Permanent cardiac pacing through the right supraclavicular subclavian vein approach.

BACKGROUND: The infraclavicular subclavian route is commonly used for insertion of permanent pacing leads. However, the subclavian vein route may at times be a very difficult way to gain access to the heart. OBJECTIVE: To explore a new route to reliably and safely insert pacing leads. METHODS: The right supraclavicular subclavian vein route was selected to implant permanent leads in patients with critical illness or in situations where access through the infraclavicular approach was difficult. Access was achieved by Yoffa's venipuncture technique. A subcutaneous arc tunnel was made to pull the leads over the clavicle, which first arched close to the sternoclavicular joint and then curved to the inlet of the leads. The pacemaker was implanted in a right infraclavicular surgical pocket. RESULTS: This technique was used in 44 patients. The venipuncture time of 4.4+/-1.2 min was faster with the supraclavicular route than with the infraclavicular route. However, there was slightly more blood loss with the supraclavicular route. Total duration of implantation was similar for both routes (supraclavicular route 72+/-16 min and infraclavicular route 75+/-20 min). Lead dislodgement, lead fracture and skin erosion did not occur. CONCLUSION: Pacing through the right supraclavicular subclavian vein route is a safe and reliable alternative in cases where access through the infraclavicular route is difficult.

[Study on the dynamic changing profiles of telomeric restricted fragment length among sex balanced different age groups].

OBJECTIVE: In human, both in vivo and in vitro, telomere shortening appears to be a major component of cell senescence and aging. The detailed telomere shortening status and mechanism in peripheral blood cell is needed to be further characterized. METHODS: One hundred and twenty three peripheral blood samples were collected from healthy individuals of different age groups and the mean telomeric restricted fragment (TRF) was measured using Southern Blotting with Dig labeled probe. The samples of different groups were homogenized in sex components as indicated by chi 2 test of sex ratio of different test groups (P > 0.05). RESULTS: The average length of TRF is shortening with aging and distinguished shortening dynamic profiles could be observed. Further analysis showed that there might be a shortening peak near the age of 5. CONCLUSION: There are distinguished dynamics profiles of telomere shortening among different age groups. Thus, the results indicate that it might be possible to infer individual age by telomeric restricted fragment length assay.

[Genetic studies of 15 STR loci in Guangxi Miao population].

OBJECTIVE: The study was carried out to investigate genetic polymorphism of 15 STR loci in Guangxi Miao population. METHORDS: DNA samples from southern China 274 Miao unrelated individuals were screened by using AmpFlSTR Identifiler PCR Amplification Kit and 3100 Genetic Analyzer. RESULTS: These 15 loci meet the Hardy-Weinberg expectations. The matching probability of the 15 STR loci was 5.04 x 10(-17), and combined paternity of exclution was 0.9999993 in Guangxi Miao population. CONCLUSION: Our results showed Identifiler PCR Amplification systems of 15 STR loci were useful enough to forensic case work in Guangxi Miao population.

[Characteristics of forms of fluorine in soils and influential factors].

Forms of fluorine in 15 different soils were studied using sequential chemical extraction procedure. The fluoride forms in soils were separated into five fractions: water soluble fluoride (Ws-F), exchangeable fluoride (Ex-F), fluoride bound to Fe/Mn oxides (Fe/Mn-F), bound to organic matter (Or-F), and residual fluoride (Residual-F). The contents of different fluoride forms were decreased in the order: Residual-F >> Ex-F > Ws-F > Or-F > AoFe-F. Correlation analyses showed that Ws-F was significantly correlated with Ex-F (r = 0.775* *). Ex-F was correlated with Fe/Mn-F (r = 0.5753*) and Organic-F (r = 0.5529*). Stepwise linear regression analysis was used to test the effect of various soil components on the contribution of fluoride forms, indicating that soil pH, organic matter, clay content, and exchangeable calcium content were the important influential factors and pH was the most important influential factor. In addition, soil parent materials had also influence on forms of fluorine.

Induction of drug resistance and protein kinase C genes in A2780 ovarian cancer cells after incubation with antineoplastic agents at sublethal concentrations.

We examined the inducibility of drug resistance (MDR1, MRP1, LRP) and protein kinase C (PKC) isozyme (alpha, epsilon, eta, theta, tau, zeta) corresponding genes in A2780 ovarian cancer cells after a 24-hour treatment with adriamycin (ADR), camptothecin (CAM), etoposide (ETO) or vincristine (VCR). Sublethal concentrations of drugs were used to exclude short-term effects caused by selection. Cell cycle analysis was performed to identify possible correlation between resistance factors, PKC isozymes and proliferation. We found a mostly combined induction of MDR1, LRP, PKC tau and PKC zeta by CAM, ETO and VCR. PKC alpha, epsilon, eta and theta gene expression altered variably. Cell cycle analysis showed that A2780 cells responded with a marked G2/M arrest after a 24-hour treatment with CAM, ETO and VCR but an association between the induction of PKC isozymes corresponding genes and proliferation was not seen. Our analysis points to a possible link between atypical PKC tau/PKC zeta and MDR1/LRP in cytostatic stress response of cancer cells.

Improved assessment of functional resectability in repeated hepatectomy by computer-assisted operation planning.

Assessment of the post-resectional functioning liver parenchyma is one of the most important issues in operation planning of liver resections. However, preoperative assessment of the future liver remnant's vascularization is in particular difficult in repeated hepatectomy because of the often altered intrahepatic vascular system after prior vascular dissection. Therefore, in these resections preoperative calculation of the remaining liver parenchyma with unimpaired vascular function is uncertain and imprecise. Based on the case of a 62-year-old woman with a recurrent colorectal liver metastasis in segments VII and VIII after prior left-lateral liver resection (segments II/III) we describe the potential use of computer-assisted operation planning for assessment of functional resectability in repeated hepatectomy.
