[Arterial stenoses of the arms in Horton's temporal arteritis. Apropos of 3 cases. A review of the literature].

The authors describe three histologically documented cases of temporal arteritis complicated by arteritis of the upper limbs confirmed by arteriography. The evolution during treatment was followed up until recovery. A review of the literature underlines the frequency of upper limb arteritis in Horton's disease. It may be the presenting feature or, more commonly, occur during the evolution of the condition. It may give rise to ischaemic symptoms and/or an arterial bruit and/or asymmetric blood pressure readings which should be looked for routinely. Doppler ultrasound may be helpful. Arteriography shows that the lesions are usually bilateral, on the subclavian and axillary arteries. Giant cell arteritis has been found on arterial biopsy. Upper limb arteritis necessitates renewed or increased steroid therapy which may be beneficial in the absence of thrombosis. These arterial lesions have also been reported in some cases of polymyalgia rheumatica. Several cases of isolated upper limb arteritis without temporal arteritis or polymyalgia rheumatica have also been described.

Low-dose cytosine arabinoside treatment for acute nonlymphocytic leukemia in elderly patients.

Thirty patients older than 65 years of age with acute nonlymphocytic leukemia were treated with low-dose cytosine arabinoside (10 mg/m2 subcutaneously every 12 hours for 15 to 21 days). Fifteen achieved complete remission and five had partial remission. Treatment was more effective when initial bone marrow cellularity was low (P = 0.007). Four of six patients with secondary leukemia entered complete or partial remission. Therapy was well tolerated with reduced myelosuppression and few number of early deaths. Sixteen patients followed the whole treatment as outpatients. Six of 12 patients who achieved complete remission showed no evidence of post-therapeutic bone marrow aplasia. These data are consistent with the view that low-dose cytosine arabinoside acts on leukemic cells as a differentiating agent.

[Treatment of malignant hemopathies with aracytine in low doses. Analysis of 159 cases].

Based on the in vitro experiments which showed the capacity of Aracytine (AraC) to induce differentiation of leukemic cells, low dose AraC treatment of patients with acute myeloid leukaemia (AML) or myelodysplastic syndromes (MDS) has been available since 1980. Many studies involving only small numbers of patients have demonstrated contradictory results regarding the efficacy or the mechanism of action of this regimen. A retrospective clinical study is presented, including 159 patients treated in 11 French institutions and 1 German institution (AML: 99 patients; MDS: 38 patients). The study confirms the efficacy of low dose AraC, especially in elderly patients with hypoplastic AML. Although, less severe than with high dose chemotherapy, toxicity needs regular monitoring. The mechanism of action cannot be ascertained by this clinical study, however half of the complete remissions were obtained with no bone marrow aplasia. Studies with low dose AraC and/or other in vitro differentiating agents need to be continued.

[Specific cutaneous localizations in multiple myeloma and Waldenström's macroglobulinemia. Apropos of 4 cases].

The specific skin locations of multiple myeloma (M. M.) and Waldenström's macroglobulinemia (M. W.) are rare. The authors report here 4 cases: 2 within M. M., the other 2 within lymphoplasmocytic lymphomas with IgM dysglobulinemia (M. W.) or in situ tumoral secretion of IgM. Observation no. 1. A 56-year-old man is admitted for investigations for spontaneous fractures of the ribs. The skin examination reveals about ten purple, nodular and sometimes ulcerated lesions of the back which appeared over more than one year. The histology of a cutaneous nodule shows a dense infiltrate of plasma cells with numerous atypical cells. The laboratory investigations reveal a IgG, kappa M. M. A polychemotherapy leads to a healing of the nodules in 3 months. Observation no. 2. A 58-year-old man is admitted for investigations of a proteinuria (6.80/24 h) which will be imputed to a lambda chains M. M. One year and a half later appear subcutaneous thoracic nodules. The histology of the skin confirms the diagnosis of metastatic skin plasmocytoma; the patient will die a few months later in spite of the chemotherapy. Observation no. 3. A 84-year-old woman is admitted for investigations of a dysglobulinemia. On the cutaneous part there are tumoral purple plaques symmetrically disposed on the 2 cheeks and the nose. The histology of the skin shows a dermo-hypodermal tumoral infiltrate essentially made of lymphoplasmocytoid cells. In direct immunofluorescence study there is a kappa tumoral secretion of mu type.(ABSTRACT TRUNCATED AT 250 WORDS)

[Association of chronic lymphoid leukemia and epidermoid carcinoma of the mouth floor].

Authors report a scarce case of association in stomatologic and otorhinolaryngologic area. After a general study of this type of association, in which the average of 6,8% of carcinoma is usually admitted in lymphocytic leukemia, a case of lymphocytic nodular lymphoma, a disease very close to chronic lymphocytic leukemia, is reported with secondary discovering of an epithelioma. Importance of detection must be underlined, and the treatment of carcinoma is discussed.
