Implications of metronidazole pharmacodynamics for therapy of trichomoniasis.

A liquid chromatographic technique for the detection of metronidazole (MDZ) and its major metabolites, combined with a vaginal sampling technique employing extraction of drug from vaginal swabs, was used to evaluate the concentration of MDZ in the vaginal fluid during therapy. Wide variation in the absolute concentration of unchanged drug was noted, although the average vaginal levels were about half the serum concentration 2 h after treatment had been initiated and comparable to the serum levels 6 and 24 h after treatment. Oral administration of MDZ invariably resulted in the presence of the drug in the serum and urine but the drug was not always detected in the vaginal secretions. The presence of the relatively inactive acetyl metabolite in the vaginal fluid of patients who failed to respond to therapy for trichomoniasis suggested the possible role of drug modification as a contributing factor in the lack of therapeutic success. The presence of MDZ in women in whom the cervix had previously been surgically removed attests to the transport or transudation of the drug across the vaginal epithelium without involvement of the secretory activity of the cervix.

Pharmacokinetics of piperacillin in the postpartum patient.

Six women were given 1 g of piperacillin on the 2nd postpartum day and serum concentrations of the drug were measured at 0.5, 1, 2, and 4 h after infusion. The same women returned for an identical test 6 months later and hence served as their own controls. A second group of 6 women were treated identically except that they received the first dose of piperacillin on the 3rd postpartum day. The serum levels of antibiotic were consistently lower during the puerperium than at 6 months. The levels obtained at 2 compared to 3 days post partum were similar, but were about one half the levels obtained at 6 months post partum. These results indicate that antibiotic therapy for postpartum endomyometritis may need to take account of the fact that the altered pharmacokinetics of pregnancy persists into the puerperium. If used in this setting, the dose of piperacillin may need to be increased by 50% over the normal adult dose.

Critical assessment of antibacterial properties of human amniotic fluid.

A low molecular weight fraction of human amniotic fluid was found to be rapidly bactericidal for Escherichia coli ATCC 33908. Incubation in an atmosphere of CO2 but not N2 diminished the antibacterial effect suggesting that loss of CO2 and change in pH may be partly responsible for the observed antibacterial activity of the low molecular weight fraction of amniotic fluid. Incubation in pH 8.5 buffers did not duplicate the rapid killing of bacteria observed in the low molecular weight fraction of amniotic fluid, suggesting that elevated pH and an antibacterial factor from amniotic fluid act synergistically. The pH change in amniotic fluid which occurs in room atmosphere may account for some of the antibacterial effect which has been reported for amniotic fluid incubated aerobically.

Effects of postcesarean section febrile morbidity on subsequent fertility.

The records of 1319 patients undergoing cesarean section were evaluated to determine the relationship between postoperative febrile morbidity and fertility during the five years after the operation. Infection was significantly more common among patients undergoing primary as compared with repeat cesarean section and was three times more frequent among patients with membranes ruptured for 12 hours or more compared with those with membranes ruptured less than 12 hours. After correction of the data for voluntary infertility, neither postcesarean section endometritis nor pelvic cellulitis had a demonstrable effect on the rate of pregnancy during the five years after cesarean delivery, but pelvic abscess was associated with an apparent reduction in fertility as reflected by the frequency of pregnancy among these individuals, which was approximately half the rate observed in other infected and noninfected individuals in this study.

Antibacterial activity in amniotic fluid from west virginia women.

Amniotic fluids from women in West Virginia were evaluated for the presence of antibacterial activity by a bioassay technique. 41% of 32 third trimester samples were inhibitory. Evaluation of additional test organisms emphasized differences in susceptibility of various bacterial strains; two isolates of Escherichia coli were found which were more sensitive to the phosphate-sensitive antibacterial system of amniotic fluid than in the initial test organism. The stated prevalence of inhibition may therefore be considered a conservative estimate.

Blood transfusion reaction in a patient with immunoglobulin A deficiency.

Selective deficiency of serum IgA is the most common immunodeficiency in humans; when immunodeficient individuals receive blood transfusions a severe anaphylactoid reaction can develop. The present report describes such a patient. After the transfusion reaction a hemagglutination inhibition assay revealed that her blood contained less than 1.0 micrograms/ml of IgA and an anti-IgA antibody that reacted with the 2 IgA proteins, isotypes IgA1 and IgA2. Immediately after the reaction the patient's serum anti-IgA antibody titer was 1:16,384, and when reevaluated 5 weeks later it was 1:8000. All 3 of her children were shown to be IgA deficient, and 2 of them had antibodies against IgA2. This type of anaphylactoid reaction can be avoided by transfusing blood from IgA-deficient donors, frozen deglycerolized red cells, or red cells that have been washed several times to extract all IgA proteins.

Infectious complications of cervical cerclage.

A study of 115 gravid patients who required cervical cerclage indicated that the optimum time to perform the surgical procedure was between the fourteenth and eighteenth weeks. If the operation is delayed until later in the second trimester, the incidence of chorioamnionitis is increased 2.6-fold, and premature rupture of the membranes prior to the thirty-second week of gestation is trebled. Amniocentesis performed in nine patients with chorioamnionitis and intact membranes after cerclage demonstrated the polymicrobial nature of the amniotic fluid infection. The isolation of such organisms at Bacteroides bivius, Eubacterium lentum, and Staphylococcus epidermidis indicates their potential importance in chorioamnionitis. Early recourse to amniocentesis is recommended when chorioamnionitis is suspected, and, moreover, prophylactic antibiotics should be used when cerclage is performed after the eighteenth week of pregnancy.

Biochemical characterization of phosphoglucose isomerase and genetic variants from mouse and Drosophila melanogaster.

A simple and unique procedure was developed to purify phosphoglucose isomerase variants from the whole mouse body extracts and Drosophila homogenate. It involved the use of an 8-(6-aminohexyl)-amino-ATP-Sepharose column followed by a preparative isoelectric focusing. In each case, the enzyme in the homogenate was adsorbed by ionic interaction on the ATP-Sepharose column. Substantial purification was achieved by the affinity elution with the substrate-glucose-6-phosphate. Mouse and Drosophila phosphoglucose isomerase as well as the corresponding variants were shown to be dimers of similar molecular weight and to exhibit similar kinetic properties. The isoelectric points for the variants from DBA/2J and C57BL/6J mice were determined to be 8.4 and 8.7 respectively, while they were 6.8 and 6.3 respectively for Drosophila and 4/4 variants. Differential thermal stability was observed for the two mouse variants but not for the Drosophila ones. Amino acid composition analysis was performed for both mouse and Drosophila enzymes. Rabbit antisera for mouse (DBA/2J) and Drosophila (2/2) enzymes were raised. Within each species, complete immunological identity was observed between the variants. The antisera were used to characterize the null mutants of phosphoglucose isomerase identified in the mouse and Drosophila populations. By rocket immunoelectrophoresis, the null allele of the naturally occurring heterozygous null variant of Drosophila was shown to express no cross-reacting materials (CRM).
