Treatment of unilateral iliac artery disease by femorofemoral bypass grafting.

Between Jan. 1, 1970 and June 30, 1977, 50 men and 23 women underwent femorofemoral bypass grafting. The average age of the group was 64.7 years. The procedure was performed for disabling claudication in 50 patients and for limb threatening ischemia in 23. Knitted Dacron grafts were used in all but two patients. The operative mortality was 4.1% and the late mortality 21.9%. There were six complications related to the prostheses, three infected grafts and three false aneurysms. Thrombosis of the graft occurred in 15 patients; the graft was successfully revised in 2. The cumulative 5-year patency rate determined by life-table methods was 73.4%. The causes of failure appeared to be well defined and unilateral iliac artery disease. The donor iliac artery, poor runoff through a diseases deep femoral artery on the recipient side and infection of the graft. This study indicates that femoro-femoral bypass has a definite place in the management of patients with unilateral iliac artery disease. The procedure can be performed on selected patients with a low operative mortality and an acceptable patency rate.

Comparison of Ga-67 citrate images obtained with rectilinear scanner and large-field Anger camera.

Two methods for Ga-67 citrate imaging were compared on 20 patients. Scans were performed using approximately equal procedure time with two instruments: (A) a dual 5-in. rectilinear scanner with medium-energy collimator, with a single window spanning both the 93-keV and the 185-keV spectral peaks; and (B) a large-field (15-in. diam) Anger camera equipped with moving table, medium-energy collimator, and three windows covering the 93-keV, 185-keV, and 300-keV peaks separately. Sixteen abnormal sites and 24 normal sites were selected for comparison. Each site was evaluated by four physicians experienced in interpreting Ga-67 citrate images. The observers performed significantly better using the images obtained with the large-field camera (three windows) than with the dual 5-in. scanner (single window).

Statistical issues in pharmacoepidemiological case-control studies.

Pharmacoepidemiology is the study of the use and the effect of drugs in large numbers of people. The work relies largely on standard epidemiological principles, which, applied in a new context, bring particular constraints and opportunities. In January 1994, a cluster of children with cystic fibrosis (CF) who developed strictures in the colon was reported. It was suggested that these may have been associated with high strength pancreatic enzyme preparations which had been introduced into use in these patients approximately one year previously. This hypothesis has been evaluated by a case-control study which confirms the association with high-strength pancreatic enzyme usage. The study is described and is used to illustrate statistical issues to provide a framework for future research in statistical methodology in pharmacoepidemiological case-control studies.
