[Antitumour study with combined IL-2 gene therapy and radiation for head and neck squamous cell carcinoma].

OBJECTIVE: To assess the efficacy of combination therapy with IL-2 gene transfer and radiation in an immunocompetent murine model that parallel more closely the clinical therapy of head and neck Squamous cell carcinoma(HNSCC). METHOD: Tumors were established in the floor of mouth in C3H/HeJ mice with SCC VII cell line. Lipid-DNA complexed (lipoplexes) by using polycationic liposome-Mediated transduction for HNSCC were transducted in tumor-bearing mouse by direct intratumoral gene transfer. The local tumor radiation with 2 Gy were done in second day. Tumor size were measured before and after the treatment as compared to different single treatment groups and the controls. After tumors were subcultured, the supernatants were collected for IL-2 expression by enzyme-linked immunosorbent assay (ELISA). Natural killer (NK) cell activity and cytotoxic T-lymphocyte (CTL) activity were also assayed by LDH method. CD4+ and CD8+ T-lymphcyte in tumour tissues were examined by immunohistochemistry. RESULT: HNSCC tumor growth was significantly inhibited after a combined IL-2 gene and radiation therapy as compared to the controls. Increased secreted levels of IL-2 protein expression were found in combined and single IL-2 gene treated groups. The combination and IL-2 gene treated groups produced greater activation of CTL and NK than the other groups. The significant CD4+ and CD8+ T lymphocyte infiltration was distributed in tumor tissues after IL-2 gene therapy. CONCLUSION: Combined IL-2 gene therapy and radiation could significantly inhibited HNSCC tumor growth in the murine model and efficiently induced antitumor immunity of the host.

[The comparative study of clinical therapy of secretory otitis media in nasopharyngeal carcinoma patients].

OBJECTIVE: To investigate the treatment of secretory otitis media in nasopharyngeal carcinoma (NPC) patients. METHOD: Fifty eight patients of NPC with secretory otitis media were divided into two group by random before radiotherapy. Patients were treated by nasal and nasopharyngeal debridement and cleansing and tympanic membrane puncturing suction in group I, while patients were treated by myringotomy or ventilation tube insertion in Group II. After the patients were followed up for six months, the two group were compared by efficient rate and complication rate. RESULTS: Fifty one out of total 58 patients survived after half an years follow-up. Statistics of surviving patients showed the efficient rate in group I was 82.1% versus 57.1% in group II. Differentiation was found between two group (chi 2 = 4.14, P < 0.05). Complication rate was 3.6% in group I and 28.6% in group II. There was differentiation between two group (chi 2 = 4.7, P < 0.05). CONCLUSION: The nasal and nasopharyngeal debridement and cleaning and eardrum puncturing suction was superior to myringotomy or ventilation tube insertion.

[Study on rabbit mesenchymal stem cells differentiation to the adipogenic or osteogenic lineage in vitro].

Rabbit bone marrow-derived mesenchymal stem cells(MSCs) are multipotent. We studied the adipogenic and osteogenic differentiation potent using adipogenic supplement (AS) or osteogenic supplement (OS) in vitro. Specific markers of this induced adipogenic and osteogenic lineage were identified. The findings showed that the rabbit MSCs are capable of differentiating into adipogenic and osteogenic lineages spontaneously. On the 21st day, approximately 75% rabbit MSCs were induced to adipogenic or osteogenic cells in medium containing AS or OS, respectively. These results demonstrated that the differentiation of MSCs could be regulated in vitro. The underlying molecular mechanisms of adipogenic or osteogenic differentiation await elucidation.

[The expression and relevent study of NO and TNF-alpha in nasal polyp tissue].

OBJECTIVE: To study the expression of NO and TNF-alpha in nasal polyp tissue and explore the effect and relevance of them in the formation of nasal polyp. METHOD: The level of NO and TNF-alpha were measured by RIA and chemical method (Griess Reaction) in nasal polyp tissue, inflammatory mucosa and serum of 33 nasal polyp patients and normal mucosa and serum of 10 deviated septum patients who hospitalized in our hospital from Nov. 1999 to Mar. 2001. RESULT: The level of NO and TNF-alpha in nasal polyp and inflammatory mucosa are higher than that in normal mucosa (P < 0.01). There is significant difference of NO between nasal polyp and inflammatory mucosa (P < 0.01) but not of TNF-alpha (P > 0.05). The level of NO and TNF-alpha in serum of two groups are similar (P > 0.05). There are negative relevance tendency between NO and TNF-alpha in nasal polyp of inflammatory mucosa (r = -0.411, P < 0.02 and r = -0.476, P < 0.01). There is a positive relevance tendency of TNF-alpha between nasal polyp and inflammatory mucosa (r = 0.808, P < 0.01). CONCLUSION: 1. NO and TNF-alpha play important roles in the formation of nasal polyp. 2. There is a negative relevance tendency of NO and TNF-alpha in nasal polyp tissue which reveals the possible negative feedback of NO in the inflammation. 3. The study confirms the theory of microenvironment of nasal polyp and reveals that nasal polyp is separated relatively from and associated closely with the inflammation of nasal cavity.

[Using two-dimensional gel electrophoresis to analyze the regeneration microenvironment of facial nerve and its response to acupuncture].

OBJECTIVE: To investigate the distribution of proteins in the regeneration microenvironment of facial nerve and its response to acupuncture. METHODS: We created an animal model using rabbit to imitate the regeneration microenvironment of facial nerve and analyzed the proteins in the microenvironment through the two-dimensional gel electrophoresis. RESULTS: We found that most proteins in the regeneration fluid were under relative molecular mass 80 x 10(3) and in the range of pI 4-7. An average of 800 +/- 53 protein spots was found in the control group compared to the 850 +/- 78 spots in the acupuncture group. In the matching spots, more than ten proteins were different between the two groups. CONCLUSION: There is no evidence to show that acupuncture has a positive effect on the regeneration microenvironment of facial nerve.

[Expression of oncogene C-jun in the cells of rat nasal mucosa and relevance with rhinorrhea].

OBJECTIVE: To study the expression of oncogene C-jun in rat nasal mucosa with allergic rhinitis and the relevance with rhinorrhea. METHOD: By using EA to make animal model of allergic rhinitis successfully; the amount of rhinorrhea was calculated at 30 min. 4 h, 8 h. Immunohistochemical technique, computer image analysis technique and histologic examination were used to investigated the expression of C-jun in rat nasal mucosa at the same time. The relevance between them was used explored. RESULT: There was a lot of C-jun protein at 30 min. The difference of C-jun between 30 min group and control group was significant (P < 0.01) and the relevant coefficient was r = 0.784 (P < 0.05). The C-jun protein decreased at 4 h, but there was still significant difference (P < 0.05) and r = 0.626 (P < 0.05). The C-jun was as same as control group at 8 h, There was no difference between them. CONCLUSION: The expression of oncogene C-jun in inflammatory cells may be a marker in early stage of activation of inflammatory cells, and related to the producing and secreting of cytokines and inflammatory mediators in inflammatory cells of allergic rhinitis.

[Studies of mouse interleukin-2 gene therapy for head, and neck sequamous cell carcinoma using polycationic liposome-mediated transduction].

OBJECTIVE: To investigate the immunological mechanism of mouse IL-2 gene therapy and the optimal lipid to DNA ratios of lipid-DNA complexed (lipoplexes) by using polycationic liposome-mediated Tumors were established in the transduction for head and neck squamous cell carcinoma (HNSCC). METHODS: floor of mouth in C3H/HeJ immunocompetent mice with SCCVII cell line. Lipoplexes with various lipid to DNA ratios (L:D) and naked DNA were transducted in vivo by direct intratumoral gene transfer and in vitro. The supernatants of SCCVII cell and tumour tissues were collected for IL-2 expression by enzyme-linked immunosorbent assay. Natural killer (NK) cell activity and cytotoxic T-lymphocyte (CTL) activity were also The optimal L:D ratio for IL-2 expression in vitro was not assayed by lactate dehydrogenase method. RESULTS: consistent with that in vivo. By use of lipoplexes with L:D = 3:1, higher IL-2 expression of tumor tissues and greater activities of NK cell and CTL of murine spleen were noted in the treated group as compared with those A comparison of naked plasmid and lipid-complexed found in naked DNA and empty plasmid (EP). CONCLUSION: DNA revealed that lipoplexes were more effective for intratumoral gene transfer to HNSCC. The results indicate that the formulation and dosage of polycationic L:D complexes play a key role in determining the level of intratumoral transgene expression.

[The relationship of p53 gene mutation to cell differentiation and metastasis of laryngeal squamous cell carcinoma].

OBJECTIVE: To inquire about the relationship of p53 gene mutation to the histopathological findings and clinical manifestation in cases of laryngeal squamous cell carcinoma(LSCC). METHODS: The fresh samples from 60 cases of LSCC were examined. Polymerase chain reaction and silver staining-single strand conformation polymorphism (PCR-SSCP) and DNA direct sequencing were used to detect the mutation of p53 gene in exons 5-8. RESULTS: The mutation rates were 69.2% and 85.3% in patients at clinical stage I-II and stage III-IV respectively (P>0.05). In the well-, moderately- and poorly-differentiated cell of LSCC, the mutation rates were 52.9%, 83.3% and 94.7% respectively (P<0.05). The p53 gene mutation rate of LSCC patients with neck lymph-node metastasis was 96.4%, whereas that of patients without neck lymph-node metastasis was 62.5% (P<0.05). Twenty samples showed positive results in SSCP; 19 samples showed deletion and mutation in codons 125-292 by DNA direct sequencing. CONCLUSION: The mutation of p53 gene in exons 5-8 was closely related to cell differentiation and the neck lymph-node metastasis of LSCC, but it was not related to the clinical stages of the LSCC cases.

Combined IL-2 and IL-12 gene therapy for murine head and neck squamous cell carcinoma.

OBJECTIVE: To evaluate the efficacy of combined interleukin (IL)-2 gene and IL-12 gene therapy in murine model with head and neck squamous cell carcinoma (HNSCC). METHODS: HNSCC model was established in the mouth floor of C3H/HeJ mice with SCCVII cell line. Lipid-IL2 and lipid-IL12 plasmid complexes were introduced either alone or in combination into the tumor by direct intratumor gene injection. Tumor size was measured before and after the treatment to evaluate the response of the different treatment and control groups. Enzyme-linked immunosorbent assay (ELISA) was used to measure the IL-2 and IL-12 expression. Lactic dehydrogenase (LDH) assay was used to evaluate the activity of Cytotoxic T-Lymphocyte (CTL) and natural killer (NK) cells. RESULTS: Growth of HNSCC was significantly inhibited in combined IL-2 and IL-12 gene therapy group as compared with the other groups (P < 0.01). Increased level of IL-2 and IL-12 protein expression was found in both combined and single treatment groups. Greater activity of CTL and NK was also observed in these two groups as compared with the controls. CONCLUSION: Both IL-2 and IL-12 gene therapy is able to inhibit the growth of HNSCC and induce the host antitumor immune response efficiently in the murine model. Combination of the two in gene therapy may be additive or synergistic in antitumor effect.

[Experimental reconstruction of extensive anterior defect of rabbit trachea with the use of free auricular cartilage].

OBJECTIVES: To determine whether free auricular cartilage grafts can be used to reconstruct the extensive anterior defect of rabbit trachea and observe the difference between autograft and allograft. METHODS: Twenty New Zealand white rabbits were divided into autograft group (n = 10) and allograft group (n = 10). All grafts were taken from the right auricle, and defect included 8 to 10 rings of trachea. The gross morphorlogical features, endoscopic examinations, biomechanic determinations and histological findings of grafts were assessed at 1,2,4,8 and 12 weeks after operation. RESULTS: Eighteen rabbits survived. Mild tracheal stenosis was observed under endoscope. The maximum stress per mm at 0,4,8 and 12 weeks was 2.54 +/- 0.19, 1.31 +/- 0.21, 1.72 +/- 0.22 and 1.96 +/- 0.08 kPa/mm, respectively. Histological analysis revealed that the viable chondrocytes and neochondrocytes at 12 weeks accounted for 62.0% +/- 3.45%, 65.89% +/- 48% in the autograft group and 60.1% +/- 3.98%, 55.20% +/- 7.57% in the allograft group. No marked immunological differences between the auto- and allograft groups were noted. CONCLUSIONS: Free auricular cartilage can be used to reconstruct the extensive anterior defect of trachea in both auto- and allo-transplantations.

[A randomized controlled study on the efficacy of modified sclerotherapy in treating angioma of ear, nose and throat].

OBJECTIVE: To evaluate the efficacy and security of modified and traditional sclerotherapy in treating angioma in ENT. METHOD: For the Angioma of derma and hypodermis (naso face, auricles, neck) 56 cases and 82 cases of mucosa and hypomucosa (the cavities of nose, throat and mouth) were randomized divided into 2 groups each.: The 5% Sodium Morrhate + Triamcinolone Acetonide (The Modified Sclerotherapy n = 28, n = 41) and only 5% Sodium Morrhate (Traditional Sclerotherapy n = 28, n = 41) were respectively injected into bodies of Angioma, as one time a week for 2-6 weeks, on average 19 days. RESULT: The rates of cure and effective for Modified and Traditional Sclerotherapies were 84.1%, 65.2% and 91.3%, 85.5% respectively. There is a significant in statistics for effective rates and there is a notable difference in cured rates between the two therapies. It shows that Modified Sclerotherapy has an obvious advantage (P < 0.05). Although the efficacy is similar between the two therapies for angioma of mucosa and hypomucosa, the difference has a significant in efficacy for angioma of derma and hypoderma between the two therapies (P < 0.05) and the side-effects were very slight for Modified Sclerotherapy. CONCLUSION: There is a more prominent efficacy and security for Modified Sclerotherapy than Traditional Sclerotherapy.

[Analysis of randomized controlled trials on otorhinolaryngological diseases in China].

OBJECTIVE: To evaluate the quality of randomized controlled trials (RCT) in otorhinolaryngology in China and offer evidence for the improvement of RCT. METHODS: Five kinds of Chinese journals of clinical otorhinolaryngology were searched, and RCTs were identified and analyzed according to the standards of Evidence Based Medicine. RESULTS: Two hundred and eighty seven issues were referred to, and eighty-one RCTs were finally identified and analyzed. Of these RCTs, 34.57% (28/81) had definite diagnostic standards, 38.27% (31/81) had including standards and 33.33% (27/81) had excluding standards; only 1.23% (1/81) got the approval of the participants; 40.74% (33/81) had moderate sample size, 3.70% (3/81) had large sample size and non of them mentioned sample size estimation; 81.48% (66/81) didn't report the method of randomization and 38.27% (31/81) had baseline comparison; 18.52% (15/81) didn't define the control interventions and 8.64% (7/81) even didn't explicate the experimental intervention; 32.10% (26/81) used blank comparison; 86.42% (70/81) didn't use blindness; 37.04% (30/81) didn't mention the adverse effects; 23.46% (19/81) used accredited standards to evaluate the outcomes; 11.11% (9/81) mentioned the loss of following-up and only 1.23% (1/81) treated the loss with statistics methods. CONCLUSION: The quantity and quality of otorhinolaryngological RCTs couldn't meet the clinical need. More high quality RCTs are required to improve the level of prevention and cure of otorhinolaryngologic diseases.

[Vasodilators for sudden sensorineural hearing loss: a systematic review of randomized controlled trials].

OBJECTIVE: To assess the effects and safety of vasodilators on sudden sensorineural hearing loss(SSHL). SEARCH STRATEGY: Electronic databases: MEDLINE from 1966, EMBASE from 1974, the Cochrane Controlled Trails Register, Chinese Bio-medicine Database from 1989. Hand search: Five kinds of Chinese otolaryngology journals were selected. Literature references were checked intensively. SELECTION CRITERIA: Randomized controlled trials comparing vasodilators with placebo or other drugs in patients with SSHL. DATA COLLECTION AND ANALYSIS: Three reviewers independently accessed the quality of trials and extracted the data. RESULTS: Thirteen trials with 1,155 patients were eligible and covered in the systematic review. Ten of the trials were conducted in developed countries, and three in China. None of the four trials showed the effects of vasodilators on SSHL were better than that of placebo. None of the seven trials comparing different drugs showed the effects of one kind of vasodilators were better than that of the other vasodilators. Two trials showed that some other drugs, such as batroxobin and hypaque were probably better than the vasodilators (dextran, papaverine, 654-2, danshen). Eight trials showed side effects of vasodilators, such as pruritus, allergy, etc. CONCLUSIONS: Based on the systematic review of current eligible randomized controlled clinical trials, there is no evidence to prove that vasodilator therapy is more effective than placebo or other therapies for SSHL, or the effects of one kind of vasodilators to be better than that of the other vasodilators. So far we can't draw a reliable conclusion about the effects of vasodilators for SSHL. In addition, we must pay attention to their potential adverse reactions.

[Preliminary clinical study of application of cyntridini triphosphatis and dexamethasone in treatment of sudden deafness].

OBJECTIVE: To investigate the effect of application of cytridini triphosphatis (CTP) combined with dexamethasone on sudden deafness. METHODS: Two-hundred and seven patients (312 ears) with sudden deafness were randomly divided into CTP group (n = 159 ears) and control group (n = 153 ears). Besides intravenous administration of CTP, another treatments in CTP group were the same as that in control group including hyperbaric oxygen, vasodilator, energy preparation such as ATP, CoA and dexamethasone (DXM). RESULTS: The hearing threshold was (75.56 +/- 30.24) dB HL in CTP group and (72.50 +/- 40.50)dB HL in control group (P > 0.05) before treatment. The average value of the hearing threshold after treatment was decreased by (50.08 +/- 21.47) dB HL in CTP group and (19.45 +/- 19.12) dB in control group(P < 0.05), respectively. CONCLUSION: The application of CTP combined with DXM can significantly improve the effect of treatment on sudden deafness and CTP may enhance the recovery rate in the patients with sudden deafness, which was much higher than that treated by the traditional methods. However, the mechanism of the effect of CTP coupled with DXM is still unknown and further study is necessary.

[Tissue-engineered auricled cartilage: an experimental study].

OBJECTIVE: To study the feasibility of engineering auricled neocartilage with chitosan/polylacticacid-polycrylactone (PLA-PCL) network scaffolds and to search the difference between dynamic and silent chondrocytal culture techniques. METHODS: Chondrocytes from auricled cartilage of 4 weeks old New Zealand White rabbit were seeded onto chitosan/PLA-PCL network. Ten cell-polymer scaffolds were divided into two groups: dynamic group (n1 = 5) with rotating bioreactors and silent group (n2 = 5) with ordinary dishes to culture. Using scan electroscope, grossly histological and immunohistological techniques, the morphological evaluation was done individually at 1st week in vitro, 4th and 8th week in vivo. RESULTS: Chondrocytes adhered and grew up well on the network, but more quantities of chondrocytes, Glycoaminoglygan (GAG) and type II collagen were found in dynamic group. There was an obvious difference between dynamic and silent group (P < 0.05). CONCLUSION: Chitosan/PLA-PCL network scaffold is good for adhesions and growth of chondrocytes. Furthermore, dynamic cell culture method is better than silent method for formation of neocartilage.

[Expression of p15 protein in poorly differentiated nasopharyngeal carcinoma].

OBJECTIVE: To explore the role of p15 protein in the pathogenesis of poorly differentiated nasopharyngeal carcinoma (NPC). METHOD: Using immunohistochemical LSAB (Labeled Streptavidin Biotin method), we examined 55 samples of poorly differentiated NPC and 15 samples of nasopharyngeal normal tissue for expression of p15 protein. RESULT: The p15 protein expression was located in the cytoplasm and nuclear membranes. The positive rate of p15 protein in NPC is significantly lower than that in normal tissue (P < 0.05). Important correlations were found between the p15 protein expression and clinic stages, T classification of NPC. But p15 protein wasn't correlated to neck lymph node metastasis and 5-years survival rate. CONCLUSION: The absence of p15 protein may play an important role in the pathogenesis and development of NPC, the expression of p15 protein may be used to evaluate the biological behavior of NPC.
