Plasma growth hormone (GH) levels were measured during the first 2 hours of sleep without electroencephalogram monitoring and then after arginine infusion in 28 children investigated for short stature. Ten children considered GH deficient on clinical and biochemical grounds had concordantly low GH levels during sleep and after arginine stimulation. Of the 18 children without GH deficiency, 17 had GH levels greater than or equal to 15 mU/l during the sleep test (mean peak 39 mU/l) and 13 had GH levels greater than or equal to 15 mU/l after arginine infusion (mean peak 25 mU/l). A sleep test is safe, reliable, and practicable for routine clinical use.
Samples of saliva (4 in 24 hours), collected at monthly intervals for the first 6 months of life in 8 term infants by their mothers, were analysed for cortisol by radioimmunoassay. Values in the first month were more variable, daily mean values were greater, and amplitudes of variation were greater than in subsequent months. The circadian rhythm appeared by the third month.
A 10 1/2 year old boy presented with raised intracranial pressure and precocious puberty caused by a human chorionic gonadotrophin (HCG) secreting pineal germinoma. After craniospinal irradiation he improved clinically and his HCG concentration fell to an undetectable value. Two years later he remains well. Pineal germinomas are highly radiosensitive obviating the need for difficult surgery associated with high mortality.

The molluscan neuropeptide FMRFamide (Phe-Met-Arg-Phe-NH2) has diverse actions on excitable tissues of molluscs, including hearts, noncardiac muscles, complex organs, and neurons. The intracellular transducing mechanisms are also diverse and are not readily correlated with particular responses. FMRFamide increases cyclic AMP levels concomitant with both cardioexcitation and inhibition, but not with muscle contraction. In the same tissues, the effects of 5-hydroxytryptamine are dissimilar and are always accompanied by a cyclic AMP increase. FMRFamide and acetylcholine cause similar tonic contractions of the Busycon radula protractor muscle and identical catch contractures of the mytilid anterior byssus retractor muscle, but the ionic basis of excitation and the sources of activator calcium for contraction are not the same for the two agonists. A comparative study of structure-activity relations showed that FMRFamide receptors are heterogeneous. Helix aspersa ganglia contain no FMRFamide, but a close analog occurs and has been tentatively identified. Evidence supporting a proposed homology between FMRFamide-like and opioid peptides is summarized. The effects of the amphiactive heptapeptide Tyr-Gly-Gly-Phe-Met-Arg-Phe-NH2 on the venus clam rectum support this hypothesis.
We describe an enzyme-linked immunosorbent assay for urinary albumin, performed on microtiter plates with use of commercially available antisera and peroxidase conjugate. The assay range is 3-1000 micrograms/L, the sensitivity 625 pg. The method is suitable for measurement of albumin excretion in either normal or pathological urine. For 20 normal children, the range of urinary albumin excretion was 1.7-22.9 mg/24 h.
Analogs of the molluscan neuropeptide FMRFamide were tested on four different molluscan muscle preparations which show qualitatively different responses to the peptide; the structure-activity relations are basically similar, but not identical. The C-terminal amide and the Arg3 residue are critical for FMRFamide-like activity on all four preparations. In contrast, analogs extended at the N-terminal or with conservative substitutions for the Phe1 or Met2 residue are approximately equipotent to FMRFamide. These structural requirements parallel those for the C-terminal tetrapeptide amide of gastrin.

Adriamycin, bleomycin, 5-fluorouracil and methotrexate have been used in combination in a Phase II study and as adjuvant chemotherapy for patients with locally advanced bladder cancer. Although early results with metastatic disease showed considerable antitumour activity, longer follow-up has shown this drug combination to be ineffective.
