Interferon in synovial fluid and serum of patients with rheumatic disease.

Interferon (IFN) was sought in simultaneously obtained samples of synovial fluid (SF) and serum from patients with a variety of rheumatological diseases. IFN was identified in 11 of 84 samples of SF and 10 of 84 specimens of serum. IFN positive specimens were contributed by patients with rheumatoid arthritis (RA), systemic lupus erythematosus, seronegative spondyloarthropathies, pseudogout, coccidioidomycosis and unclassified arthritis. Significantly, IFN was frequently found either in SF or in serum but generally not in both of the simultaneously obtained fluids. The highest titers of IFN found in SF were from patients with RA.

N-formimidoyl thienamycin (MK0787): in vitro activity against anaerobic bacteria.

The in vitro activity of N-formimidoyl thienamycin (MK0787) was tested against 239 anaerobic bacteria clinical isolates: 70 of Bacteroides fragilis, 18 of B. distasonis, 16 of B. thetaiotaomicron, 10 of B. vulgatus, 24 of Bacteroides spp., 22 of B. melaninogenicus (all three subspecies), 26 of Fusobacterium spp., 10 of Peptococcus spp., 15 of Peptostreptococcus spp., 15 of Clostridium perfringens, and 13 of Clostridium spp. Ninety-five percent of the isolates were inhibited by less than or equal to 0.125 microgram/ml, and all were inhibited by less than or equal to 4 micrograms/ml.

Telangiectasia and optic atrophy in cone-rod degenerations.

The diagnosis of cone-rod dysfunction is made with the electroretinogram (ERG). Characteristically, the photopic ERG is worse than the scotopic ERG, and both are abnormal. Of a larger group of patients with retinal dystrophy, 20 cases of cone-rod dysfunction were identified. All patients had progressive disease, and all three main modes of inheritance were represented. There was remarkable similarity of findings among all patients, including temporal disc atrophy, telangiectasia of disc vessels, and little to no pigmentary retinal changes. Patients were not night-blind unless advanced disease was present. These patients suggest that neither telangiectasia nor temporal optic atrophy is pathognomonic for Leber's optic neuropathy nor dominant optic atrophy, respectively. Using a combination of electrophysiologic testing, fundus changes and modes of inheritance may prove helpful in better classifying the different types of retinal dystrophies, including retinitis pigmentosa.

Evidence for an endorphin dysfunction in methadone addicts: lack of ACTH response to naloxone.

Chronic exogenous opiate administration might be responsible for the acute and protracted abstinence syndrome by producing a prolonged decrease in the availability of endogenous opioids (endorphins). However, the hypothesis that potent exogenous opiates may have anti-endorphin effects has been difficult to test. We have been investigating this hypothesis with neuroendocrine test paradigms which have provided preliminary evidence of anti-endorphin effects for chronic methadone. Naloxone-induced ACTH response data from chronic methadone addicts offers preliminary support for the hypothesis that chronic exogenous opiate administration has anti-endorphin effects. The subjects were 7 male methadone addicts who had been addicted to greater than or equal to 40 mg of methadone and 7 male healthy opiate-naive volunteers. Naloxone failed to produce a significant increase in ACTH in methadone addicts while opiate-naive normal volunteers demonstrated a significant naloxone-induced release of ACTH. Five of the seven methadone addicts ahd no demonstrable ACTH response to naloxone. These impaired naloxone response data reported here for recently detoxified addicts suggest that chronic methadone administration comprises the functional integrity of the endorphin system. Prolonged abstinence, post-detoxification depression and other affective symptoms which contribute to relapse may result from a prolonged endorphin derangement.

Emergency psychiatric home visiting: report of four years experience.

An Emergency Psychiatric Home Visiting Team made 624 visits to 443 patients in a 4 year period. In situations where patients were frequently psychotic, violent, or threatening violence, the team was sucessful at facilitating emergency care and defusing crisis situations. The team was equivocally successful at preventing hospitalization. Facilitation of hospitalization often became a more important function than prevention. Few visits were for suicidal behavior. No team member was ever harmed in the course of a visit. The team also served as a major psychiatric care provider and social support to a group of chronic patients and families who utilized the service multiple times.

Pharmacy in family practice: a description and evaluation.

Expanded pharmacy functions within a private practice setting are described. These activities are a component of a pharmacy training program. The medical practitioners evaluated the program, eliciting an overall positive response. Additionally, future services were priority rated by the staff, demonstrating more accessibility to the services, patient education, and chronic disease care programs as the most desirable projects. As the Clinic and the medical residency expand, the activities of the clinical pharmacist shall continue to develop.

Human energy expenditure during level walking on a treadmill at speeds of 54-130 m min-1.

Controversy exists in the literature as to the accuracy of the formulae used in predicting oxygen consumption (VO2) during level walking. The purpose of this study was to develop an equation for use at speeds of 54-130 m min-1. The VO2 and energy expenditure (EE) of 25 healthy subjects were measured while walking on a level, motor-driven treadmill at four varying speeds (54, 81, 107 and 130 m min-1). The regression analyses revealed that a quadratic model was a better predictor (p less than 0.05) of both the VO2 and EE than a linear model. The quadratic equation relating VO2 and EE to walking speed (X) is as follows: VO2 (ml kg-1 min-1) = 17.77 - 0.3023X(m min-1) + 0.0027X2 (R2 = 0.935, SEE = 1.5) EE (kcal min-1) = 6.14 - 0.1076X (m min-1) + 0.00093X2 (R2 = 0.830, SEE = 0.89).
