Influence of anode-filter combinations on image quality and radiation dose in 965 women undergoing mammography.

PURPOSE: To evaluate how anode-filter combinations influence image quality in and mean glandular dose to breasts of different thicknesses and compositions. MATERIALS AND METHODS: Mammograms were obtained with a molybdenum (Mo) anode and a Mo filter at 26 kVp, a Mo anode and a rhodium (Rh) filter at 27 kVp, or a tungsten (W) anode and a Rh filter at 26 kVp in 965 women. One anode-filter-tube voltage combination was used in the right breast and another in the left. The mean glandular dose to each breast was calculated. RESULTS: Image contrast was highest in the Mo-Mo mammograms. However, depiction of the glandular tissue, pectoral muscle, and skin and subcutis was significantly (P < .001) better with the Mo-Rh and the W-Rh than with the Mo-Mo combination. The average mean absorbed doses to the glandular tissue for W-Rh and Mo-Rh were 50% and 75%, respectively, of that for Mo-Mo. CONCLUSION: Breast thickness is the most important parameter in selection of an anode-filter-tube voltage combination. Compared with Mo-Mo, both Mo-Rh and W-Rh gave good image quality of the mammary gland and a considerably lower absorbed dose. Mo-Rh-27 kVp is recommended for breast thicknesses of 60 mm or less; W-Rh-26 kVp, for breast thicknesses of greater than 60 mm.

Nortriptyline and pituitary-thyroid function in affective disorder.

Twenty out-patients with depressive illness were studied with regard to thyroid hormone concentrations, basal and TRH-stimulated TSH, and prolactin levels before and during treatment with nortriptyline. Concentrations of plasma nortriptyline and 10-hydroxy-nortripyline were analysed while on 75 mg and 150 mg/day. No correlations between clinical outcome and side effects on the one hand and drug concentrations on the other were demonstrated. Provided the age factor was considered, basal and stimulated hormone levels did not differentiate between healthy controls and patients with "endogenous" and those with "exogenous" depression. Nortriptyline treatment did not influence serum hormone levels.

Formation of GABA via oxidative deamination of putrescine in ovaries of immature rats after gonadotrophin stimulation.

The diamine oxidase catalyzed metabolism of 14C-putrescine was studied in the ovaries of immature rats with and without stimulation by gonadotrophin. This stimulation resulted in a statistically significant increase in the deamination rate of putrescine with a very predominant production of gamma-aminobutyric acid. No formation of polyamines was observed which indicated an importance of putrescine or of products of its oxidative deamination during this cellular activity.

Combined radionuclide circulation and static imaging of the brain. Space-occupying lesions.

Combined static and dynamic imaging of the brain was performed in 398 patients with suspected space occupying lesions. The imaging findings were analyzed from two points of view: Firstly, the radionuclide results were compared with pathological anatomical findings and, secondly, the diagnostic yield in patients with various symptoms was evaluated. Thus we obtained the number of false positive and false negative results. The findings of the combined imaging procedure were correctly negative in 85% of the patients with non-focal symptoms and false positive in only 1%. The circulation study alone gave false positive results in 7% and so did the static study alone. The combined imaging procedure was pathologic in 46 of 83 cases of space-occupying lesions (55%). There were only three patients in whom a pathologic circulation study was the only indication of a space-occupying lesion (4%). The static study alone gave correct positive findings in 23 more cases (28%). We conclude that a pathologic blood flow study alone is of limited interest. In the presence of a pathologic static study the circulation procedure gives additional information, especially in meningioma and subdural hematoma. In patients with symptoms indicative of supratentorial space-occupying lesions of the brain combined imaging is recommended.

Peripheral neuropathy in monoclonal gammapathy with cryoglobulinemia and arteritis.

We report a patient with IgM gammapathy, cryoglobulinemia, Raynaud's phenomena, purpura hyperglobulinemica of the legs and polyneuropathy. Endoneural vasculitis with infiltrations of eosinophilic and neutrophilic granulocytes and an extensive loss or Wallerian degeneration of myelinated nerve fibers were seen on histopathologic examination of a sural nerve biopsy specimen. The microscopic picture differed somewhat from that observed previously in cryoglobulinemic vasculitis. Although vasculitis is most often believed to represent an immunologically mediated lesion, we propose an alternate explanation, namely, that the disease manifestations in the present case were secondary to cold-induced effects of the cryoglobulin on the microcirculation.

Hepatic and pancreatic dysfunction in anorexia nervosa: a report of two cases.

Two patients with anorexia nervosa with evidence of pancreas and liver injury are reported. Both showed evidence of organ dysfunction in the presence of cachexia. Serum amylase, ASAT (GOT), and ALAT (GPT) became normal parallel with body weight. On the basis of reports on the effects of malnutrition on pancreas and liver function, it is suggested that nutritional pancreas and liver dysfunction might represent hitherto unrecognized complications of anorexia nervosa.

Light chain myeloma with features of the adult Fanconi syndrome: six years remission following one course of melphalan.

A 41-year-old woman, presenting with renal failure, renal glucosuria and moderate anemia, was found to have light chain myeloma, indicated by a kappa chain M-component in the serum, heavy urinary excretion of kappa chains and plasma cell infiltration of the bone marrow. After administration of one course of melphalan, resulting in transient pancytopenia, the light chains disappeared completely, renal function returned to normal, glucosuria disappeared and the Hb concentration normalized. During an observation period of six years she has remained in good health and there has been no sign of relapse.

Nephrotic syndrome due to subacute glomerulonephritis -- association with hydrocarbon exposure?

A 59-year-old man developed a nephrotic syndrome 40 days after hydrocarbon exposure of 3 days' duration. Renal biopsy gave evidence of a subacute proliferative glomerulonephritis. A lasting remission was obtained with immunosuppressive therapy. The case history is discussed in the light of recent studies indicating a causal relationship between hydrocarbon exposure and glomerulonephritis. The reversible nature of the disease in the present case is discussed in relation to the disease in experimental animals induced by a single administration of heterologus antigen.

Uptake of 99mTc-tripolyphosphate in thrombotic leg.

Thirtyeight patients with suspected thrombosis in the legs were investigated scintigraphically using 99mTc-tripolyphosphate. The ratio between the activity in the leg with suspected thrombosis and the other leg was calculated. The results were compared with those obtained with phlebography. A pathologically high activity ratio was found in all but one of the 18 patients in whom phlebography showed a thrombus. The ratio was normal in 13 of the 20 patients with normal phlebographic findings. The possible use of the method described as a postoperative screening test is discussed.
