Assessing damage to the respiratory tract following thermal injury.

Physicians may institute respiratory therapy at several points along the course of treatment of the burn patient. In the emergency room, they may have to introduce an airway before they can transport the patient to a burn unit. Physicians may have to administer oxygen or place the patient on mechanical ventilation to reverse ventilatory failure, start treatment of the damage to the airways, or prevent or respond to respiratory complications. Any time a physician must choose respiratory therapy, he must base his decision on accurate assessments of the upper and lower airways.

Pericarditis with effusion casued by Actinomyces israelii.

Actinomyces israelii is a rare cause of pericarditis. Infection spreads to pericardium from an intrathoracic focus. Branching gram-positive filaments are seen on microscopic examination of clinical specimens. Cultures should be performed under anaerobic conditions. Both surgical drainage and antibiotics (penicillin) are required for treatment. Nocardiosis may resemble actinomycosis in many respects.

Assignment of the imidazole ring nitrogen protons of histidine 48 in the proton NMR spectrum of ribonuclease A in water solution.

Several exchangeable resonances, designated a, b, c and d are observed in the 11-14 ppm (from 2,2-dimethyl-2-silapentane-5-sulfonate) region of the proton spectrum of ribonuclease A in water solution. We describe a number of lines of evidence suggesting the assignment of peaks b and c to the N1 and N3 protons of His 48, which occupies an interior position in the protein remote from the active site. This evidence includes the observation that the binding of Cu(II) and 3'-CMP (cytidine 3'-monophosphate) has no effect on these resonances. Further evidence includes pH titration data showing a pKa of approx. 2 for these protons, solvent exchange rates in the native state and with disulfide bridges IV-V and III-VIII cleaved, the observation of the carboxymethylated enzymes CM-His12-RNAase A and CM-His119-RNAase A, and of the modified enzymes Des(1-21)-RNAase A (S-protein) and Des(119-124)-RNAase A.

The impact of personal and situational variables on career patterns among nurses from three types of educational programs.

Relationships between educational preparation and employment choices were investigated in a study of the career paths of nursing graduates from three types of educational programs: a community college diploma program, a generic baccalaureate program, and a postdiploma baccalaureate program. Selected personal and work-related correlates of career paths for the three groups also were explored. All available graduates for the years 1984 to 1990 from the three programs (n = 1,544) were sent a self-administered questionnaire. Data were collected concerning demographic variables, employment details, ongoing professional development, and level of satisfaction with nursing. Analysis of the data demonstrated clear variations in the career paths of the three groups of graduates, supporting the belief that the type of educational program does indeed have a steering effect on the subsequent career choices of these nurses.

Toward new designed proteins derived from bovine pancreatic trypsin inhibitor (BPTI): covalent cross-linking of two 'core modules' by oxime-forming ligation.

A 25-residue disulfide-cross-linked peptide, termed 'oxidized core module' (OxCM), that includes essentially all of the secondary structural elements of bovine pancreatic trypsin inhibitor (BPTI) most refractory to hydrogen exchange, was shown previously to favor nativelike beta-sheet structure [Carulla, N., Woodward, C., and Barany, G. (2000) Synthesis and Characterization of a beta-Hairpin Peptide That Represents a 'Core Module' of Bovine Pancreatic Trypsin Inhibitor (BPTI). Biochemistry 39, 7927-7937]. The present work prepares to explore the hypothesis that the energies of nativelike conformations, relative to other possible conformations, could be decreased further by covalent linkage of two OxCMs. Optimized syntheses of six approximately 50-residue OxCM dimers are reported herein, featuring appropriate monomer modifications followed by oxime-forming ligation chemistry to create covalent cross-links at various positions and with differing lengths. Several side reactions were recognized through this work, and modified procedures to lessen or mitigate their occurrence were developed. Particularly noteworthy, guanidine or urea denaturants that were included as peptide-solubilizing components for some reaction mixtures were proven to form adducts with glyoxylyl moieties, thus affecting rates and outcomes. All six synthetic OxCM dimers were characterized by 1D (1)H NMR; three of them showed considerable chemical shift dispersion suggestive of self-association and mutual stabilization between the monomer units.

How reliable are selected scales from population-based health surveys? An analysis among seniors.

The objective of the study was to assess the internal consistency and test-retest reliability of selected scales from four national and provincial surveys used to study the health status of community-dwelling seniors. Items on physical impairment, psychological well-being, activities of daily living, chronic health problems, social support, and stressful life events were selected from these surveys and administered in a group of 1,054 seniors, aged 65 and over, living in Wellington County, Ontario. Each scale exhibited high internal consistency (Cronbach's alpha > or = 0.70) except for the stressful life events and social support scales. The intra-class coefficients (rho) for all scales were > 0.80. In addition, almost all single items measures used in this study also exhibited high test-retest reliability (kappa > 0.80) except for measures of perceived health status (kappa = 0.69) and availability of help in crisis situation (kappa = 0.48).

Solid-phase synthesis of bovine pancreatic trypsin inhibitor (BPTI) and two analogues. A chemical approach for evaluating the role of disulfide bridges in protein folding and stability.

The linear sequence of bovine pancreatic trypsin inhibitor (BPTI) has been assembled by stepwise Fmoc solid-phase peptide synthesis on a polyethylene glycol-polystyrene (PEG-PS) graft support with p-alkoxybenzyl ester anchoring. Similar methods were used to prepare two analogues, the first with all six half-cystine (Cys) residues replaced by alpha-amino-n-butyric acid (Abu), and the second with replacement of Abu at four Cys positions while retaining the native pairing between positions 14 and 38. Following cleavage from the support, the linear molecules (reduced form) were purified by semipreparative reversed-phase high performance liquid chromatography (HPLC). The native structure of BPTI was then formed by oxidation of a dilute solution of the protein at pH 8.7 in the presence of oxidized glutathione. The BPTI analogue with one disulfide bridge was obtained following treatment with dimethyl sulfoxide (DMSO)-pH 6 buffer (1:9). Overall yields of homogeneous proteins were 2-4%, and further characterization was provided by amino acid analysis, sequencing, ion electrospray mass spectrometry, analytical HPLC, and capillary zone electrophoresis (CZE). Purified synthetic BPTI with the native sequence was indistinguishable from natural material by the analytical and biophysical criteria applied, including circular dichroism (CD) spectra and inhibition of trypsin action. Studies are in progress to evaluate conformational features of the analogues which respectively lack two, or all three, of the native disulfide bridges.

HIV testing practices of primary care physicians: an Ontario survey.

To determine whether physicians are knowledgeable about the risk factors associated with HIV and whether perceived testing practices coincide with testing guidelines, 1,236 physicians were surveyed about which patients they would recommend for HIV testing. A total of 480 usable responses were obtained (response rate of 50% of the eligible sample). Most would suggest testing to partners of intravenous drug users, patients who received blood transfusions between 1978 and 1985, homosexuals and bisexuals, and patients with more than two sexually transmitted diseases. Only 46.4% would suggest testing to patients with more than two sexual partners, and 65.8% would test anyone who asks. Other patients to whom physicians would recommend testing were listed by 16.2%. Logistic regression (F-step) was used to examine the relationship between physician characteristics and likelihood of recommending testing. As a rule, physicians are knowledgeable about the major risk factors and would recommend testing according to guidelines. However, there may be a difference between questionnaire responses and actual practice.
