Lupus nephritis in children--a review of 167 patients.

BACKGROUND: Relatively few studies have been made of children with lupus nephritis. The prognosis of children with lupus nephritis is ominous for those with diffuse proliferative glomerulonephritis and active interstitial inflammation. Up to now few studies have been made on this subject. OBJECTIVES: To evaluate the clinical course, histopathology, and prognosis of lupus nephritis in children, to identify the risk factors for renal failure and mortality, and to share our experience in treating lupus nephritis in children. METHODS: Retrospectively, 167 children under 18 years of age with lupus nephritis at Veterans General Hospital-Taipei, Taiwan from 1979 to 1991 were studied. All patients received renal biopsy and follow-up biopsies were performed in 36 children. The clinical and serologic parameters at the time of renal biopsy were recorded. RESULTS: There were 55 (33%) patients with class II, 30 (18%) with class III, 69 (41.3%) with class IV, and 13 (7.8%) with class V nephritis based on initial biopsy. The mean follow-up time was 59 months. Follow-up biopsies were histologically stationary in 29 patients, progressive in five, and regressive in two. The results revealed that those with persistent hypertension, anemia, increased serum creatinine concentration, and decreased creatinine clearance rate at initial biopsy were more prone to develop renal failure. Low titer of CH50 hemolytic assay appeared to be a poor prognostic indicator. The overall renal and patient 5-year survival rates were 93.1% (135/145) and 91.08% (143/157), respectively. They were 87.7% (50/57) and 82% (55/67), respectively, of patients with class IV proliferative glomerulonephritis. CONCLUSIONS: The prognosis of children with class IV nephritis in this study was better than that reported previously. All children surviving without renal failure were maintaining their normal lives with little organ dysfunction. The improved results may be due to earlier renal biopsy for precise histopathologic definition, better supportive care, and selective use of aggressive therapy, including methylprednisolone pulse therapy, intravenous cyclophosphamide, intravenous prostaglandin E1 therapy, high-dose intravenous gammaglobulin therapy, and cyclosporin A for those with high risk factors.

[Improvement of memory function of fornix-fimbria transected rats by transplantation of the superior cervical ganglion into hippocampus].

Memory impairments of passive avoidance response were observed in 38 Wistar rats with bilateral fornix-fimbria transection. After fornix-fimbria lesions the degree of performance decreased from 65.3% to 13.6% (P < 0.01). Autotransplantation of superior cervical ganglion (SCG) into bilateral dorsal hippocampi improved memory function to a considerable extent. In the end of the behavioral experiments, implanted rats were sacrificed for histofluorescence study of grafts and measurement of norepinephrine (NA) content in the hippocampus. These experiments showed that the hippocampal NA content in implanted rats was considerably higher than that in untransplanted fornix-fimbria transected rats and consequently suggested that improvement of memory function was to some extent due to supplement of monoamine transmitter by the transplanted SCG.

Plant low-molecular-mass heat-shock proteins: their relationship to the acquisition of thermotolerance in plants.

Among the heat-shock proteins (HSPs) induced in response to heat-shock (HS) in all organisms, the low-molecular-mass (LMM) HSPs are the most abundant and unique in plants. The HSP genes at the transcriptional and the translational levels have been well-characterized; however, relatively little is known about the physiological function of HSPs. We have been studying the biological role of plant LMM HSPs based on the suggestion by Minton, Karmin, Hahn and Minton ([1982] Proc. Natl. Acad. Aci. U.S.A. 79, 7107-711) that HSPs may act as heat-stable proteins to non-specifically stabilize other proteins which are highly susceptible to inactivation or denaturation by heat. For the future, LMM-HSP gene transfer into organisms that do not synthesize plant LMM HSPs would be a way of determining the function of LMM HSPs, and success could be of practical importance.

Early detection of the tip position and complication of the intra-aortic balloon pump catheter by perioperative transesophageal echocardiography.

BACKGROUND: Intraoperative verification of the precise balloon position of intra-aortic balloon pump(IABP) is necessary and important. We introduced transesophageal echocardiogram (TEE) as one of our routine intraoperative image monitor to accomplish this identification fluently, conveniently and also precisely. METHODS: 56 patients who needed the hemodynamic support when weaning the cardiopulmonary bypass were introduced to receive IABP and the balloon position was verified by TEE. To confirm the accurate location, the position of balloon tip was rechecked with postoperative portable chest x-ray. RESULTS: The balloon tip was attempted to locate in the proximal descending aorta, which was verified via TEE to be successful in 98.1% of patients by postoperative chest x-ray. Five (8.9%) patients failed to pass the balloon catheter at the first time due to high arterial resistance. One iatrogenic aortic dissection (1.8%) and one case in the wrong lumen (1.8%) were detected by TEE. No early death was contributed to IABP insertion. CONCLUSIONS: The application of TEE in the perioperative period helps not only detect IABP complication early but also verify of balloon catheter position quickly. It can replace the conventional role of fluorography.

Binding and localization of M(r) 72,000 matrix metalloproteinase at cell surface invadopodia.

Degradation (turnover) of collagenous matrix occurs on the surface of specialized membrane extensions termed "invadopodia," which are sites of cell invasion into the extracellular matrix. Here we show the localization of the M(r) 72,000 type IV collagenase of the matrix metalloproteinase family at invadopodia. When added exogenously, latent M(r) 72,000 collagenase binds to invadopodia of chicken embryo fibroblasts transformed by Rous sarcoma virus, whereupon the bound collagenase loses its propeptide. The collagenase binds to a component contained within the detergent extract of transformed cells, and increased levels of the active M(r) 62,000 form of the collagenase are seen here. Such an association is not detected in the detergent extract derived from normal cells. Using a recently developed cell fractionation procedure to collect cell surfaces enriched in invadopodia, we show that the M(r) 72,000 collagenase associates with the invadopodial fraction and active forms of the enzyme become immobilized on the collagenous surface. Thus, invadopodia direct intense localized degradation of the extracellular matrix by concentrating active membrane-associated collagenases at sites of cellular invasion.

Detection of beta-thalassemia carrier by direct analysis of beta-globin gene lesions.

DNA was prepared from peripheral blood mononuclear cells of 114 Chinese with low erythrocyte mean corpuscular volume and analyzed by allele-specific DNA amplification for the presence of mutant alleles in the beta-globin gene that account for about 90% of beta-thalassemia in Chinese. A total of 9 mutations of the five most frequent mutant alleles were detected in 8 individuals. All mutant sequences were confirmed later by DNA sequencing. However, no mutation of these mutant alleles was detected in the remaining 106 individuals with low erythrocyte mean corpuscular volume including 22 who also had Hb A2 content of 6.0% or more. Our results seem to suggest that the presence of beta-thalassemia allele does not correlate very well with red blood cell indices and that direct DNA analysis by allele-specific DNA amplification is an accurate method to identify beta-thalassemia heterozygotes.

Herpes simplex virus type 1 genes in human mononuclear cells and affecting cell-mediated immunity.

To evaluate whether herpes simplex virus type 1 (HSV-1) replicates in freshly isolated human mononuclear cells (HMNC) and results in cell-mediated immunity, HMNC purified from the peripheral blood of a healthy HSV-1-seronegative individual were infected with HSV-1 (KOS strain). The kinetic data indicated that HSV-1 particles were detected in HMNC at 9 hr postinfection, and the highest HSV-1 titer was obtained at 24 hr postinfection. It was found that the newly synthesized HSV-1 DNA increased in HMNC corresponding to the duration of time postinfection. The HSV-1 tk mRNA was detected in HSV-1-infected HMNC by Northern blot analysis. The Western blot analysis indicated that the HSV-1-infected HMNC could express the viral proteins. These results suggest that HSV-1 replicates in freshly isolated HMNC and expresses its DNA, RNA, and proteins. Interleukin-2 (IL-2) production and cell-mediated cytotoxicity (CMC) were also determined in HSV-1-infected HMNC but were lower than those found in uninfected HMNC. On the other hand, HSV-1 DNA was detectable in HMNC of patients suffering from recurrent HSV-1 infection. The results show that, in the recrudescent phase, the patients' HMNC contain more HSV-1 DNA than in the convalescent phase. We conclude that the impairments of immune responses are related to the infection of HMNC by HSV-1.

Cytokines predict coronary aneurysm formation in Kawasaki disease patients.

In this study, we measured serially the serum levels of cytokines including interleukin-6 (IL-6), IL-8, soluble IL-2 receptor (sIL-2R) and tumour necrosis factor alpha (TNF-alpha) in 60 patients with Kawasaki disease (KD) and evaluated the clinical significance of these cytokines in predicting coronary aneurysm formation. Of the 60 patients, 12 were complicated with coronary aneurysm. Blood samples were collected within the 1st week after onset of fever, then once a week for the 1st month, and once a month for another 5 months. The serum levels of IL-6, IL-8, sIL-2R and TNF alpha were measured using an ELISA or RIA method. Our results show that the changes in serum IL-6 and IL-8 were faster than those of sIL-2R and TNF alpha. Within the 1st week, the serum levels of IL-6 and IL-8 were significantly higher in the patients with than in those without coronary aneurysm (P < 0.001). In addition, the serum levels of IL-6 and IL-8 obtained in the 1st week were highly correlated (P < 0.001) with those of C-reactive protein and erythrocyte sedimentation rate, and the serum levels of sIL-2R and TNF alpha were also increased at the 1st week reaching the highest level in the 2nd week. In the 2nd week, the serum levels of sIL-2R and TNF alpha were significantly higher in the patients with than in those without coronary aneurysm (P < 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Identification and characterization of epitopes on Cyn d I, the major allergen of Bermuda grass pollen.

BACKGROUND: We identified three epitopes on Cyn d I by using four anti-Cyn d I monoclonal antibodies (MoAbs). METHODS: In a cross-inhibition binding assay, the binding of MoAbs 1-61 and 10-7 to Cyn d I was completely blocked by each other but not by MoAbs 4-37 and 11-7; the binding of MoAb 4-37 and MoAb 11-7 to Cyn d I was inhibited by themselves but not by other MoAbs. The epitope recognized by MoAbs 1-61 and 10-7 is designated as E1, and those recognized by MoAbs 4-37 and 11-7 are designated as E2 and E3, respectively. RESULTS: In a radioallergosorbent inhibition assay, we found that MoAbs 1-61 and 4-37 (1:50 diluted) can inhibit the binding of human Immunoglobulin Es to Cyn d I by more than 30%, whereas MoAb 11-7 was less efficient (reduced by only 6%). These results suggest that both E1 and E2 are major allergenic epitopes but that E3 is only a minor one. Further characterization of E1 and E2 reveals that they are labile in alkaline but resistant to acid and sodium periodate treatments. Moreover, E1 is heat-labile, but guanidine- and urea-sensitive, whereas E2 is not. Both E1 and E2 lost their antigenicity after reduction and alkylation. CONCLUSIONS: Results of the present study provide important information on the physicochemical properties of major allergenic epitopes on Cyn d I, which may be useful for future development of therapeutic peptides for patients allergic to Bermuda grass pollen.

Fatal gas embolism as a complication of Nd-YAG laser surgery during treatment of bilateral choanal stenosis.

A 40-day-old infant boy underwent Nd-YAG laser surgery because of congenital bilateral choanal stenosis. Cyanosis and cardiovascular collapse occurred during the operation. Resuscitation was initiated, but in vain; the patient died. The evolution of clinical events was consistent with a diagnosis of gas embolism. In the investigation of causes, the use of a sapphire tip with the Nd-YAG laser and the cooling of the tip with N2 gas were thought to have contributed to the fatal outcome. The authors warn of the potential risk of gas embolism with the Nd-YAG laser and a coaxial gas cooling system, and they emphasize the importance of monitoring for gas embolism in high-risk patients.

Plasma levels of atrial natriuretic factor in moderate to severe obstructive sleep apnea syndrome.

To evaluate atrial natriuretic factor (ANF) secretion during sleep in obstructive sleep apnea syndrome (OSAS), plasma ANF was measured every 3 hours before and after effective nasal continuous positive airway pressure (CPAP) treatment in 10 patients with moderate to severe OSAS and 10 normal subjects. The results showed daily changes in ANF levels in normal controls and in OSAS patients after effective therapy, with a nadir at 0300 hours and a peak at 2100 hours. There was no significant daily variation of ANF levels in patients with OSAS before therapy, and ANF levels from midnight to 0900 hours were significantly higher before, as compared with after, therapy. These results indicate that OSAS patients have abnormal ANF secretion. Effective nasal CPAP therapy led to normalization of ANF secretion during sleep.

Evaluation of gastric emptying in severe, burn-injured patients.

OBJECTIVE: The aim of this study was to evaluate the possible effect of a severe burn on gastric emptying by determining the absorption kinetics of orally administered acetaminophen. DESIGN: A prospective, controlled study. SETTING: A ten-bed burn center in a 1,300-bed university hospital. PATIENTS: Ten adult patients suffering from second-degree burn involving > 20% of total body surface area and 20 normal, healthy volunteers who acted as controls. INTERVENTIONS: Patients received routine treatment such as nutritional support and cimetidine. However, opiates were stopped for at least 12 hrs before the start of the study, and nonsteroidal anti-inflammatory drugs as alternatives were used. After an 8-hr fast, the subjects ingested 0.5 g acetaminophen with 200 mL of water. The plasma concentrations of acetaminophen were determined by high-performance liquid chromatography, and the absorption kinetics was estimated from determination of time to reach the maximum plasma concentration, the maximum plasma concentration, and the area under the plasma concentration-time curve. MEASUREMENTS AND MAIN RESULTS: The mean time for reaching the maximum plasma concentration was 33 +/- 24 (SD) mins in patients and 39 +/- 24 mins in the healthy volunteers. The mean area under the plasma concentration-time curve from time 0 to 120 mins and the mean maximum plasma concentration were 556 +/- 190 micrograms/mL/min and 9.5 +/- 3.5 micrograms/mL in patients, and 539 +/- 131 micrograms/mL/min and 7.8 +/- 2.8 micrograms/mL in volunteers, respectively. There was no statistical difference between groups in the time to reach the maximum plasma concentration, the area under the plasma concentration-time curve from time 0 to 120 mins, and the maximum plasma concentration. The time for reaching the maximum plasma concentration was not correlated with the severity of the burn (% area of burn) and the duration of healing (days) after burn. CONCLUSIONS: We conclude that severe burn injury does not affect the kinetics of gastric emptying, and that 200 mL of water ingested 2 hrs before anesthesia is quite safe in severely burned patients. Also, the absorption kinetics of acetaminophen was not altered by burn injury.

A comparison between vecuronium and atracurium in myasthenia gravis.

We evaluated the effect of vecuronium bromide and atracurium besylate on the train-of-four response in the management of muscle relaxation in 20 patients with myasthenia gravis (MG) who were undergoing thymectomy. We confirmed the safe use of these two non-depolarizing muscle relaxants in MG patients. Vecuronium (0.04 mg.kg-1) demonstrated a lesser clinical duration than did atracurium (0.2 mg.kg-1) (38 +/- 19 vs 50 +/- 21 min, mean +/- s.e.mean). The recovery time for vecuronium patients was shorter than that for atracurium patients (22 +/- 18 vs 38 +/- 18 min), but the time until onset of neuromuscular blockade was longer with vecuronium (246 +/- 105 vs 107 +/- 103 s). During spontaneous recovery from neuromuscular relaxation, at T1/C of 25% and 100%, the train-of-four fade with vecuronium was significantly greater than that with atracurium (0.04 +/- 0.02, 0.16 +/- 0.03 vs 0.17 +/- 0.01, 0.83 +/- 0.03), suggesting that vecuronium had a greater prejunctional effect than did atracurium.

The use of transesophageal echocardiography to evaluate the effectiveness of patent ductus arteriosus ligation.

The ligation of patent ductus arteriosus (PDA) is a comparatively easy operation, but some complications are possible. The most common complication is incomplete ligation of the PDA; others include inadvertent ligation of the descending aorta or left pulmonary artery, transient rise in systemic blood pressure and increased left ventricular afterload, and acute right heart failure due to pulmonary hypertension. The completeness of the PDA ligation is usually determined only by the operating physician's experience, including the use of an esophageal stethoscope or a finger on the lesion to feel for vibration. These methods sometimes fail to detect an incomplete ligation. With transesophageal echocardiography (TEE), we have monitored the entire course of the PDA ligation directly without interrupting the surgical procedure, and precisely determined the completeness of the ligation. We also expect that TEE will enable us to avoid other complications as well.
