Foundations of arrhythmia interpretation.

Basic arrhythmia monitoring is being used with increasing frequency in both inpatient and outpatient care settings. Nurses are being asked to interpret cardiac rhythms and develop critical thinking skills that assist in evaluating the significance of the rhythms identified for individual patients. The foundations needed for nurses to evaluate cardiac rhythms and assess the significance of the rhythms identified are provided. Cardiac anatomy, physiology, and cardiac properties are used to provide a framework for interpreting and understanding rhythms.

Managing dobutamine infusions.

Each year, over 400,000 new cases of congestive heart failure (CHF) are diagnosed. Dobutamine, a synthetic inotropic agent, is used to boost myocardial function. While in the past dobutamine has been administered to patients under close observation in critical-care units, this treatment is now considered safe and cost-effective for use with patients on general medical-surgical units and even in the home when strict monitoring guidelines are followed. In these settings, assessment protocols and schedules guide nursing practice. Assessment priorities and suggested parameters for their frequency provide guidelines for nursing management.

Assessing staff competency.

In the absence of a national credentialing center for nursing and in the interest of ensuring competencies, Forbes Regional Health Center has developed a framework for internal credentialing. This framework, while initiated in a specialty area, has application to all areas of acute care including the medical-surgical areas. Program development within this framework provides a consistent, structured method of documenting selected clinical proficiencies and facilitates the achievement of individual learning needs.
