Effect of doxycycline on pre-menstrual syndrome: a double-blind randomized clinical trial.

Thirty patients with well-defined symptoms of pre-menstrual syndrome were randomly treated with the antibiotic doxycycline or placebo. The antibiotic-treated group showed a highly significant reduction of symptoms. Subsequent antibiotic treatment of the original placebo group similarly diminished the symptoms in this group. A 6-month follow-up demonstrated that the improvement in symptom scores was permanent and independent from the presence of the antibiotic. Luteal phase endometrial biopsies showed a high incidence of out-of-phase endometrium. An unexpectedly high percentage of endometrial biopsy cultures yielded positive findings for mycoplasma, Chlamydia trachomatis and anaerobic bacteria. There were no characteristic hormonal changes in this study group. An infectious aetiology, possibly a sub-clinical endometrial or ovarian infection, behind certain cases of pre-menstrual syndrome is postulated.

Reassessment of external insulin infusion pumps.

OBJECTIVE: To discuss the potential role of continuous subcutaneous insulin infusion (CSII) therapy in patients with insulin-dependent diabetes mellitus (IDDM). DATA SOURCES: Published studies describing intensive insulin therapy are reviewed and evaluated. DATA SYNTHESIS: CSII delivers insulin at a preset basal rate and at bolus doses, when needed, throughout the day. Although this technology allows for greater lifestyle flexibility, the risks of hypoglycemia and ketoacidosis are also increased if the pump malfunctions. Studies have shown that CSII therapy may offer some advantages over conventional insulin therapy; however, the full impact of these benefits has yet to be determined. The National Institutes of Health has recommended seven commercially available insulin pumps for patient use. Cost ranges between $2000 and $3500; the average lifespan is about five years. CONCLUSIONS: CSII may be a viable alternative to multiple daily injections for maintaining glycemic control in patients with IDDM who require intensive insulin therapy.

Management needs of head nurses and supervisors: designing a continuing education course.

Studies have confirmed the need for continuing education (CE) management courses for hospital nurse managers facing new challenges as decentralization has occurred. Increased problem-solving and decision-making skills are necessary as middle nurse managers attempt to manage more effectively in the current healthcare climate. Head nurses and supervisors may well have the most stressful positions in hospitals. A needs assessment of these individuals was conducted in order to design a management course to provide practical knowledge for contemporary nurse managers.

Nurses' knowledge of the code for nurses.

"Investigation of Professionalism in Nursing Behaviors" was funded by the Beta Upsilon Chapter of Sigma Theta Tau International and the College of Nursing, Arizona State University. Researchers asked 514 nurses in eight western states if they had a copy of the Code For Nurses. Most respondents did not have a copy of the Code; however, they indicated that adherence to the Code was essential to the professional nurse. Years of practice, educational background, participation in autonomous activities, and present position were variables used to determine those respondents most likely to possess a copy of the Code. Implications for continuing educators are discussed.

[Lacrimal gland hypertrophy: revealing sarcoidosis].

We present a case of systemic sarcoidosis in a 34-year-old woman initially presenting with bilateral and symmetric proptosis caused by lacrimal gland enlargement. Based upon clinical, biological and radiological findings, sarcoidosis was suspected with lacrymal gland, parotid and pulmonary lesions. Biopsy of enlarged lacrimal gland for histological examination revealed a non caseating granuloma compatible with the diagnosis of sarcoidosis. Sarcoid lesions regressed with corticosteroid therapy.

Chondromyxoid fibroma of a metatarsal.

The authors present a case report on a cartilage producing tumor of bone. Because of its rare nature and atypical form afflicting the foot, diagnosis is a challenge. Often baseline examinations such as xerograms, radiographs, and blood values will not present all of the information necessary for diagnosis. However, when coupled with magnetic resonance imaging, CT scan and/or bone scan, information important to the staging and preparation for definitive treatment can be secured, although many lesions of bone cannot be definitively diagnosed until such time that a histologic examination is performed.

Guidelines for doctors on identifying and helping their patients who batter.

While there are a growing number of medical guides for assisting physicians to identify and help victims of domestic violence, there has been scant attention to how physicians can best respond to perpetrators. The medical model's deficient grasp of violence, combined with the minimizing and excuse-making strategies employed by batterers hinder physicians' ability to detect batterers in their practices and to prescribe the right solutions. Earlier detection is possible, however, when doctors adopt routine diagnostic procedures for all patients and ask informed follow-up questions when there are indications of domestic violence. Finally, physicians should become aware of the effective batterer treatment programs in their areas and make this information easily available to their patients who batter.
