Embolization of the hypogastric arteries in the control of massive vaginal hemorrhage.

Percutaneous, transcatheter Gelfoam embolization of the hypogastric arteries was utilized in a patient to control vaginal hemorrhage secondary to far-advanced carcinoma of the cervix. The advantages of this method are outlined, and consideration of its use is recommended for patients in similar situations. The various types of particulate material used in transcatheter embolization and the situations in which they have been used are reviewed.

Diagnostic features of right ventricular myxoma.

The clinical diagnostic features of right ventricular myxoma are described in a recent patient and related to the 15 cases previously reported. The presence of a pulmonic systolic ejection murmur with a delayed (120 to 140 msec.) and accentuated pulmonic second sound, or calcification in the region of the right ventricular outflow tract should suggest this lesion. Cardiac catheterization with angiocardiography in diagnostic. Ga scanning may assist in the diagnosis and followup after surgical removal of the myxoma. Early surgical removal will avoid the possibility of sudden death.

The National Health Planning Act: significance for mental health agencies.

In examining the National Health Planning and Resources Development Act, the authors identify nine areas that have special meaning for those responsible for planning, administration, and delivery of mental health services. Believing that mental health planning can be done more effectively by mental health agencies, they recommend a series of steps that can be taken by mental health administrators to maintain responsibility for planning their programs while ensuring close coordination with general health planning activities. It is their conviction that mental health authorities should commit themselves to the planning process and put their commitment to work by recruiting knowledgeable and skilled planning staffs.

Flow microfluorometric patterns of human bone marrow and tumor cells in response to cancer chemotherapy.

A rapid propidium iodide staining method was used for analysis of single-cell suspensions of bone marrow and tumor biopsies by flow microfluorometry. With this technique, information on the proliferative status of target tissues can be obtained within 10 min of sample removal. DNA histograms and labeling index of sequential bone marrow biopsies from a patient with Stage IV diffuse lymphocytic leukemia and treated with 1-beta-D-arabinofuranosylcytosine infusion showed pronounced reduction in the percentage of cycling cells. In contrast, sequential tumor biopsies from a melanoma patient on methotrexate-citrovorum factor rescue therapy showed no changes. In sequential bone marrow biopsies of 3 patients on high-dose methotrexate-citrovorum factor rescue, initial accumulation of cells in G1-S (Day 1) was followed by a significant proliferative response (Days 4 to 7) and return to pretherapy values. In contrast, no recovery similar to that of the bone marrow was seen in tumor cells.

Association of exogenous estrogen and endometrial carcinoma.

To determine the association between the incidence of endometrial cancer and the use of estrogen in menopausal and post-menopausal women, we retrospectively compared 317 patients with adenocarcinoma of the endometrium with an equal number of matched controls having other gynecologic neoplasms; 152 patients used estrogen, as compared to 54 of 317 controls. Thus, the risk of endometrial cancer was 4.5 times greater among women exposed to estrogen therapy. When estrogen use was adjusted for concomitant variables such as obesity, hypertension, diabetes, parity, referral pattern, age at diagnosis, year of diagnosis and other gynecologic neoplasms, the magnitude of the increased relative risk was associated with several of these variables, and was highest in patients without obesity and hypertension. Exogenous estrogen therapy is associated with an increased risk of endometrial carcinoma, but this increased relative risk is less apparent in patients with physiologic characteristics previously associated with an increased risk.

A clinically useful coding system for normal coronary artery anatomy.

Cine coronary arteriograms of 121 patients with normal coronary arteriograms with and without associated valvular or myocardial disease were reviewed to describe normal coronary artery anatomy and the distribution of potentially graftable vessels. Coronary arterio grams were divided into right, mixed, and left interior emphasis systems, depending upon the blood supply to the inferior surface of the left ventricle. Whether a coronary artery at its origin was less than 50%, between 50-70% of the left anterior descending coronary artery (LAD) at its origin was described in a simple lettering and numbering code to allow easy comparison. The diameter of the LAD at its origin averaged 4.2 mm. All vessels greater than 70% of the LAD were clearly graftable. The frequency (in percent) with which that vessel was greater than 70% of the LAD in right, mixed, and left systems, respectively, was as follows: right coronary artery 100. 100. 0. LAD 100. 100. 100. intermediate artery 8.7 12.5 33.3 obtuse marginal artery 53.7 43.7 44.4 atrioventricular groove artery 0. 56.2 100. diagonal branch of the LAD 15. 6.3 11.1. This simple method of classification is recommended to provide more accurate comparison in published series and to assist the coronary arteriographer in his interpretations.

Investigation of possible oncogenic action of zinc polycarboxylate cement by implantation in mice and hamsters.

Powdered, sterilized zinc polycarboxylate was implanted subcutaneously in 56 young hamsters and 54 mice, using equal numbers of each sex, with 24 sham operated hamsters and 26 mice as controls. The hamsters were sacrificed after 15 months and the mice after 12. Implant material was recovered from one of 42 surviving mice and from 16 of 43 surviving hamsters. Benign adenomas of the gut, thyroid, sebaceous glands and ovary were found in 3 experimental mice and in one control. Benign adrenal adenomas were found in 3 experimental hamsters and in one control. One control hamster developed a rhabdomyosarcoma in a limb. One experimental hamster developed a leiomysarcoma in close relation to an implant.

Selenium content of food consumed by Canadians.

Four composite diets from three cities, each representing the daily per capita consumption of foods in Canada, contained on analysis 191, 220, 113, and 150 mug selenium. Cereals provided the most selenium (62-112 mug) followed by meat, poultry, and fish (25-90 mug) and dairy products (5-25 mug). The average daily intake of selenium in Canada was also calculated from published analytical data and the per capita disappearance of unprepared foods. The total intake was 197 mug/day, and the major sources were wheat flour (98 mug), pork (21 mug), poultry products (24 mug), and fish (17 mug). Because the average diet is rich in selenium, the possibility of a deficiency in the adult is considered to be remote. Milk is relatively low in selenium, and thus the greatest deprivation in humans would occur during infancy.
