Cytologic features of islet-cell tumors.

Although a number of reports have demonstrated the accuracy of fine needle aspiration (FNA) in the diagnosis of nonendocrine pancreatic carcinomas, the cytomorphology of islet-cell tumors (pancreatic endocrine tumors) is not well defined. This paper describes the cytologic features of three histologically confirmed cases of islet-cell tumors. The three tumors occurred in one man and two women, who were 63, 64 and 70 years of age, respectively. Each patient underwent FNA of pancreatic mass with computed body tomography guidance. The aspirates contained large numbers of tumor cells in two cases and a smaller number in one case. The cells, distributed singly and in small groups, were small and round or polygonal, with scant to more often abundant, dense or granular cytoplasm. The nuclei were often located eccentrically and were round to oval, with smooth nuclear borders and finely stippled chromatin. Many nuclei contained a single nucleolus. Multinucleated cells were present and generally contained two or three nuclei. Although the diagnosis of islet-cell tumors in fine needle aspirates is difficult, the cytomorphologic features of these tumors are sufficiently distinctive to suggest the diagnosis, especially when a relatively monomorphic population composed predominantly of single cells is present.

Septic Discitis.

The clinical and laboratory findings in 15 patients with septic discitis are reported. The clinical picture was that of a sub-acute illness characterized by back pain, spinous process tenderness, and sciatic nerve root irritation. There was a considerable delay from clinical presentation to diagnosis (average 14 wk). The erythrocyte sedimentation rate was the most useful diagnostic laboratory blood test and the best indicator of disease activity. The relative value of plain radiographs, tomography, and radionuclide bone scans is discussed. Needle aspiration of the disc space was found to be a useful diagnostic test. We found that a diagnosis of septic discitis could be made even in the absence of positive bacterial cultures from various biologic fluids or from the disc space. Long-term oral and intravenous antibiotic therapy with controlled rest resulted in a favourable outcome.

Coexistence of rheumatoid arthritis in married couples: a search for etiological factors.

Epstein-Barr virus (EBV) and the genetic marker HLA-DR4 both appear to play a role in the etiology of rheumatoid arthritis (RA). Antibodies to viral capsid antigen and the early antigen complex of the EB virus as well as the HLA phenotype were studied in 6 families. Both spouses had classical or definite RA but none of their offspring, all adult, had any evidence of RA. At least 1 spouse of each couple had DR4 and this phenotype was found in 4 of the 8 offspring. All parents and their offspring had been infected with EBV. This study supports the association of DR4 and RA. The presence of DR4 in 4 normal adult offspring was not associated with the absence of primary infection or enhanced levels of EBV-specific antibodies.

Reticuloendothelial function in rheumatoid arthritis: correlation with disease activity and circulating immune complexes.

Fourteen patients with active rheumatoid arthritis had their reticuloendothelial function tested with clearance of heat damaged radiolabeled autologous erythrocytes. Only 3 of these 14 patients had prolonged red blood cell (RBC) clearance. No correlation was found between RBC clearance and level of circulating immune complexes, disease activity or disease duration.
