[Dens fracture in elderly patients and surgical management].

The problems of surgical therapy in seven patients aged over 70 years are discussed. Clinical and radiological findings as well as operative treatment (anterior transaxial screw osteosynthesis or posterior atlanto-axial arthrodesis) are analysed in detail. Five out of 7 patients treated in this way could be early mobilised. Two patients died on multiorgan failure after an initial uneventful postoperative course (28.6% mortality rate). A stable osteosynthesis was obtained in all cases; a complete bony fusion as could be radiologically demonstrated, occurred in four out of the five survivors. We feel that despite this relatively high mortality rate surgical treatment provides better recovery chances and a higher quality of life to such patients.

Left lung asthma.

The chest radiograph of a woman with acute asthma showed signs of obstructive emphysema of the left lung. Fiberoptic bronchoscopy excluded obstruction of a large bronchus. The signs disappeared after antiasthmatic treatment, suggesting that they were caused by airway closure. In usual asthma, airway closure affects both lungs; the reason for the unilateral predominance we observed is unknown.

[Ovarian hyperstimulation syndrome during in vitro fertilization stimulations. Comparison between the experience of the Center for Medically Assisted Procreation of the Saint Antoine maternity department and a review of the literature].

The authors present their experience with the severe ovarian hyperstimulation syndrome when they were using stimulation for IVF and ET and they compare it with the literature. Over a period of 23 months. 154 stimulations were carried out using a long protocol associated with a CnRH agonist and hMG. 142 vaginal aspirations were carried out. The clinical pregnancy rate was 19% per aspiration and 21.9% per transfer. Of the 142 patients who were aspirated five had severe ovarian hyperstimulation with a favourable outcome. The authors suggest a physiopathological comparison for ovarian hyperstimulation and the management to be carried out when this complication occurs. They emphasise how potentially serious this complication is and that there has been no physiopathological explanation for its recrudescence since GnRH agonist have started to be used.

Parasitic hazard with sewage sludge applied to land.

A modification of the FAUST technique allowed a highly regular recovery of Taenia saginata eggs from sewage sludge, as well as their quantification. Despite the low viability (8%) noted, the viable T. saginata egg level remains high (20.10(6)/ha) and offers a serious risk for cattle even after a 3-week "no-grazing" period.

Susceptibility to human cutaneous leishmaniasis and HLA, Gm, Km markers.

A relationship between markers related to the immune response (HLA system, Gm and Km immunoglobulin allotypes) and susceptibility to cutaneous leishmaniasis was investigated in a population of Hmong refugees who had recently settled in French Guiana. Two approaches were used: 1) case/control comparisons of the marker phenotype distribution to detect possible associations; 2) multiple-case family studies to search for marker-linked genes. When the distribution of HLA-A, B, C, antigens and Gm, Km allotypes was compared between patients and controls, only a significant decrease of HLA-Cw7 antigen among leishmaniasis patients was detected (p = 0.01). No interaction between any two of these markers and the disease was found. On the other hand, neither an HLA, Gm or Km susceptibility gene could be demonstrated in the informative sets of affected siblings. These results are discussed with respect to those reported in other infectious diseases.
