONS: The administration of ADM-MAM combined with the external static magnetic fields is effective for targeting location and is of clinical value as a preoperative adjuvant chemotherapy for human advanced gastric carcinoma.

[Chemical constituents of fruits of Gardenia sootepensis Hutch].

OBJECTIVE: To demonstrate the chemical constituents of the fruits of Gardenia sootepensis. METHOD: Column chromatographic technology was employed to separate the water soluble part, and the compounds obtained were elucidated by spectral and chemical analysis. RESULT: Ten compounds have been isolated, seven of which elucidated respectively as D-mannitol, beta-sitosterol, deacetylasperulosidic acid methyl ester, geniposidic acid, geniposide, scandoside methyl ester and dimeric molecule of quinide. CONCLUSION: All the compounds were obtained from this plant for the first time.

[Studies on chemical constituents and pharmacological effects of the stem of Cynomorium songaricum Rupr].

OBJECTIVE: In order to exploit the medicinal resources and provide a scientific evidence for the clinical use and quality of Cynomorium songaricum control, the chemical constituents and pharmacological effects of the stem of the plant were studied preliminarily. METHOD: The chemical constituents were studied by spectral analysis and chemical method. The pharmacological effects were studied by an experiment on the defecation and deficiency of kidney function. RESULT AND CONCLUSION: Seven compounds were isolated from the stem of C. songaricum and identified as beta-sitosterol(I), palmitic acid (II), ursloic acid (III), daucosterol (IV), (-)-catechin (VI), naringenin-4'-O-pyranogluoside (VII) and succinicyacid (XII). Compounds VI and XII were isolated from this species for the first time. The pharmacological results show that the different extracts facilitate catharsis and replenish the deficiency of kidney yang.
