[Imaging procedures and follow-up in pediatric surgical diseases].

The most important modern imaging diagnostics are the ultrasound, scintigraphy, computed tomography and recently the nuclear magnetic resonance (NMR). The diagnostic tools should be used one after another, always starting with the easiest to perform, with low risk and no harm to the patient, at low costs. In routine diagnostic procedures (e.g. blunt abdominal trauma) this will be the sonography. The result of the ultrasound may give rise to other, but more invasive methods. Computed tomography, e.g. may give more detailed information about position, size and operability of a tumor whereas scintigraphy has its strong point in disclosing functional organ disorders (e.g. kidney, liver and especially in splenic transplants).

[Behavior of chloramphenicol residues following intramuscular administration in swine].

Residues of chloramphenicol (CAP) were examined in 24 pigs after intramuscular injection of 30 mg CAP/kg body weight. Two pigs were slaughtered after 3, 6, 12, 18, 24, 36 hours, 2, 3, 6, 10, 21 and 30 days, respectively. CAP-concentrations were determined in muscle, blood, urine, liver, kidney, bile, and fat. Methods used were gas-liquid chromatography and radioimmunoassay. Detection limits reached were 1-5 ppb. The concentration-time curves obtained reflected a long elimination phase and allowed only calculation of this half-life. Elimination half-life was estimated to be for muscle, blood and urine 160-170 hours, for kidney 310 and for bile 250 hours. Significant correlations were found to exist between CAP-concentrations in plasma and muscle. It appears that blood would be a good body fluid for monitoring CAP-residues in tissue.

Intravenous cis-platinum for invasive bladder cancer. Safety and feasibility of a new approach.

The five-year survival rate of patients with locally invasive bladder cancer (stages T3-T4, Nx, Mo) is less than 50% whatever treatment is given. In an effort to improve the prognosis of patients with this disease, we have incorporated into a management protocol two cycles of intravenous administration of cis-platinum before surgery or radiotherapy (which function as the "definitive" treatment). In 10 of the first 12 patients treated, tumour necrosis has been demonstrated histologically, and definite clinical responses have been observed in eight patients after two cycles of cis-platinum treatment. The toxicity has been acceptable, and subsequent management has not been compromised.

[Toxicologic survey of findings in putrefied corpses. Corpse putrefaction in air].

Autopsies were conducted on 234 putrefied corpses (exposed to the air, not buried) at the Institute for Forensic Medicine of the University of Hamburg from January 1979 to May 1983. Estimating the time it had taken to reach the respective degree of putrefaction was almost impossible. Only anamnestic clues provided evidence to ascertain the approximate time of death (between 3 days and 46 months ago). The cadavers were found in different locations: outdoors, inside heated or non-heated rooms. Some corpses wore clothes; others were naked; some were covered with blankets or other material. Chemicotoxicological analyses consisted of routine methods and additional modern sophisticated techniques: capillary gas chromatography, combined GC/MS, infrared spectrometry, radio- and enzyme-immunological tests. In organ specimens and body fluids of 47 corpses organic medicaments or poisons could be detected. Nearly all of these 47 forensic autopsies were carried out by request of the legal authorities to rule out legal intoxication as well as the influence of illegal drugs or medicaments. Unlike 20 years ago, today it is possible to detect and quantify organic drugs and poisons as well as their metabolic products, especially minor or major tranquilizers or barbiturates, in highly putrefied specimens. In spite of analytical progress, interpretation of some chemotoxicological results remains difficult. Poor anamnestic data provide insufficient clues. Therefore, pathologicoanatomical and histological results have special significance for the final conclusions. Good cooperation between the pathologist and toxicologist is one of the imperative prerequisites to come to a satisfying result.

[Results of operative treatment of femur fractures in polytraumatized children (author's transl)].

Operative treatment of femur fractures in 20 polytraumatized children had worse after-results than conservative treatment in 14 comparable fractures except for the CCD angle. The indication for operative treatment should be made very cautiously, also in polytraumatized children. The indications are listed. The most frequency indication for operative treatment after polytrauma was long-lasting severe motor agitation due to cerebral contusion.

Interaction of deaf and hearing preschool children.

Hearing and hearing-imparied children attending an integrated preschool were observed in a free-play situation. The principal objective was to discern how hearing and deaf children interacted and modified their communication skills as a function of hearing status. The data indicate that hearing children interact more frequently with other hearing children on several behavioral categories (approaches, vocalizations, social play, and physical contact). Although there was a tendency for deaf children to approach other deaf children more frequently than they approached hearing children, they showed no preferences for other interactions as a function of peers' hearing status. These findings are consistent with those of research on other exceptional children, such as the mentally retarded, in showing that, while hearing-impaired children showed no peer preference, they were least preferred by normal peers.
