Experimental studies on the pathway for migration and the development of Taiwan Taenia in domestic pigs.

The pig is the most favorable experimental intermediate host of Taiwan Taenia. Cysticerci in infected pigs are located in the liver except for a few extrahepatic ones. The present study was designed to investigate the pathway of migration of the oncospheres of Taiwan Taenia in the pig. In the first group, each of 5 Small-Ear-Miniature (SEM) and one Landrace-Small-Ear-Miniature (L-SEM) pigs were injected with 5000 hatched oncospheres into the ear vein. Three SEM and one L-SEM pigs were found to harbor 88 degenerated or calcified cysticerci only in the liver 51-81 days after injection. In the second group, each of 3 L-SEM pigs were injected with 5000 hatched oncospheres into the jugular vein. One of the 3 pigs was found to have 5 cysticerci (2 mature and 3 degenerated or calcified) only in the liver at 89 days post-injection. In the third group, each of 4 SEM, three L-SEM, and 3 L-SEM pigs were injected with 10,000, 10,000, and 5000 hatched oncospheres, respectively, directly into the portal vein after surgical opening of the abdominal cavity. All 10 pigs were found to be infected with a total of 1088 cysticerci (44 mature and 1044 degenerated or calcified) only in the liver 23-62 days after injection. Although the sites of injection in these three groups were different, the liver was the only final location of the cysticerci. These findings give strong evidence that the oncospheres migrate to the liver through the venous circulation and develop in this organ.

The etiology of lung cancer in nonsmoking females in Harbin, China.

A case-control study of female lung adenocarcinoma in Harbin, China involving interviews with 120 cases of nonsmoking females, matched 1:1 with controls, showed that high coal use, indoor air pollution, exposure to coal dust, frequent frying and deep frying food, and a history of lung cancer in the family were statistically significant risk factors for female adenocarcinoma. High personal income, spacious living quarters, and frequent consumption of carrots were protective factors for adenocarcinoma.

Hyperglycemia during hypothermic cardiopulmonary bypass does not alter postbypass vascular endothelial responses in dogs.

BACKGROUND: Hyperglycemia during hypothermic cardiopulmonary bypass (CPB) may alter intrinsic vasomotion by reducing endothelial-dependent vasorelaxation. Using a canine model of hypothermic CPB, this study tested whether hyperglycemia altered the vasodilator response to acetylcholine (ACh) and the vasoconstrictor response to phenylephrine (Phe). METHODS: In 20 anesthetized dogs, the left femoral arteries were excised and placed in gassed (95% O2-5% CO2) cold Krebs's solution. The animals were randomized into two groups undergoing 120 minutes of 28 degrees C CPB using membrane oxygenators. A hyperglycemic group (n = 10) received a continuous infusion of 50% dextrose to maintain blood glucose level greater than 500 mg/dL; a normoglycemic group (n = 10) received 0.9% saline. After rewarming and discontinuing CPB, the right femoral arteries were excised. Vessel rings were placed in a suffusion bath, and changes in isometric tension were measured. Dose-response relationships (ACh: 10(-9) to 10(-6)M; Phe: 3 x 10(-8) to 10(-4)M) and -log ED50 sensitivity to ACh and Phe before and after CPB were compared. RESULTS: Serum glucose during hypothermic CPB was significantly greater in glucose-treated dogs (525 +/- 9 mg/dL) than controls (109 +/- 5 mg/dL; p < 0.05). After CPB, -log ED50 values for ACh changed from 7.7 +/- 0.1 to 7.5 +/- 0.2 (p < 0.05) in normoglycemic dogs and from 7.8 +/- 0.1 to 7.6 +/- 0.1 (p < 0.05) in hyperglycemic animals, indicating similar and significant rightward shifts of the dose-response relationship to ACh after CPB in both groups. Neither hyperglycemia nor CPB altered the vasoconstrictor response to Phe. CONCLUSIONS: The reduction in ACh-mediated vasorelaxation after CPB did not differ between hyperglycemic and normoglycemic animals, indicating that hyperglycemia does not contribute to impaired vasorelaxation after CPB. Because Phe-induced vasoconstriction was unaffected, hyperglycemia during hypothermic CPB does not appear to increase the potential for postbypass vasospasm.

Incidence of post-transfusion hepatitis in Taiwan before and after introduction of anti-HCV testing.

The effectiveness of second-generation anti-hepatitis C virus antibody (anti-HCV) screening of blood donations for the prevention of non-A, non-B post-transfusion hepatitis (NANB PTH) was assessed. A prospective study of 192 transfusion recipients was performed to compare the incidence of NANB PTH after the introduction of the second-generation anti-HCV test with the incidence before its introduction. We used a polymerase chain reaction to detect HCV-RNA and HBV-DNA in the sera of patients with NANB PTH. The incidence of acute post-transfusion hepatitis C was 11% (8 of 71) before the screening for anti-HCV as compared with 2.5% (3 of 121) after the screening (p < 0.05). Viremia was detected within the first five weeks of infection in 10 patients with acute post-transfusion hepatitis C. However, there was no significant difference in the incidence of non-A, non-B, non-C (NANBNC) PTH before screening (3 of 71, 4.2%) compared with after screening (3 of 121, 2.5%). Usually, NANBNC PTH was not clinically important. Anti-HCV screening of blood donors significantly reduces the incidence of post-transfusion hepatitis C, but not the incidence of NANBNC PTH.

Cordyceps sinensis as an immunomodulatory agent.

Effects of various fractions of methanol extracts from fruiting bodies of Cordyceps sinensis on the lymphoproliferative response, natural killer (NK) cell activity, and phytohemagglutinin (PHA) stimulated interleukin-2 (IL-2) and tumor necrosis factor-alpha (TNF-alpha) production on human mononuclear cells (HMNC) were studied. Two of the 15 column fractions (CS-36-39 and CS-48-51) significantly inhibited the blastogenesis response (IC50 = 71.0 +/- 3.0 and 21.7 +/- 2.0 micrograms/ml, respectively), NK cell activity (IC50 = 25.0 +/- 2.5 and 12.9 +/- 5.8 micrograms/ml, respectively) and IL-2 production of HMNC stimulated by PHA (IC50 = 9.6 +/- 2.3 and 5.5 +/- 1.6 micrograms/ml, respectively). TNF-alpha production in HMNC cultures was also blocked by CS-36-39 and CS-48-51 (IC50 = 2.7 +/- 1.0 and 12.5 +/- 3.8 micrograms/ml, respectively). These results indicated that neither CS-36-39 nor CS-48-51 was cytotoxic on HMNC, and that immunosuppressive ingredients are contained in Cordyceps sinensis.

Volume expansion increases right ventricular infarct size in dogs by reducing collateral perfusion.

STUDY OBJECTIVE: Plasma volume expansion is frequently recommended to correct the low output state resulting from right ventricular (RV) infarction. However, any subsequent increase in pericardial and RV filling pressures from volume expansion could impair RV collateral blood flow. We examined whether volume expansion in dogs before right coronary ligation reduced collateral perfusion and worsened the extent of RV necrosis. DESIGN: Randomized experimental study. SETTING: Animal research laboratory in university medical center. PARTICIPANTS: Forty anesthetized, closed-chest dogs were randomly assigned to normovolemic, pericardium opened (n = 10) or intact (n = 10) groups, and hypervolemic, pericardium opened (n = 10) or intact (n = 10) groups. INTERVENTIONS: Hypervolemic animals received 24 mL/kg of 6% hetastarch. All animals underwent 90 min right coronary ligation, followed by 120 min reperfusion. Collateral coronary blood flow (radioactive microspheres) and area of necrosis (An) were determined in the area at risk (Ar). MEASUREMENTS AND RESULTS: Stroke volume decreased in all groups with ischemia but remained 25 to 40% greater in both hypervolemic groups than in normovolemic animals (p < 0.05). In hypervolemic animals with intact pericardium, RV end-diastolic pressure increased to 10.4 +/- 2.1 mm Hg (mean +/- SD), a value that significantly exceeded those of the other three groups. During RV ischemia, collateral perfusion in the Ar was similar in both normovolemic groups and in hypervolemic animals with opened pericardium (mean range, 12.9 +/- 8.8 to 13.8 +/- 7.6 mL/min/100 g; p = NS), and the An/Ar varied from 11.8 +/- 6.3 to 18.6 +/- 17.4% (p = NS). In contrast, in hypervolemic animals with intact pericardium, collateral perfusion decreased to 7.2 +/- 3.5 mL/min/100 g and the An/Ar was increased to 38.2 +/- 18.6% (p < 0.05 compared with other groups, respectively). Overall, An/Ar was inversely related to collateral blood flow in the Ar (r = -0.46; p < 0.05) and correlated positively with RV end-diastolic pressure (r = 0.61; p < 0.05). CONCLUSIONS: Volume expansion preserved stroke volume during RV ischemia, independent of pericardial integrity. However, volume expansion in animals with an intact pericardium increased RV infarct size by twofold to threefold secondary to reduced periischemic collateral perfusion. This detrimental effect of volume expansion on infarct size was prevented by opening the pericardium.

Technical note: optimization of breed composition to maximize net merit of synthetic populations.

Determination of optimal breed (strain or line) composition is important when synthesizing a new population. In this study, the relative proportions of different breed contributions to a synthetic population are derived so that net merit of the resulting synthetic population is at a maximum. Net merit (aggregate genotype) is defined as a linear combination of genotypic values weighted by economic values. Two methods of optimizing breed compositions for multiple traits were derived. The first method was component optimization, which is an extension of a previously derived method for a single trait. A numerical example was given to illustrate the computation. The second method was index optimization, derived from a base index that is defined as a linear combination of phenotypes weighted by the economic values. The two procedures were shown to be equivalent.

Association between milk protein genetic variants and genetic values of Canadian Holstein bulls for milk yield traits.

The kappa-casein, beta-casein, and beta-lactoglobulin genotypes of 566 bulls were identified by polymerase chain reaction. The associations of these milk protein types with the daughter yield deviations of the 566 bulls for milk, fat, and protein were examined with a mixed model analysis using an animal model. The results suggest that kappa-casein and beta-casein loci had no significant effects on ETA of the bulls for milk, fat, and protein. The effect of beta-lactoglobulin on ETA of protein approached significance. Results suggest that increasing the frequency of the A variant of beta-lactoglobulin in young bulls entering progeny testing improves ETA for protein of the bulls, but the expected improvement is limited.

Clinical analysis of choledochoduodenal fistula with cholelithiasis in Taiwan: assessment by endoscopic retrograde cholangiopancreatography.

OBJECTIVES: Choledochoduodenal fistula (CDF) is occasionally found during endoscopic retrograde cholangiopancreatography (ERCP). Cholelithiasis is suspected to be the leading cause in some endemic areas. We focus on this cause of CDF to determine which clinical characteristics are relevant to formation of fistulas and to learn whether CDF of various types would imply different clinical significance. METHODS: In 1882 ERCP studies from 1988 to 1993, we found 27 CDF with cholelithiasis in 1066 patients. Their clinical backgrounds and ERCP findings were compared with those of 492 patients who had cholelithiasis but no CDF. RESULTS: The prevalence of CDF was 2.53%. A longer past history of biliary stones, recurrent biliary tract infection (BTI), and the presence of common bile duct stones (CBS) were factors relevant to the formation of fistula. In the case of 24 distal fistulas, including seven of type I and 17 of type II, there was concurrent distal CBS. Three cardinal features of fistula of the distal type were: 1) the length of CDF was less than 1.5 cm, 2) its orifice was just around or on the papillary fold, and 3) all cases of distal type II had prominent pneumobilia, less jaundice, and larger CBS than type I. Aggressive endoscopic or surgical treatment of distal type CDF decreased the recurrence of BTI, as indicated by surveillance for 1 yr. Three fistulas of the proximal type were longer and drained into the duodenum far from the papilla. All of these cases deserved early surgical intervention. CONCLUSIONS: CDF really serves as a chronic sequel of cholelithiasis. Different clinical features of CDF of various types help one to establish diagnosis and treatment. To avoid recurrence of BTI, aggressive therapy to correct CDF is mandatory.

Long-term outcome of triple therapy in Helicobacter pylori-related nonulcer dyspepsia: a prospective controlled assessment.

OBJECTIVE: To ascertain whether triple therapy alters the history of Helicobacter pylori (HP)-related nonulcer dyspepsia (NUD). METHODS: Forty-one young (<45 yr) dyspeptic patients were confirmed to be HP-related NUD by serology, rapid urease test, and antral biopsy. Endoscopy excluded the presence of ulcer. These cases were randomly plotted into control (n = 21) and triple therapy (n = 20) groups. In the former group, H2 blocker was given for 2 months and then intermittent antisecretory agents for up to 1 yr. In the latter group, 20 patients received a course of triple therapy and then were treated like the control group. The symptom scores (range: 0-10) were collected on enrollment, and at the end of 2nd, 6th, and 12th months. Each case had serial tests of HP IgG ELISA titer on start, at weeks 2, 4, and 8, at the 6th month, and at the end of the 1st yr. The second endoscopy was done in the 9th wk for eradication survey, and the third endoscopy, at the end of the 1st yr to resurvey the HP status. The histological gradings of biopsy specimens, sampled on each endoscopy, were compared. RESULTS: In the triple therapy group, the rate of eradication of HP was 75%. At he end of the 2nd month, the HP-eradicated cases of the triple therapy group improved the symptom score more significantly then the noneradicated cases (2.42+/-1.37 vs.4.76+/-1.58, p <0.001). At the ends of the 6th month and 1st yr, the symptom scores of the eradicated cases improved more significantly than those of the control group (6th month, 0.61+/-1.18 vs.] 2.66+/-2.06; 1st yr, 0.82+/-1.17 vs.] 3.56+/-2.89, p <0.001). The decline trend of ELISA titers occurred only in eradicated cases and became significantly evident from the 4th wk (0.30+/-0.15 vs.] 0.49+/-0.07, p <0.05) and thereafter. Both acute and chronic pathological grading was improved in the triple group at the end of the 1st yr (acute, 1.95-0.46; chronic, 1.9-0.92; p <0,01) CONCLUSION: Compared with control therapy at 1 yr, triple therapy showed greater symptomatic, serological, and histological improvements. Therefore, triple therapy is beneficial to symptomatic HP-related NUD.

Technetium-99m-DMSA renal SPECT in diagnosing and monitoring pediatric acute pyelonephritis.

UNLABELLED: This study compares the sensitivity of 99mTc-dimercaptosuccinic acid (DMSA) renal SPECT with planar scintigraphy, concluding the importance of 99mTc-DMSA renal SPECT for the early diagnosis of acute pyelonephritis (APN) in patients under 3 yr of age. METHODS: Twenty-seven children under 3 yr of age, with clinical and/or laboratory suspicion of APN, were investigated. All 99mTc-DMSA renal SPECT and planar images and voiding cystoureterogram (VCUG) were obtained within 3 days of hospitalization. RESULTS: In the first examination, renal cortical defects were detected in 23 patients (42 kidneys) with SPECT and in 9 patients (11 kidneys) with planar scintigraphy. One year after treatment, constant renal cortical lesions were observed in 11 patients (14 kidneys) with SPECT and 4 patients (4 kidneys) with planar scintigraphy. The high grades of vesicoureteral reflux (VUR) (grade >or= 3) correlate better with APN diagnosed by SPECT (34 kidneys) than by planar scintigraphy (8 kidneys). Multiple renal cortical defects (number of lesions >or= 4) were only seen in patients under 1.5 yr old and none of those with a negative 99mTc-DMSA renal SPECT had a positive 99mTc-DMSA renal planar scintigraphy at any time. There is a significant difference (p < 0.05) between the diagnostic ability of these two methods of examination. CONCLUSION: Our results suggest that 99mTc-DMSA renal SPECT should be used, where possible, instead of planar DMSA in routine examination of children with clinical suspicion of APN, especially for those under 3 yr of age.

Esterification of oxysterols in human serum: effects on distribution and cellular uptake.

Oxysterols, oxidized derivatives of cholesterol, may enter the circulation as contaminants of cholesterol-containing food, arise through peroxidation of lipoproteins, or be generated by enzymatic reactions. They are found in serum associated either with lipoproteins or with albumin. In these studies, 25-hydroxycholesterol (25OHC) was used as a model oxysterol to investigate the effect of esterification on the association of oxysterols with serum components and their delivery to cultured cells. 25OHC added in vitro to fresh human serum was readily esterified during incubation at 37 degrees C, most likely by serum lecithin:cholesterol acyltransferase (LCAT) as it was blocked by known inhibitors of LCAT. The 25OHC-esters formed were identified as monoesters by comparing their elution on high performance liquid chromatography and thin-layer chromatography with that of chemically synthesized 25OHC mono- and diesters. Esterification doubled the percentage of 25OHC associated with lipoproteins, concomitantly decreasing the amount associated with albumin. Whereas unesterified 25OHC readily transferred between isolated lipoproteins, 25OHC-esters remained associated with donor lipoproteins unless human lipoprotein-deficient serum was added. That cholesteryl ester transfer protein (CETP) mediated transfer of 25OHC-esters was demonstrated by the ineffectiveness of rat lipoprotein-deficient serum as well as by the ability of IC-4, an anti-CETP monoclonal antibody, to suppress the transfer. Esterification of 25OHC in serum limited its entry into human erythrocytes and fibroblasts (GM 3468A cells) in vitro. Up-regulation of fibroblast low density lipoprotein (LDL)-receptors enhanced the uptake of esterified 25OHC from medium, but did little to enhance the total uptake of 25OHC. Thus, esterification of oxysterols in serum shifts their distribution away from albumin into LDL and high density lipoprotein (HDL) and limits their availability to cells in culture.

Ectopic locations of Taenia saginata asiatica cysticerci in the abdominal cavity of domestic pig and monkey.

Eleven Small-Ear-Miniature pigs, 1 Landrace Small-Ear-Miniature pig, and 1 monkey (Macaca cyclopis) were infected experimentally with eggs of Taenia saginata asiatica. They were found to harbor cysticerci on the liver surface, each with a thin, whitish fiber connecting to another location in the same lobe, or to other lobes of the liver; some were also connected individually to the diaphragm, abdominal muscles, and gall bladder with a short, thin fiber. Moreover, they also harbored extrahepatic cysticerci. Most of these were observed on the omentum and in the peritoneal cavity, except for some suspended individually in the abdominal cavity by a long, thin, whitish fiber. The origin of the extrahepatic cysticerci and whitish fibers is probably from the surface of the liver of the host.

Cloning and characterization of a cDNA encoding an 18.0-kDa class-I low-molecular-weight heat-shock protein from rice.

A novel cDNA clone, Oshp18.0 cDNA, encoding a rice (Oryza sativa L. cv. Tainong 67) 18.0-kDa heat-shock protein (HSP), was isolated from a cDNA library of heat-shocked rice seedlings by use of the rice HSP cDNA, Oshsp17.3 cDNA, as a probe. The sequence showed that Oshsp18.0 cDNA contains a 749-bp insert encoding an ORF of 160 amino acids, with a predicted molecular mass of 18.0 kDa and a pI of 7.3. Sequence comparison reveals that Oshsp18.0 cDNA is highly homologous to other low-molecular-weight (LMW) HSP cDNAs. Also, the results of hybrid-selected in vitro translation clearly establish that Oshsp18.0 cDNA is the rice 18.0-kDa LMW HSP-encoding cDNA clone. The recombinant Oshsp18.0 fusion protein produced in Escherichia coli was of the size predicted, and was recognized by the class-I rice 16.9-kDa HSP antiserum. The results suggest that Oshsp18.0 cDNA is an 18.0-kDa class-I LMW HSP- encoding cDNA clone from rice.

Differential expression of cytokine genes in cervical cancer tissues.

The expression of genes coding for inflammatory cytokine interleukin-1-alpha (IL-1 alpha), IL-6, interferon-gamma and tumor necrosis factor-alpha from 15 normal cervix, 11 cervical intraepithelial neoplasia and 13 cervical cancer tissues was investigated. The cytokine messenger ribonucleic acids were reverse transcribed and amplified in the presence of biotinylated and dinitrophenylated primers. Amplified DNA was then captured onto streptavidin-coated microwell plate and quantitatively measured in a colorimetric reaction using ant-DNP antibodies conjugated to horse radish peroxidase. There is no change of IL-1 alpha, IL-6 and tumor necrosis factor-alpha gene expression in either cervical intraepithelial neoplasia or cervical cancer tissues. But the transcription of interferon-gamma gene is significantly reduced in both cervical intraepithelial neoplasia and cervical cancer tissue as compared to normal cervix. This study demonstrated that reverse transcription and quantitative polymerase chain reaction coupling to colorimetric microwell plate assay is a sensitive and useful method to quantitate multiple cytokine gene expression. Our results also suggest that cervical epithelial cells are capable to express cytokines and that interferon-gamma may play a role in the pathogenesis of cervical cancer since its reduced expression may influence inflammation and immunity of the cervical tissues.

Identification of a de novo point mutation resulting in infantile form of Pompe's disease.

One patient in a nonconsanguineous Taiwanese family with infantile glycogen storage disease type II (Pompe's) disease was found to have a de novo mutation of G1933 to C transition [corrected] in exon 14 of the human lysosomal alpha-D-glucosidase gene. Patient was homozygous and both parents were heterozygous for the mutant allele. The mutation caused an Asp to His substitution at amino acid position 645. The mutation was introduced in wild type lysosomal alpha-D-glucosidase cDNA and the mutant construct was expressed in vivo. The Glu to His substitution was proven to cause significant loss of enzyme activity. In homozygous form it leads to the severe infantile phenotype of Pompe's disease.
