Does the outcome of the tidal breathing and dosimeter methods of assessing bronchial responsiveness in children with asthma depend on age?

As minute volume increases with age, a study was carried out to determine whether the measurement of bronchial responsiveness to pharmacological agents with the tidal breathing technique in children might be influenced by age. Bronchial responsiveness to histamine administered by tidal breathing was therefore compared with that produced with a dosimeter in 25 children with asthma aged 5-18 years. Bronchial responsiveness was defined as the concentration of histamine that caused a 40% rise in pulmonary resistance (PC40) measured by random noise forced oscillation at 6 Hz. Values of PC40 measured by the tidal breathing method were lower than those obtained with the dosimeter method, presumably owing to differences in the dose administered and variations in the pattern of breathing. The difference between the two methods was not related to age, however. It is concluded that the tidal breathing and the dosimeter methods are both suitable for the measurement of bronchial responsiveness in children of various ages and that both can be used in longitudinal studies.

Ceramic brackets.

Ceramic brackets have been understandably welcomed by patients; they are the best attempt so far at producing an orthodontic appliance which combines the aesthetic needs of the patient with the technical performance required by the orthodontist. Nevertheless, the only advantage that ceramic brackets have over stainless steel brackets is one of appearance and serious questions about bracket fracture and tooth damage during bracket removal remain unanswered at the present time. There is considerable interest throughout the world in the industrial development and use of ceramics and it may be that future generations of ceramic brackets solve some or all of the problems that currently exist. At the present time, however, ceramic brackets should be used cautiously within the limitations of the material and not simply as an alternative to metal brackets. Any adverse incidents concerned with their use should be reported promptly to the supplier.

Scope and limitations of orthognathic surgery.

Modern orthognathic surgical procedures allow correction of bony disproportion in almost any part of the face, but are limited in the fine tuning of tooth position and occlusion. However, carefully planned combined surgical and orthodontic treatment can produce dental and skeletal results of a high standard. If a GDP is presented with a malocclusion beyond the scope of normal orthodontic treatment, then referral to an oral surgery/orthodontic clinic is indicated. This article provides an overview of what the team on such a clinic can achieve.

Sensory nerve morbidity following Le Fort I osteotomy.

The Le Fort I osteotomy has been used increasingly frequently in the management of dentofacial deformity since the wide acceptance of the down-fracture technique. The improved access provided by this technique allows movement of the Le Fort I segment in three planes. This paper reviews briefly the neuroanatomy of the area and considers how the surgical technique of Le Fort I osteotomy interferes with the sensory nerve supply. The sensory nerve function in ten patients who underwent Le Fort I osteotomy is reviewed.
