Peripapillary neovascular membranes in presumed ocular histoplasmosis.

We studied 39 eyes with ocular histoplasmosis and peripapillary neovascular membranes for an average of 12 months. On the basis of clinical findings, 19 were selected for treatment with photocoagulation. Of the treated eyes, 12 (63%) had a visual result of 6/12 (20/40) or better. Of the 20 untreated eyes, six were clinically untreatable and 14 were electively followed up. All but one of the eyes electively followed up had a good visual result. Proximity of the neovascular membrane to the fovea was the most important prognostic factor in determining visual outcome. All eyes with membranes less than 1/4 disk diameter from the fovea lost central vision. All membranes greater than one disk diameter had excellent results. The indications for treatment were prolonged serous or hemorrhagic detachment of the fovea, documented progression of the membrane, and possibly, loss of central vision in the fellow eye from peripapillary disease. Residual and recurrent neovascularization were significant treatment complications.

Histocompatibility antigens and diabetic retinopathy.

Of 160 patients with onset of diabetes at or after 30 years of age, the 84 with no evidence prevalences of HLA-A1 and B8 when compared with the 76 with retinal complications or with the 282 healthy blood donors. In addition, in 90 patients with onset of diabetes before age 30 years, we could confirm the reported significant increase of HLA-B8 and decrease of B7, but no differences were noted between those juvenile-onset diabetics with and those without retinopathy.

The flammability limits of lean fuel-air mixtures: thermochemical and kinetic criteria for explosion hazards.

The present state of knowledge is reviewed concisely in terms of the experimental methods used, the effect of apparatus size, accuracy of data, methods of data presentation, and the sensitivity of the limits to initial temperature and pressure. The heat of combustion per mole of gas mixture at the lean limit is a reliable thermochemical criterion for the flammability of organic fuels with comparable reactivities. The limit calorific value for the heavy paraffins is 11.5 +/- 0.1 kcal mole -1. However, kinetic effects strongly influence this value. Highly reactive fuels (hydrogen, acetylene) require lower energy contents, whereas less reactive fuels (ammonia) require higher values. Hydrogen-starved fuels (carbon monoxide, cyanogen) show marked anomalies and are sensitive to impurities that can provide H-atom chain carriers. These kinetic effects are reflected in the experimentally measurable burning velocity of the fuel. This parameter is a key ingredient in the theory of flammable limits, which is briefly sketched. Five competing processes dissipate power from the combustion wave and quench it at some characteristic limit velocity. The prevalent consensus that the limits are controlled by natural convection is clearly demonstrated, and the complex interplay of kinetics and thermochemistry follows logically therefrom.

Denial and terminal illness.

Denial in the terminally-ill is often seen as a problem that health care professionals, particularly social workers need to fix. Rather than seeing denial as a part of acceptance, it is seen as just the opposite. Denial surfaces to establish control in an uncontrollable situation such as terminal illness. The social worker's challenge is to help the patient and family see their own strengths and make their own decisions. As the patient and family regain some control over their lives, denial is often replaced with other, more functional coping mechanisms.

Placebo-controlled study of pramiracetam in young males with memory and cognitive problems resulting from head injury and anoxia.

The current study evaluated under double-blind placebo-controlled conditions, the safety and efficacy of 400 mg pramiracetam sulphate TID in treating memory and other cognitive problems of males who have sustained brain injuries. The results of the study indicate that subject performance in measures of memory, especially delayed recall, evidenced clinically significant improvements after the administration of pramiracetam sulphate as compared to placebo. This improvement was maintained during an 18-month open-trial period on the medication as well as during a 1-month follow-up period after the pramiracetam was discontinued.

Investigation of pulsed light for terminal sterilization of WFI filled blow/fill/seal polyethylene containers.

A study was performed to assess the ability of pulsed light to sterilize water for injection in blow/fill/seal polyethylene containers. Pulsed light uses intense, short duration flashes of broad spectrum white light to produce high levels of microbial kill. In a first phase of testing, containers of 0.5, 5, 15, and 120 mL nominal volume were inoculated with Bacillus pumilus endospores, Bacillus subtilus variety niger strain globigii endospores, Bacillus stearothermophilus endospores, and Aspergillus niger conidiospores. Approximately 10(6) colony forming units of each test spore were individually inoculated into 22 replicate containers of each sample volume. Two of these containers served as inoculation recovery controls, and 10 were treated using each of two pulsed light exposure methods: single-sided treatment, or treatment within a reflective cavity. Both treatments employed flashes of intense broad spectrum pulsed light delivered at one flash per second. Cavity treatment used 10 flashes to treat each container within a reflective cavity containing a single lamp. Cavity treatment yielded no recoverable survivors for any of the challenge spores from the contents of any of the 160 total samples. Single-sided treatment used 20 approximately 1-J/cm2 flashes from a single lamp-reflector projecting onto one side of the container. Single-sided treatment yielded no recoverable survivors from the contents of the containers for any of the bacterial endospores tested, but Aspergillus niger survival was detected in 4 of the 40 single-side treated samples. A second phase of tests examined the pulsed light inactivation of Bacillus pumilus spores for a range of inoculation levels. High levels of Bacillus pumilus spore kill were obtained using only a few cavity flashes. The results show that pulsed light can provide high levels of microbial lethality and possesses potential for use as a terminal sterilization method for water for injection in filled, sealed polyethylene containers.
