Outcome assessment of the French internship graduate programme.

In 1995, the first dental postgraduate residency programme was established in France. Selection of the applicants was performed by means of a selective national competitive examination among dental students in the final year of the undergraduate studies. The main objective of this 3-year general practice internship was to provide a clinical training for future academic dentists. This study describes the characteristics and the professional outcome of the student population enrolled in this programme. The French internship programme represents a new and welcome modification on the training of future dental academicians and establishes similarities with other programmes in Europe.

Effect of pentoxifylline on renal toxicity of cyclosporine: results of a clinical trial after heart transplantation.

BACKGROUND: Pentoxifylline was suggested to prevent the renal release of endothelin caused by cyclosporine. METHODS: We studied the renal-sparing effect of pentoxifylline in 44 patients who underwent heart transplantation between 1991 and 1994 and were randomized to a group treated with pentoxifylline (400 mg three times daily) or to a control group. All patients were treated according to the same immunosuppression protocol, including induction with perioperative rabbit anti-thymocyte antibody and maintenance with azathioprine, cyclosporine, and prednisone. Five patients withdrew voluntarily because of lack of compliance, and five patients died during the first month of the study. RESULTS: There was no difference between the two groups in regard to age, sex, initial cardiopathy, the number of acute rejections, and the number of infection episodes. Urinary clearance of creatinine averaged 1.1 +/- 0.1 ml/sec, 1.3 +/- 0.1 ml/sec, and 1.3 +/- 0.1 ml/sec in the control patients (n = 16) and 1.2 +/- 0.1 ml/sec, 1.4 +/- 0.1 ml/sec, and 1.3 ml/sec in patients treated with pentoxifylline (n = 18) at initiation, 12 months, and 24 months of the study (p > 0.05), respectively. At these three times, the serum creatinine levels averaged 106 +/- 4 mmol/L, 119 +/- 4 mmol/L, and 126 +/- 5 mmol/L in the control group and 94 +/- 4 mmol/L, 114 +/- 4 mmol/L, and 127 +/- 5 mmol/L in patients treated with pentoxifylline, respectively (p > 0.05). Trough levels of cyclosporine throughout the study period averaged 212 +/- 8 mmol/L and 206 +/- 8 mmol/L in the control and treated groups, respectively (p > 0.05). Endothelin blood levels averaged 0.4 +/- 0.2 pg/ml for nine control patients and 0.4 +/- 0.1 pg/ml for a group of 10 patients treated with pentoxifylline (p > 0.05). CONCLUSIONS: Pentoxifylline in association with cyclosporine did not result in a significant improvement in renal function during the first 2 years after heart transplantation.

Prosthetic use of carbon fiber-reinforced epoxy resin for esthetic crowns and fixed partial dentures.

The traditional alloys used as metal bases for fixed partial dentures are accompanied by various problems such as corrosion, allergy, toxicity, casting, and preparation for both patient and prosthodontist and in magnetic resonance imaging for diagnosis. A DGEBA-DDM type polyepoxide resin and carbon fibers composite has been introduced that addresses some of these considerations. A three-part study of the clinical behavior of a limited number of prostheses has shown positive results confirming the biofunctional capacity of polyepoxy-carbon materials and their gingival biocompatibility.

[Gingival preparation].

For the general practitioner, the superficial periodontal structure is an obstacle when trying to gain access, control and visualization to juxta or infra gingival preparations. A temporary restoration will maintain and protect those limits and in many cases it is a very efficient procedure. Sometimes it is necessary to enhance the gingival preparation by sophisticated methods. There are many different technics available to the practitioner. In this article we will describe the technics with easy clinical use and those respecting the gingival tissue (mechanical, physico-chemical).

[Full band impressions].

Complete impression with rings achieves the same definition than complete impressions alone and gives a final quality similar to unitary guided impressions. The required steps are the following: 1st clinical: adjustment of the unitary rings, laboratory: elaboration of metallic resin boxing and individual tray, 2nd clinical: complete impression with rings. This method is of a particular interest because of its facility and its possible adaptations to various impression pastes or clinical situations. No gingival conditioning is required to the impression, and increased indications of the method can thus be seen when esthetics or periodontal conditions are a priority.
