Recurrent pneumothorax.

A young man with spontaneous pneumothorax who suffered from fourth recurrence, is being reported. He was treated by surgical pleural abrasion.

Bronchoalveolar lavage--cellular characteristics in patients with idiopathic pulmonary fibrosis and sarcoidosis.

Bronchoalveolar lavage (BAL) was carried out in 30 patients of idiopathic pulmonary fibrosis (IPF), 10 patients of sarcoidosis and 10 normal subjects who served as controls. The total cell counts were higher both in the IPF (24.8 x 10(5)/ml) and sarcoidosis (19.3 x 10(5)/ml) as compared to the normal controls (9.8 x 10(5)/ml). The difference was statistically significant in both the groups (p less than 0.001 and less than 0.05 respectively). Patients with IPF showed a predominant polymorphonuclear response although the lymphocytes were also increased whereas patients of sarcoidosis had a lymphocytic response in the BAL fluid. In 6 patients with IPF the study was repeated after 6 weeks of steroid therapy and a decrease in the total cell count and polymorphonuclear cells (%) was noted.

Systemic lupus erythematosus during isoniazid therapy.

A case of systemic lupus erythematosus (SLE) in a young female which developed during the course of isoniazid therapy for pulmonary tuberculosis is described. SLE in this case is believed either to be induced or precipitated by isoniazid. Pertinent literature has been reviewed.

Short course chemotherapy (9 months) for lymphnode tuberculosis.

Eight-two patients with tubercular lymphadenitis received rifampicin, isoniazid and ethambutol for 9 months. Complete disappearance of glands after this period occurred only in 74.3% of cases. Extension of therapy upto 1 year showed clearance in 10 more cases with an overall response rate of 87.2%. It is concluded that 9 months of therapy with these 3 drugs is not satisfactory for tubercular lymphadenitis.

Combination chemotherapy in inoperable lung cancer--a trial of 2 regimens.

Combination chemotherapy with 3 drugs (vincristine + cyclo-phosphamide + methotrexate of adriamycin) were administered to 36 cases of advanced bronchogenic carcinoma. The median survival for patients with small cell carcinoma (SCC) was 24.5 weeks whereas that for non small cell carcinoma was 27.5 weeks (P greater than 0.05). Fifty-five per cent of the patients survived beyond 6 months; 30.5 per cent survived beyond 1 year and 11 per cent of the patients survived beyond two years. Adriamycin did not prove more effective than methotrexate. However, omission of adriamycin from the regimen may reduce the cost of chemotherapy.

Correlation of lung function tests with dyspnoea scale, 12 minute walking test and visual analogue scale in patients with COPD.

Twenty-five patients with chronic bronchitis and emphysema were studied in an attempt to evaluate clinical methods for rating dyspnoea. Whereas the Medical Research Council (MRC) dyspnoea scale and the 12-minute walking test showed good correlation with pulmonary function tests, the visual analogue scale (VAS) did not. When the dyspnoea scales were compared with one another, the MRC scale and the VAS and the MRC scale and 12 minute walking test showed good correlation with one another.

Wegener's granulomatosis in north India. Radiologic manifestations in eleven patients.

The chest radiograph in Wegener's granulomatosis reveals several striking, but not pathognomonic abnormalities. Nine (82%) of eleven patients with Wegener's granulomatosis had abnormal chest radiographs. Infiltrates were seen in all patients and were nodular in seven (77%). Six patients had cavities which were most often unilateral and thick walled. Serial radiographs showed the simultaneous appearance and resolution of infiltrates in 4 of 7 instances. Bilateral pulmonary involvement (67%), pleural effusion (33%), and pericardial effusion (22%) were also seen. Paranasal sinus involvement was seen in 4 of 5 studies. The chest radiograph was interpreted as tuberculosis in eight patients, resulting in delayed diagnosis and incorrect treatment.
