Effect of Metagenomic Next-Generation Sequencing on Clinical Outcomes of Patients With Severe Community-Acquired Pneumonia in the ICU: A Multicenter, Randomized Controlled Trial.

BACKGROUND: Metagenomic next-generation sequencing (mNGS) was previously established as a method that can increase the pathogen identification rate in patients with severe community-acquired pneumonia (SCAP). RESEARCH QUESTION: What is the impact on clinical outcomes of mNGS of BAL fluid (BALF) in patients with SCAP in the ICU? STUDY DESIGN AND METHODS: A multicenter randomized controlled open-label clinical trial was conducted in 10 ICUs. Patients were randomized in a 1:1 ratio to undergo BALF assessment with conventional microbiological tests (CMTs) only (ie, the CMT group) or BALF assessment with both mNGS and CMTs (ie, the mNGS group). The primary outcome was the time to clinical improvement, defined as the time from randomization to either an improvement of two points on a six-category ordinal scale or discharge from the ICU, whichever occurred first. RESULTS: A total of 349 patients were randomized to treatment between January 1, 2021, and November 18, 2022; 170 were assigned to the CMT group and 179 to the mNGS group. In the intention-to-treat analysis, the time to clinical improvement was better in the mNGS group than in the CMT group (10 days vs 13 days; difference, -2.0 days; 95% CI, -3.0 to 0.0 days). Similar results were obtained in the per-protocol analysis. The proportion of patients with clinical improvement within 14 days was significantly higher in the mNGS group (62.0%) than in the CMT group (46.5%). There was no significant difference in other secondary outcomes. INTERPRETATION: We found that compared with the use of CMTs alone, mNGS combined with CMTs reduced the time to clinical improvement for patients with SCAP. Measurement of intraocular pressure after LASIK by dynamic contour tonometry.

Changes of corneal properties induced by laser in situ keratomileusis (LASIK) results in low inaccurate intraocular pressure (IOP) readings by Goldmann applanation tonometry (GAT). Before and after LASIK, the applied value of IOP, measured by dynamic contour tonometry (DCT) in comparison to GAT, was evaluated. Before and 1, 4 weeks after LASIK, the IOP in 30 cases (60 eyes) was measured by GAT and DCT respectively. The obtained results were statistically processed by SPSS11.5 statistical software. The results showed that central corneal thickness (CCT) could affect GAT measurements but not DCT measurements. The comparison of IOP one and 4 weeks after LASIK revealed that the readings from GAT was separately decreased by 5.00 +/- 1.12 and 5.45 +/- 1.13 mmHg as compared with those before LASIK, while those from DCT had no significant difference. It was concluded that LASIK-induced changes of CCT could influence the accuracy of GAT measurements, but had no influence on those from DCT. DCT was more beneficial to the measurements of IOP in normal eyes and those subject to LASIK surgery.

Tri-focal tensor-based multiple video synchronization with subframe optimization.

In this paper, we present a novel method for synchronizing multiple (more than two) uncalibrated video sequences recording the same event by free-moving full-perspective cameras. Unlike previous synchronization methods, our method takes advantage of tri-view geometry constraints instead of the commonly used two-view one for their better performance in measuring geometric alignment when video frames are synchronized. In particular, the tri-ocular geometric constraint of point/line features, which is evaluated by tri-focal transfer, is enforced when building the timeline maps for sequences to be synchronized. A hierarchical approach is used to reduce the computational complexity. To achieve subframe synchronization accuracy, the Levenberg-Marquardt method-based optimization is performed. The experimental results on several synthetic and real video datasets demonstrate the effectiveness and robustness of our method over previous methods in synchronizing full-perspective videos.

Protection by puerarin against MPP+-induced neurotoxicity in PC12 cells mediated by inhibiting mitochondrial dysfunction and caspase-3-like activation.

Puerarin, a main isoflavone glycoside distributed in Pueraria lobata (Willd.) Ohwi, showed inhibitory activity on H2O2-induced PC12 cells damage in our previous work. However, there is insufficient evidence in protective mechanism of puerarin, especially that relating to the mitochondrial function. In this study, when cells were pretreated with puerarin prior to 0.4 mM MPP+, protective roles were accompanied by a reduction of cell viability loss, morphological changes of apoptosis and apoptotic rate. To explore the protective mechanism of puerarin in MPP+-induced PC12 cells, mitochondrial function and caspase-3-like activity were measured. The results indicated that puerarin inhibited the release of mitochondrial cytochrome c to cytosol and the loss of mitochondrial membrane potentials. In addition, puerarin also reduced MPP+-induced caspase-3-like activation. Taken together, the above results suggest that pretreatment of PC12 cells with puerarin could block MPP+-mediated apoptosis by mitochondria-dependent caspase cascade.

[The heredity of flower colors and the discovery of flower color chimera in chrysanthemum species].

The reciprocal crosses of yellow colored chrysanthemum x red colored chrysanthemum and white colored chrysanthemum x red colored chrysanthemum were conducted in order to analyze the heredity of flower colors. The results revealed that the heredity of flower colors was very complicated, and mainly exhibited matroclinous characteristics when red colored materials was used as maternal parent but not in the combinations when the yellow or white colored materials were used as maternal parents. The incomplete dominance and mosaic dominance also existed in the heredity of chrysanthemum flower colors. The flower-color chimeras with two kinds of flower buds were discovered in the cross of 3501 x 3509, i.e. one side of the flower buds was completely in red color, which was same as the parental material of 3509, and another side was generally in yellow color with red spots on them. Cytological analysis showed that two sides were both with 36 chromosomes, indicating that the formation of chimera was not resulted from the changes of chromosome numbers, but from the destruction of pigment synthesis genes by the insert of transposable element.
