Study of the pedigree of a patient with type 3 hyperlipoproteinaemia and sinking prebeta lipoprotein.

A 49-year-old woman, suffering from peripheral vascular disease, was found to have two lipoprotein abnormalities, namely, type III hyperlipoproteinaemia and sinking prebeta lipoprotein. Twenty-one members of the kindred were investigated in an attempt to determine the pattern of inheritance of both of these abnormalities. In a 21-year-old son of the proband plasma electrophoresis was in keeping with a type V hyperlipoproteinaemia. It is suggested that this may be a stage in the development of the characteristic type III pattern. If this is so it is consistent with previous suggestions of an incompletely penetrant single autosomal allele. Sinking prebeta lipoprotein was found in the plasma of two other members of the family. If this abnormality is also inherited as a single autosomal allele it must have a very low penetrance and the possibility that the abnormality is acquired cannot be excluded.

Changing relative proportions of apolipoproteins CII and CIII of very low density lipoproteins in hypertriglyceridaemia.

The very low density lipoproteins (VLDL) of fasting serum from subjects with normo- and hypertriglyceridaemia (Type IV and V) were separated into subfractions by density gradient centrifugation. The tetramethylurea (TMU)- soluble apolipoproteins were separated by polyacrylamide gel electrophoresis and the relative proportions of apo CII and CIIIs determined. There were highly significant correlations between the concentration of VLDL triglycerides and apo CII (r---0.92), apo CIII1 (r=+0.88) and the ratio apo CII/apo CIII1 (r= --0.94). It was suggested that the decreasing ratio apo CII/CIII1 with increasing triglyceride levels might cause a resistance to lipoprotein lipase and therefore a defect lipolysis of VLDL based upon a changed ratio apo CII/apo CII1 might be part of the pathogenesis of the hypertriglyceridaemia.

Assessment of Hitachi HME-20 pulse and blood-pressure monitor.

The performance of the Hitachi HME-20 pulse and blood-pressure (BP) monitor in comparison with direct intra-arterial BP recording and electrocardiographic monitoring is described. Highly significant (p less than 0.001) correlations were found between intra-arterial systolic and diastolic pressures and pressures recorded by the Hitachi monitor. Similarly, the electrocardiographically computed heart-rate, and that given by the Hitachi monitor were significantly correlated (p less than 0.001). Systolic blood-pressure was underestimated by a mean of -12 mmHg and tended to become more erroneous when intra-arterial pressure was greater than 150 mmHg. These results are comparable to more expensive pulse and blood-pressure monitors. We conclude that the instrument can reproduce a satisfactory estimate of heart-rate and blood-pressure and may be of particular use when a change of blood-pressure is of prime importance, rather than an absolute measurement.

Computer interpretation of pulmonary function tests.

A computer program for interpretation of pulmonary function tests, including flow-volume loops, lung volumes, and diffusing capacity, is described. Comparisons of data obtained before and after administration of bronchodilators and comparison with previous tests are also available as part of the program. Computer interpretation of pulmonary function testing accelerates the reporting of test data and may have a role in education in the discipline of respiratory medicine and related fields.
