Surgical treatment of displaced olecranon fractures by tension band wiring technique.

Forty-five displaced olecranon fractures including 14 accompanying dislocated radial heads and seven radial head fractures were treated over a 13-year period by the tension band wiring technique. The use of supplemental internal fixation when necessary allows excellent results with the use of this technique, even in the presence of severe comminution or radial head dislocation. Primary silicone radial head implants fractured in all three patients in which they were used, necessitating repeat surgery in two patients to date. While loss of motion in terminal extension was a common aftermath of displaced olecranon fracture (59%), it was usually minor and functionally insignificant. True Kirschner-wire migration was not a common problem and can probably be eliminated by proper technique. The presence of gaps in the intraarticular surface of the semilunar notch of the ulna produced no ill effects and was compatible with excellent results. If only those cases with isolated olecranon fractures in this series are considered, there were good and excellent results in 29 of 30 cases (97%). Excision of the olecranon fragment(s) should be reserved for those cases when anatomic restoration cannot be achieved with internal fixation.

High-energy roller injuries to the upper extremity.

Eleven cases of high-energy industrial roller injuries treated between 1980 and 1984 were retrospectively reviewed. The dominant extremity was affected in nine. Six patients sustained fractures and/or dislocations, and three of these patients required fasciotomies for clinical signs of impending compartment syndromes. All fracture/dislocations, with the exception of a scapula fracture, anterior dislocation of a thumb interphalangeal joint, and a fractured coronoid process of the ulna, required open reduction with internal fixation. Three patients required split-thickness skin grafting for extensive skin degloving. Two patients required immediate amputation. Late sequelae included prolonged edema, nutritional depletion, neuroma formation of the superficial branch of the radial nerve, late carpal tunnel syndrome, and partial brachial plexus palsy. Industrial roller injuries continue to be an occupational hazard associated with more severe crushing trauma than the low-energy wringer washer injuries first described by MacCollum (11). Attention must be paid to the treatment of crushed skin, muscle, and nerves, fracture stabilization, nutritional support, and occupational therapy. Concurrent monitoring for signs of a developing compartment syndrome and complications of rhabdomyolysis is essential.

Sonographic diagnosis of furosemide-induced nephrocalcinosis in newborn infants.

In three newborn infants who had received furosemide for a variety of indications, strongly increased echogenicity in the pyramids of the renal medulla was seen on ultrasonography. In one infant the changes were unilateral, the contralateral kidney being nonfunctional because of probable renal artery thrombosis. One infant died and medullary nephrocalcinosis was confirmed post mortem radiologically as well as by computed tomography and anatomic-histologic examination of the kidneys.

Evaluation of nitrobenzimidazoles as hypoxic cell radiosensitizers.

Radiobiological and pharmacokinetic assays were performed to determine the potential of 2-nitrobenzimidazole (NBI) as a hypoxic cell radiosensitizing agent. As judged by comparing survival curve slopes of Serratia marcescens irradiated under aerated and hypoxic conditions, the NBI enhancement ratio (ER) at 2 mM concentration was 2.4 +/- 0.2, compared with an oxygen enhancement ratio of 3.3 +/- 0.3. 2,5-Dinitrobenzimidazole (DNBI) was investigated in vitro; its ER was 3.0 +/- 0.3 at 4 mM concentration. Very poor tissue penetration of DNBI precluded further testing in vivo. Acute toxic signs appeared in C3H/HeJ mice following ip injection of NBI at 100 mg/kg. These would be partly attributable to the stress caused by the high pH of the injection vehicle. The LD50 was estimated to be 125-150 mg/kg. Mammary adenocarcinoma tumors grown in the flanks of these mice exhibited maximum NBI levels at 5 min postinjection (ip). Peak tumor radiosensitization occurred in the interval between 5 and 10 min postinjection. The ER for tumor regrowth delay was 2.1 +/- 0.3 following 50 mg/kg injected into mice 5 min before irradiation. Functional evaluation up to 40 days after treatment revealed no evidence of neurological deficit.

Safety data on zimelidine hydrochloride following an overdose.

Zimelidine, a new antidepressant, has proved to be a potent and selective inhibitor of 5-HT uptake into central 5-HT neurons. Due to its structural differences from the tricyclic antidepressants, it has also been suggested that it may have a different pharmacological profile, and be devoid of anticholinergic and noradrenaline potentiating properties. During the treatment with zimelidine hydrochloride, as a part of an open trial, one of our patients, a 38-year-old chronically depressed woman, overdosed herself with more than 10 times the actual therapeutic dose of zimelidine with no undesirable side effects. Previous studies have indicated that zimelidine in comparison with imipramine and chlorimipramine has no, or at most, a slight effect on peripheral adrenergic neurons, and has less pronounced anticholinergic properties than imipramine. The findings of our overdose case provide some support for these findings.

Asthma attack frequency in a pediatric population.

The joint occurrence of asthmatic symptomatology and psychological maladjustment does not necessarily imply a functional relationship between these two sets of phenomena. To test the contribution of psychological adjustment to the prediction of asthma symptomatology, a stepwise multiple regression was applied. Pulmonary and psychological adjustment variables were included in the regression analysis. The single best predictor of attack frequency was the pulmonary factor MMEF. Predictability was significantly increased with the family adjustment rating, the pulmonary factor FEV1, and the Child Adjustment Rating entered into the equation. The results were interpreted to be consistent with psychosomatic theory.

A comparison of moral reasoning in drug addicts and nonaddicts.

Assessed moral reasoning of 20 male addict patients and 17 nonprofessional male hospital employees by Kohlberg's methods. The mean level of moral reasoning of the drug addict group (2.97) was not found to differ significantly from that of the nonaddict comparison group (2.88). Furthermore, moral reasoning in addicts was not found to be related significantly to a number of background and personality characteristics. The findings, therefore, indicated that moral reasoning and moral behavior are not related significantly in drug addicts. It was concluded that the deviant behavior of this group is more likely the result of a deficiency in ego controls.
