MLR-specific suppressor T lymphocytes in man. II. Functional and membrane characteristics of a specific MLR suppressor subpopulation.

Human MLR-specific suppressor T lymphocytes were induced by in vitro cultivation of human peripheral T lymphocytes in the presence of soluble HLA-DR alloantigens isolated from normal serum. The suppression is specific in that responder cells autologous to the suppressor cells respond to allogeneic stimulating cells that express the same HLA-DR specificity as that recovered from serum and used to induce the suppressor cells. This antigen-specific suppressor T cell population could be divided into suppressor and non-suppressor subpopulations as a function of adherence to a 6MB Sepharose immunoadsorbent coated with the inducing HLA-DR soluble antigen. As a consequence of activation by soluble DR antigen, the suppressor T lymphocyte population as well as the column-enriched suppressor subpopulation express new membrane specificities that can be recognized by antisera from pluriparous women. The specificities that are recognized are not found on autologous, unstimulated B and T cells, nor do they appear to recognize conventional HLA-A,B,C or DR determinants.

Sodium cromoglycate and provocation tests in chronic urticaria.

Twenty patients with chronic urticaria exacerbated by aspirin or tartrazine were challenged with the offending substance after pre-treatment with either oral sodium cromoglycate or placebo in a double-blind, crossover study. The sequential vascular response test (SVR) described by Fisherman and Cohen 1-4 was used to assess response to challenge. Eighteen patients were protected by oral sodium cromoglycate and one by placebo, taking as a positive response an increase in the bleeding time of 100% or more over control values.

Correlation of the specific IgE in serum and nasal secretions with clinical symptoms in atopics.

In a unselected population of 133 young adults studied by prick testing to common allergens three groups were identified: eleven subjects with positive skin test responses and clinical symptoms of allergy; ten subjects only with psitive skin tests and the remainder with negative skin tests. All subjects with positive skin tests (with an without symptoms) were studied by RAST on the serum and nasal secretions. Specific and non-specific bronchial provocation tests (BPT) were also carried out. The serum RAST was positive in all subjects with positive skin tests, and there was good correlation between high levels of circulating specific IgE and the presence of clinical symptoms. The RAST of nasal secretions was negative in most symptom-free subjects and as a diagnostic test it was slightly better than the serum RAST. BPTs with extracts of the relevant allergens caused bronchospasm in every subject with a positive nasal secretion RAST. Only two subjects out of fifteen with a positive response were clinical asthmatics. Our results cast doubt on the clinical relevance of the BPT as it is usually conducted.

[25 years of experience in the treatment of Raynaud's disease and Raynaud-like syndromes].

Reference is made to a series of 620 cases of Raynaud's phenomenon observed at the University of Milan between 1951 and 1977. The following parameters are analysed: sex, age, aetiology (where known), incidence of involvement of the four limbs, presence or otherwise of a complete sign, percentage of trophic lesions, arteriographic picture, lab examinations performed. 534 thoracic sympathectomies were performed in 464 patients with Raynaud or Raynaud-like forms. The results are described.

Immune-complexes in malignant skin melanoma: quantitation by three methods, purification, and characterization of their non-tumor related constituents.

Circulating immune-complexes (ICs) and complement system (C) were studied in sera from 10 patients with malignant skin melanoma. The presence of ICs was demonstrated in all of them by three distinct methods. Moreover, ICs were purified in all the patient sera. The fact that ICs were detected in each serum and that the results of the analysis obtained by the different methods were not overlapping is discussed. It is stressed that the differences among the intrinsic characteristic of each technique are responsible for these discrepancies. The C profile was altered if compared with normal values, but it was not strictly characteristic for an ICs disease. In the purified ICs C3, C4, IgM, IgC1, and IgG3 were demonstrated; C1q, C1s, IgA, IgG2, and IgG4 never. It is noteworthy that a rheumatoid activity was found in the purified material and that this activity was significantly correlated with serum immuno-conglutinins (IKs) titre.

Development of IgE response in humans after tetanus toxoid immunization.

In 15 subjects immunized with two doses of purified adjuvant-free tetanus toxoid, the behaviour of specific IgE and hemoagglutinative antibodies was studied. In the majority of the subject the presence of specific IgE and hemoagglutinative antibodies was observed upon a control carried out 15 days after immunization. After reintroduction of antigen an increase both of specific IgE and hemoagglutinative antibodies was observed. No subject who had presented specific IgE in the serum after the first inoculation of tetanus toxoid showed local or general reaction after reintroduction.

Increase of T.G lymphocytes in human one-way mixed lymphocyte culture.

Human one-way mixed lymphocyte culture induces a significant increase of EA(7S) rosette-forming cells. Using fractionation procedures, an increased number of Fc receptor-bearing cells in the T high-enriched populations was found from alloactivated lymphocytes compared with similar fractions obtained after autologous control cultures. Additional experiments showed a parallel increase of E-rosette-forming cells in the Fc receptor-enriched alloactivated fractions. The results indicate that an increase of T.G lymphocytes occurs during in vitro alloactivation in man.

Increase in the percentage of sIgD-bearing lymphocytes during human mixed lymphocyte reaction.

In human one-way mixed lymphocyte reactions we have observed an increase in the percentage of sIgD bearing lymphocytes, detected by direct membrane immunofluorescent staining. This increase does not seem to be attributable either to the synthesis of sIgD receptor molecules by previously negative T or null cells or to a non specific stimulation of B cells by a factor released in the course of the MLR.

IgE synthesis in aging. Enhancement of IgE production in subjects with autoantibodies.

Serum IgE levels were measured in two groups of healthy elderly subjects: a first group of 18 with circulating autoantibodies (sign of a probable deficit of suppressor T lymphocytes) and a second group of 15 without circulating autoantibodies. The IgE levels in the first group (average 1032 U/ml) resulted significantly higher than those in the second group (average 103 U/ml), P less than 0.05. This result can be related to a deficit of suppressor T lymphocytes functionality, present in these subjects, which would cause, besides the appearance of autoantibodies, also an increase of serum IgE levels. Moreover, this phenomenon seems to confirm that in man, too, as in experimental animals, IgE synthesis is under the control of suppressor T lymphocytes specific for IgE. From a clinical viewpoint, the two populations studied do not differentiate and no symptoms of atopy were observed in subjects with high IgE levels.

A long-term follow-up study in essential cryoglobulinemia.

In a case series of 56 patients with essential cryoglobulinemia, 35 were followed-up for 4-13 years (mean 7 years). A membranous proliferative glomerulonephritis, which in about half the cases showed a progression to renal insufficiency, was the commonest complication, observed in more than one third of the patients. In 2 patients hepatic cirrhosis became manifest after a completely asymptomatic period and in 2 others a lymphoproliferative disease appeared 2 and 8 years after the onset of purpura. In 51% of patients the intial clinical pattern did not change. In searching for a correlation between the development of nephropathy and cryoglobulin characteristics, none was demonstrated studying the cryoglobulin level, the presence of autoantibody and the complement components.

Enhanced TSH stimulating effect of TRH by sulpiride in man.

Sulpiride, a specific dopaminergic blocker, was administered im to 6 normal male volunteers (100 mg) alone or in association with L-DOPA (500 mg per os, 90 min before sulpiride) or with TRH (400 micrograms iv in bolus, 30 min after sulpiride) and blood samples were obtained for TSH radioimmunoassay at various intervals before and after the treatments. Sulpiride alone produced a slight increase of plasma TSH levels which was inhibited by L-DOPA pre-treatment. The previous administration of sulpiride resulted in a marked increase of the TSH response to TRH. It is suggested that dopamine and TRH may interact to modulate pituitary secretion of both TSH and prolactin, though the hypophyseal thyrotrophs and lactotrophs show a different sensitivity to the common stimulatory and inhibitory substances.

The HLA system and multiple sclerosis.

We referred the data on association between M.S. patients and HLA complex in the different populations. Then we discussed the different aetiopathogenetic hypothesis that centre on the disease. We considered also viral and autoimmunity theory and the possibility that these two hypothesis don't wrangle but complete them.

Effect of pinealectomy on arterial blood pressure and food and water intake in the rat.

Pinealectomy in the rat induces a significant increase of the arterial blood pressure within 15 days from the surgical procedure; this hypertension is still present 30 and 60 days after pinealectomy while after 90 days it returns to the normal range. Histological examination of the kidneys of pinealectomized hypertensive rats shows wall thickening and lumen narrowing of the arterioles, adventitial and periadventitial fibrosis, dense glomeruli. The vascular lesions are not diffuse but patchily distributed. Body weight is higher in pinealectomized rats compared with normal and sham-operated animals: this difference becomes significant about 40 days after pinealectomy and gradually decreases until it disappears at the end of the third month. Food and water intake is higher in pinealectomized rats up to the 7th week; during the 8th week they return to the normal range.

Occurrence of circulating immune complexes in beta-thalassaemia major.

The presence of circulating soluble immune complexes and the level of complement were investigated in sera from 21 patients with beta-thalassaemia major, including both splenectomised and nonsplenectomised patients. A high level of immune complexes was found in half of these cases. Reduced complement levels were seen less frequently. There was no correlation between the presence of circulating immune complexes, decreased complement levels, and thpresence or absence of the spleen. The level of immune complexes increased with the age ofthe individual, i.e. with the duration of the disease.

HLA typing and affective disorders: a study in the Italian population.

HLA phenotype distribution was investigated in 91 affective patients. Significant increases over those of the control population were found in HLA-A 29 and in Bw 22 frequencies, while A 10 and A 30 were decreased. No significant difference was shown between the two clinical subgroups (41 unipolar patients and 50 bipolar ones). On comparing our data with those from other authors, Bw 16 was significantly increased. However, a high degree of heterogeneity was also shown for this antigen. Of some interest is the finding that relapsed and non-relapsed patients during long-term lithium therapy display diverging HLA phenotype distributions, with B 5 increased among the non-relapsed subjects.
