[Gingival grafts: evaluation of recovering surfacic quantimetry by image analysis].

24 gingival recessions were treated with free gingival autografts. The root coverage was evaluated by a new method: surfacic quantimetry by image analysis. This present study compares the linear and surfacic results. After statistic study, the linear coverage was evaluated at 26.74% (+0,813 mm for an initial height of 3,040 mm) and the surfacic coverage at 19.10% (+1,223 mm2 for an initial surface of 6,400 mm2).

High-flux signals and spatial localization in high-resolution 1H spectroscopy with surface coils.

To perform in vivo localized proton spectroscopy with water suppression, spin-echo sequences, made of binomial pulses, are commonly used with surface coils. The frequency selective response to such a sequence is also-spatially dependent, that is dependent on the sample shape and on the pulse angle adjustment. It is consequently pointed out in this paper that quantitative analysis for relative peak intensities may be strongly affected by the contribution of the high-flux regions. In vivo proton spectroscopy of rat brain exemplifies this difficulty. It is shown that the use of selective prepulses to suppress high-flux signals may be of poor efficiency depending on chemical shift, while the use of hard nonselective prepulses works for any chemical shift.

Socio-economic risk factors for gingival disease in children.

The present investigation was undertaken to detect children at risk of gingival disease. Two hundred and eighty-five French 7-year-olds were examined. The lesion was evaluated with regard to socio-economic status in relation with the mean gingival index. The findings show that we can define a principal risk group, children of migrant workers.

[Sociodemographic and economic evaluation of periodontal care providers under a preliminary plan].

A prospective epidemiologic survey performed over a three months period was done in association with the Health Insurance Fund of Lyon, France. The major objectives of this study were to evaluate the credentials of practitioners prescribing periodontal therapy under the health plan. 72.9% of the dentists received their dental diploma within the last 5 years and were predominantly male. None of the practitioners were specialists. Apparently, periodontal therapy represented only 10% of the treatment prescribed by the dentists surveyed.

[A survey of the oro-dental status of children in the Rhône-Alps region, 6-15 years of age].

The DMFT, DMFS and dfs indices were determined on a sample of 2,018 children, aged 6 to 15 years, representative of the Rhône-Alpes region. Plaque, calculus and gingival indices were also recorded. The results indicated that the caries experience in the deciduous dentition was important. The DMFT and DMFS indices increased with age. The DMFT increased from 1.03 +/- 1.50 at 6 years to 7.39 +/- 3.78 at 15 years and was higher in girls at the ages of 8, 14 and 15 years. The DMFS was higher in rural zones at the ages of 7, 12 and 14 years. The periodontal indices were generally low.

Two-dimensional 1H spectroscopic imaging for evaluating the local metabolic response to focal ischemia in the conscious rat.

Two-dimensional 1H spectroscopic imaging and magnetic resonance imaging were used to study focal ischemia induced by middle cerebral artery occlusion in rats. A water suppressing spin-echo sequence was used at 4.7 T. Phase encoding during the spin-echo delay (TE = 272 ms) yielded an 8 x 8 array of 35 microL voxels. The injured area of the brain had a higher lactate level and markedly lower N-acetyl aspartate, creatine and choline levels than did the non-ischemic regions. The spectroscopic imaging data clearly showed the localization of the infarct, which agreed well with both magnetic resonance imaging and the histological data obtained post-mortem. This study demonstrates the potential usefulness of combining magnetic resonance imaging and 1H spectroscopic imaging for studying animal models of stroke, and indicates the suitability of the technique for further pharmacological approaches.

[Axillary lymph node surgery in 1998].

The need for a systematic axillary clearance in breast cancer is presently under question. Alternative methods include the omission of node biopsy in very small tumours and lymphadenectomy limited to the sentinel node. This article discusses the current procedures in axillary surgery in 1998, with information concerning the relationships between the tumour characteristics and the probability of nodal involvement, the new surgical techniques aiming at reducing morbidity, with special emphasis on sentinel node biopsy, and the therapeutic protocols presently being used at the Institut Curie.

Significance of bowel wall enhancement on CT following blunt abdominal trauma in childhood.

Over a 3-year period, 12 children with blunt abdominal trauma were noted to have intense bowel wall enhancement (BWE) on CT. In four, with fatal CNS injury, there was no evidence of bowel perforation, and the changes may be related to the hypovolemic complex. In the remaining eight patients with a gastrointestinal perforation, BWE was associated with the presence of diffuse or focal bowel wall thickening and free peritoneal fluid. The enhancement and thickening were due to peritonitis secondary to perforation as shown at operation or autopsy. In three further patients with bowel perforation, thickening and enhancement were absent in two with retroperitoneal perforation and thickening, and no enhancement was noted in one patient in whom the study was considered suboptimal in retrospect. Intense BWE, therefore, aids in the diagnosis of the hypovolemic complex: CNS injury and bowel perforation. When combined with bowel thickening and free fluid, perforation and peritonitis should be strongly suggested. Enhancement and thickening should suggest perforation even if other visceral injury is present to account for the free fluid. Surgical intervention should thus be more strongly considered when bowel wall thickening accompanies BWE.

Planning dental manpower in Lebanon: scenarios for the year 2015.

The requirements for dentists in Lebanon for the year 2015 were estimated using the World Health Organization/World Dental Federation planning model. The aim was to help decision- and policy-makers in Lebanon to plan strategically for the supply of dental personnel in line with the recommendations of the Oral Health National Plan guidelines from 1995. Assumptions based on previous research in selected populations were taken to support the simulation. The number of dentists required for Lebanon in the year 2015 was estimated to be 2715 while the projected supply will be 6176. Urgent measures are needed to reduce the potential oversupply of dentists in this country.

[Nuclear cardiology in the preoperative evaluation of the patient at risk of coronary disease; meta-analysis of the contribution of thallium dipyridamole scintigraphy].

Coronary disease may be assessed by several techniques of nuclear cardiology: classical thallium myocardial scintigraphy, position scintigraphy, left ventricular angioscintigraphy. These techniques measure the response of coronary flow, myocardial metabolism, or left ventricular contractility during a physical or pharmacologic stress test. Many studies have reported the interest of dipyridamole thallium scintigraphy in the assessment of the surgical risk for a coronary event. Regarding the meta-analysis of up-to-day published studies, the surgical risk is 26% for an abnormal test against 3% for a normal study. Considering only the risk of infarction or cardiac death, corresponding probabilities are 18% and 2%, respectively. Dipyridamole stress test is a fairly safe and accurate method for discriminating between high and low risk patients in a population selected on clinical grounds.

[Sleep disorders in prison].

Prisoners often ask the psychiatrist soporific. After a clinical review, the author exposes the particular difficulty concerning psychotropic distribution patterns in this unusual institution. He puts in an ethical consideration about the concept of "hypnotic deliberately retarded effect".
