Abdominal aortic dilatation in patients operated on for carotid artery stenosis.

The prevalence of abdominal aortic dilatation among 201 men and 86 women who underwent carotid endarterectomy in 1971-1982 was studied from the date of operation to the end of 1984. Of the 109 patients who died during this time, 96 were autopsied, and 13 (13.5%) of them had aneurysm of the abdominal aorta. Ultrasonographic screening of the abdominal aorta was performed on 154 survivors, and showed dilatation in 17 (11%), 12/100 men and 5/54 women. The prevalence of hypertension, intermittent claudication, diabetes mellitus and coronary insufficiency at the time of endarterectomy did not differ between the patients with or without aortic aneurysm or dilatation. Patients who have undergone endarterectomy of the internal carotid artery constitute a group with high prevalence of abdominal aortic dilatation.

Heparinization of polytetrafluoroethylene (ePTFE) grafts. The effect on pseudointimal hyperplasia.

A study was undertaken to evaluate the effect of a covalent heparinization on thrombogenicity of ePTFE grafts in sheep. With scanning electron microscopy heparinization occurred as a thin smooth layer covering the network of ePTFE. Most of it had disappeared already after 2 hours. In long-term experiments a high and low flow part of the ePTFE graft was created in the carotid artery system. The heparinization had no effect on patency and there was no difference between the high and low flow parts of the graft. The graft surface was covered with fibrous tissue, which was significantly thicker than the pannus in the host vessel. In the graft interstices (internodal distance of 30 micron) capillaries were seen without difference between occluded and patent grafts. The capillary density was significantly higher proximally than distally.

Ulceration of the carotid bifurcation. A preliminary report on a diagnostic problem.

The prevalence of ulceration in the carotid bifurcation, as judged from high-resolution 2-D ultrasound scanning and angiography, was compared with the findings at carotid endarterectomy in patients with TIA or stroke. The surgeons registered ulceration in 26 of the 29 treated carotid arteries. Angiography and ultrasonography identified ulceration in 9 and 19, respectively, of the same arteries, indicating that high-resolution 2-D ultrasound scanning is superior to angiography in the diagnosis of carotid ulceration. All arteries with tight stenosis (diameter reduction greater than or equal to 75%), but only two of four with less than 30% stenosis were ulcerated.

Trends in civilian vascular trauma during 30 years. A Swedish perspective.

Vascular injuries operated on during a 30-year period (1955-1984) were analyzed. There has been significant increase of such injuries, particularly the iatrogenic types. The total incidence per 100,000 population and year rose from 11.0 to 26.6. Various angiographic and catheterization techniques were responsible for the iatrogenic increase. The noniatrogenic vascular injuries not infrequently were part of multiple trauma, with fractures and a variety of penetrating wounds. Numerous operative techniques were used, increasingly with reconstructive surgery. The mortality rate did not change in the study period, but the rate of amputations and of various long-term sequelae diminished.

Unilateral reconstruction of the iliac artery. A comparative evaluation.

Unilateral reconstruction of the iliac artery because of obliterating atherosclerotic disease was performed on 122 patients (group A) in the period 1971 through 1979. A bifurcation dacron bypass was used for iliac artery reconstruction in 52 patients (group B) during the same period. Of the group A patients, 24% required eventual contralateral iliac reconstruction, and in 16% redoing of the ipsilateral operation became necessary. In group B the need for redoing was 25%. The 30-day mortality was 1.6% in group A and 7.8% in group B. The patency rates were similar up to 5 years postoperatively. The authors conclude that there is a place for unilateral iliac artery reconstruction, especially in high-risk cases or for patients with short life expectation who require relief of the vascular obstruction but for whom bilateral operation is not immediately mandatory.

Venous function after thigh vein thrombosis diagnosed by the 125I-fibrinogen test. A study with occlusion plethysmorgraphy.

The long-term venous function after postoperative thigh vein thrombosis according to the 125I-fibrinogen test was analysed. 50 patients with a positive fibrinogen test postoperatively were investigated with strain gauge plethysmography 74 (27--103) weeks after operation. In five patients (10%), all female and all hip operated, pathological plethysmograms were seen.

Patency of the plantar arch as a prognostic indicator in patients with critical leg ischaemia; a retrospective study.

OBJECTIVE: To find out if patency of the plantar arch on the preoperative angiogram was an accurate indicator of prognosis of vascular intervention, of leg salvage and of survival in patients with critical ischaemia of the leg. DESIGN: Retrospective study. SETTING: University Hospital. SUBJECTS: 302 patients who underwent femoral angiography for critical ischaemia of the leg in 1985-1986. INTERVENTIONS: Examination of aortofemoral angiograms and scoring of the radiological changes according to a suggested scoring system. MAIN OUTCOME MEASURES: Correlation of scores with outcome. RESULTS: Patients with patent plantar arches had a significantly higher cumulative patency after vascular intervention (logrank chi 2 4.2, p < 0.05), leg salvage (logrank chi 2 11.9, p < 0.01) and significantly longer survival (logrank chi 2 7.8 p < 0.01) than patients with partly or completely occluded plantar arches. CONCLUSION: It is important to see the plantar arch on the preoperative angiogram. The scoring system is valuable in predicting patency after vascular intervention, leg salvage and survival.
