Low-dose streptokinase thrombolysis of axillary-subclavian vein thrombosis.

Nine patients with 10 episodes of acute axillary-subclavian vein thrombosis were treated with local low-dose streptokinase infusion. All patients except one were heparinized during the procedure with conversion to oral anticoagulation upon completion of the infusion. Recanalization was achieved in 7 patients (70%). Thrombosis recurred in 1, and in 2 patients the procedure was unsuccessful. No major complications were encountered. We concluded that local low-dose streptokinase infusion is a safe, simple, and effective method for treating acute axillary-subclavian vein thrombosis.

Immunohistochemical study of alkaline phosphatase in growth plate cartilage, bone, and fetal calf isolated chondrocytes using monoclonal antibodies.

Monoclonal antibodies prepared against alkaline phosphatase of matrix vesicles from fetal bovine growth plate cartilage were used to study the distribution of alkaline phosphatase in bovine bone and cartilage. Primary chondrocyte cultures from bovine growth plate cartilage were prepared and expression of this antigen was followed during 3 d long cultured period. Same monoclonal anti-alkaline phosphatase antibodies recognized both osteoblasts in metaphyseal bone as well as proliferating and hypertrophic chondrocytes in growth plate cartilage. Extracellular reaction was observed in lower areas of growth plate cartilage after hyaluronidase digestion of tissue sections. In isolated growth plate chondrocytes strong membrane associated reaction as well as weaker intracytoplasmic reaction were noticed and especially in large hypertrophic cells membrane reaction was clustered showing "capping-like" phenomenon. These results indicates the high similarity of alkaline phosphatase in bone and cartilage and its uneven distribution in chondrocyte plasma membrane.

A multicenter, randomized, placebo-controlled trial of a new form of intravenous recombinant tissue-type plasminogen activator (activase) in acute myocardial infarction.

A new, predominantly single chain preparation of recombinant tissue-type plasminogen activator was evaluated to determine coronary thrombolytic efficacy in 100 patients with acute myocardial infarction. At 3.6 +/- 1.2 hours (mean +/- SD) from symptom onset, patients received either intravenous tissue plasminogen activator (1.25 mg/kg body weight over 3 hours) or placebo on a 3:1 randomized, double-blind basis. Coronary angiography, performed 68 +/- 13 minutes after initiation of the study drug infusion, demonstrated patency of the infarct-related artery in 40 (57%) of 70 patients in the tissue plasminogen activator group compared with 3 (13%) of 23 patients in the placebo group (p less than 0.001). Patients in the placebo group were then eligible to receive intracoronary streptokinase. At 90 minutes the patency was observed in 49 (69%) of 71 tissue plasminogen activator patients compared with 5 (24%) of 21 placebo patients (p less than 0.001). At 120 minutes patency was observed in 59 (79%) of 75 patients of the tissue plasminogen activator group and in 10 (40%) of 25 in the intracoronary streptokinase/placebo group. A nadir value of less than 100 mg/dl fibrinogen occurred in 8 (11%) of 73 patients receiving tissue plasminogen activator versus 8 (40%) of 20 patients treated with intracoronary streptokinase (p = 0.002). Moderate or severe bleeding episodes occurred in 39% of patients treated with tissue plasminogen activator compared with 32% of patients who received placebo/intracoronary streptokinase (p = NS). Thus, this tissue plasminogen activator preparation achieves a high rate of recanalization and, at the doses employed, exhibits increased fibrinogen sparing compared with intracoronary streptokinase.

Teat burns in dairy cattle--the prognosis and effect of treatment.

Twenty cows and 20 uncalved 20 month old heifers with severely burnt teats were studied. Ten of each group received topical treatment with a mixture of lanoline, cetrimide and dimethyl sulphoxide or vitamin A and petroleum jelly, respectively. Prognosis for survival and future normal milking ability was poor in heifers (8 subsequently normal of 20 assessed) but moderate in cows (9 cows with normal and 5 cows with partial machine milking ability of 18 cows fully assessed). Treatment did not significantly increase the rate of epithelialisation of wounds. Topical treatment had no effect on the prognosis of heifers. However treated cows were more likely than control cows to survive and to show adequate teat function following subsequent calving. Severe clinical mastitis prior to calving, teat distortion and teat canal constriction were common sequelae. Recommendations on the management of heifers and cows with severely burnt teats are made.

Effect of 1,25 dihydroxyvitamin D3 on the calcium and magnesium metabolism of lactating cows.

Administration of 1,25 dihydroxyvitamin D3 (1,25(OH)2D3) to lactating dairy cows resulted in increased dietary calcium absorption and elevated concentrations of plasma calcium. Dietary magnesium absorption was unaffected by 1,25(OH)2D3 however, plasma magnesium concentration was depressed. Injections of 1,25(OH)2D3 were effective in elevating plasma calcium concentrations in both normal and hypomagnesaemic cows. This indicates a potential use for 1,25(OH)2D3 to prevent and treat hypocalcaemic cows with or without concurrent hypomagnesaemia.

Acute myocardial infarction temporally related to cocaine use. Clinical, angiographic, and pathophysiologic observations.

Ischemic chest pain syndromes and myocardial infarction occurred within minutes to hours of cocaine use in nine persons ages 23 to 39 years. Five developed symptoms after taking cocaine intranasally; three, after intravenous use; and one, after smoking cocaine. Four were habitual users and five were recreational users; eight also smoked cigarettes heavily. Ischemic syndromes recurred in five who continued to use cocaine. Coronary arteriography showed an abnormal infarct-related vessel (more than 50% stenosis, total occlusion, or intraluminal thrombus) in seven patients. The noninfarct-related vessels were normal in eight patients. The left anterior descending coronary artery and the anteroapical left-ventricular wall were involved in all patients. After three patients had successful thrombolysis of the obstructed infarct-related vessel, angiography showed a normal underlying vessel.

Local area networks. Temporary reluctance to grow.

While the impact of IBM's move to improved connectivity and processing distribution may well become a compelling factor in the LAN marketplace, the current LAN movement is driven by departmental needs for improved performance and access. The introduction of the PS/2 product line has been a significant factor over the past several months. The announcement will continue to cause turmoil in the LAN industry as vendors move to make their offerings compatible with new technologies. However, while these factors will have a marked impact on the use of LANs, it is doubtful that they will cause the LAN movement to escalate rapidly in the near future.

Emergency coronary bypass for cardiogenic shock.

Emergency coronary bypass for cardiogenic shock has been associated with a high operative mortality. From January 1983 through March 1986, 69 patients at Crawford W. Long Hospital underwent emergency coronary artery bypass. Seventeen of 69 were in shock, 15 with hypotension requiring treatment (intra-aortic balloon pump in 10; catecholamines in six). The other two patients had a low cardiac index and a pulmonary capillary wedge pressure greater than 25 mm Hg. Of these patients, nine presented with acute infarction, four with failed angioplasty, and four with uncontrollable angina. Four patients required cardiopulmonary resuscitation. After operation, 94% of the patients required catecholamine support and 71% were treated with an intra-aortic balloon pump. There were two hospital deaths (12%). The median postoperative stay for survivors was 9 days. Major complications occurred in 47%. Follow-up (100%, mean 20.5 months) revealed no late deaths, a 3 year survival of 88 +/- 8%, and a functional class of I in six patients, II in seven patients, and III in two patients. The nine patients who were working before operation all returned to work. Of the 52 emergency coronary bypass patients without shock, one patient died in the hospital (2%), 52% required catecholamines (p less than .05 vs shock group by chi-square analysis), and 12% required an intra-aortic balloon pump after operation (p less than .05 vs shock group by chi-square analysis). Median stay was 8 days. Complications occurred in 13% (p less than .05 vs shock group by chi-square analysis). Three year survival was 91 +/- 4%.(ABSTRACT TRUNCATED AT 250 WORDS)

Purification of bone alkaline phosphatase from human osteosarcoma.

Purification of human bone alkaline phosphatase, derived from human osteosarcoma tissue, has been carried to electrophoretic homogeneity. The purification procedure involved three major steps: (1) chromatography on hydroxylapatite; (2) ion exchange chromatography; and (3) gel filtration. The resultant purified enzyme is a glycoprotein, has a molecular weight of approximately 80,000 (consistent with previous reports for the bone isoenzyme), and is characteristically inhibited by modest heat (56 degrees C, 30 min) and L-homoarginine but not by L-phenylalanine. The isolation and purification procedure described can lead to the production of significant amounts of highly purified bone alkaline phosphatase. Purified ALP can be used for an analysis of minor structural differences that appear to exist between the bone, liver and kidney isoenzymes. Such information could lead to the development of a clinical diagnostic procedure specifically for bone alkaline phosphatase.

Immunoelectron microscopic localization of carbonic anhydrase III in rat skeletal muscle.

The subcellular distribution of carbonic anhydrase III in rat soleus and vastus lateralis muscles was studied using an immunogold technique. The enzyme protein was found to be distributed diffusely in the cytoplasm of skeletal muscle cells. Red skeletal muscle (mainly type I fibers) revealed very strong immunogold staining whereas in white muscle (mainly type II fibers) gold particles were almost completely absent. No immunoreaction was observed in mitochondria or in other intracellular organelles.

Embolization of the left gastric artery in the absence of angiographic extravasation.

Nine high-risk patients with endoscopic evidence of bleeding from the left gastric artery territory without angiographic extravasation were embolized to prevent rebleeding. Three were embolized with Gelfoam particles, 1 with Gianturco coils, and 5 with Gelfoam plus coils. Three patients rebled; 2 were embolized with Gelfoam only, and the third with coils only. No rebleeding occurred in the 5 patients treated with Gelfoam plus coils. No ischemic complications occurred. We conclude that embolization of the left gastric artery, despite absence of extravasation, is effective in preventing rebleeding in high-risk patients and that both Gelfoam and coils be used.

Development and cross-reactive properties of monoclonal antibodies to bovine matrix vesicle alkaline phosphatase.

Alkaline phosphatase (ALPase), concentrated in the membranes of matrix vesicles, is believed to play a role in initial calcification. To further purify, characterize, and identify this enzyme in tissue, a monoclonal antibody was developed against the ALPase of isolated fetal calf matrix vesicles. Splenic lymphocytes derived from mice immunized with Sepharose 6B-purified fetal calf matrix vesicle ALPase were fused with mouse plasmacytoma cells (line X63-Ag-8.653) using standard hybridoma technology. Hyperimmune sera and hybridoma culture supernatants were screened for the presence of specific antibody using a newly developed double-immunosorbent assay in which putative antibody is added to microtiter plate wells precoated with affinity-purified rabbit antimouse immunoglobulin. After incubation and washing, partially purified fetal calf matrix vesicle ALPase is added to each well. The enzyme adheres only to wells that contain specific anti-ALPase antibody. These wells are identified by adding the enzyme substrate p-nitrophenyl phosphate and reading the wells in a plate-reading spectrophotometer at 405 nm. A hybridoma-producing specific antibody was subsequently cloned and grown as ascities-producing tumors in pristane-primed mice. Ouchterlony analysis indicated that the cell line secretes an immunoglobulin of IgG1 class. This antibody reacts specifically with ALPase derived from calf matrix vesicles and cross-reacts with ALPase of bovine kidney, liver, and placental origin and human bone but does not cross-react with bovine intestinal ALPase or ALPase derived from matrix vesicles isolated from rachitic rat growth plate cartilage.

Information systems: the direction of things to come.

In the past, very little emphasis was placed on the development of high quality information systems support producing useful information. However, in an increasingly competitive environment, managers will need good information to make sound business decisions and to survive. Good information will be the result of an information system that is easy to use and can gather information from many different sources. Hence, in the future, information systems will move in the direction of decision support assistance, integrated patient care systems, stand-alone application systems, and increased access and integration, to help the manager in the decision-making process.

Crystallization of a Fe,Zn superoxide dismutase from the archaebacterium Thermoplasma acidophilium.

The novel Fe,Zn superoxide dismutase from the archaebacterium Thermoplasma acidophilium has been crystallized in space groups P1, P2(1) and P2(1)2(1)2, with 2,4 and 1/2 of an 84,000 Mr tetramer, respectively, estimated to be in the asymmetric unit of the unit cell. The orthorhombic crystals, which have unit cell dimensions a = 84.2 A, b = 72.7 A, c = 67.8 A, diffract X-rays to at least 2.0 A and are suitable for a determination of the three-dimensional structure of the Fe,Zn superoxide dismutase.
